Paraspinal subfascial placement of lumbar intrathecal baclofen catheters: short-term outcomes of a novel technique.

OBJECT: Techniques for the placement of intrathecal baclofen (ITB) systems have been described in detail, with special consideration given to complications from hardware placement. Risks including catheter kinking and migration, hardware erosion through the skin, and lumbar CSF leak are elevated given the often-low body mass index and poor nutritional status of this patient population. The bulk of a spinal catheter and fascial connector within the lumbar wound may increase the potential for the aforementioned risks, leading to potential risks for wound infection and breakdown. The authors' exp

Midterm Observational Study Results After Glenohumeral Arthrodesis With a Locking Compression Plate in Paralytic Shoulder.

PURPOSE: To evaluate bone union, hardware removal, complications, and functional outcomes after glenohumeral arthrodesis using a locking compression plate positioned beneath the acromion in patients with paralytic shoulder. METHODS: A retrospective observational study was performed at a tertiary referral center. Patients who underwent glenohumeral arthrodesis with a locking compression plate between 2011 and 2025 were included. Demographic data, etiology of paralysis, prior treatments, complications, and need for secondary surgery were recorded. Radiographic union was assessed during follow-u…

A systematic review examining the clinical evidence for the elbow internal joint stabilizer and associated complications.

PURPOSE: Elbow instability continues to be a challenging entity to manage in orthopedics due to the complex anatomy of the elbow joint. Historically, common management techniques involved ulnohumeral cross pinning, spanning plates or external fixation which can lead to complications such as infection, joint incongruity, or stiffness amongst others. More recently, an internal stabilizer has been introduced to allow early elbow range of motion while stabilizing the elbow. Our review aims to examine the literature pertaining to the internal joint stabilizer (IJS) with a focus on its safety profil

Risk factors and management of Nuss bar infections in 1717 patients over 25 years.

PURPOSE: An increase in postoperative infections after Nuss procedures led us to seek risks and review management. We report potential risk factors and make inferences for prevention of infections. METHODS: An IRB-approved retrospective chart review was used to evaluate demographic, clinical, surgical, and postoperative variables of patients operated on between 10/1/2005 and 6/30/2013. Those with postoperative infection were evaluated for infection characteristics, management, and outcomes with univariate analyses. RESULTS: Over this 8-year period (2005-2013), 3.5% (30) of 854 patients devel

Determining the efficacy of screw and washer fixation as a method for securing olecranon osteotomies used in the surgical management of intraarticular distal humerus fractures.

OBJECTIVES: The purpose of this study was to critically evaluate the efficacy of single screw and washer fixation in comparison with other methods for securing olecranon osteotomies. The hypothesis is that screw and washer fixation is a safe and effective means of olecranon osteotomy fixation with fusion and complication rates similar to other methods of fixation. DESIGN: Retrospective review. SETTING: Two Level I Urban Trauma Centers. PATIENTS/PARTICIPANTS: Patients were treated within the last 20 years and received 1 of 4 types of fixation (screw and washer alone, screw and washer augment

Complications After Tibial Tuberosity Osteotomy: Association With Screw Size and Concomitant Distalization.

BACKGROUND: Tibial tuberosity osteotomy (TTO) is a versatile procedure commonly used to treat patellar instability as well as to unload cartilage lesions. TTO with concomitant distalization (TTO-d) may be performed in patients with patella alta to stabilize the patella by helping it to engage in the trochlea earlier during flexion. PURPOSE: To identify and compare perioperative complications in patients who underwent TTO and those who underwent TTO-d and to analyze risk factors associated with these complications. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: We retrospectively

Long-term functional and radiographic outcomes after removal of radial head arthroplasty.

PURPOSE: If reconstruction of the radial head using osteosynthesis is not possible, radial head arthroplasty (RHA) is performed. Despite the increasing use of RHA, little reliable data is available on the functional and clinical outcomes following removal of RHA in a mid- to long-term follow-up. METHODS: This study included 27 patients who underwent removal of RHA between 2010 and 2021. The mean follow-up period was 8.8 years (2.0 to 14.2 years). The time to removal, as well as functional elbow scores (Mayo Elbow Performance Score (MEPS), Oxford Elbow Score (OES) and Disabilities of arm, shou

Retrospective Review of Complications and Revision Rates Between Isolated Talonavicular vs Talonavicular and Subtalar (Double) Arthrodesis vs Triple Arthrodesis.

BACKGROUND: Hindfoot fusion procedures are common for the treatment of end-stage arthritis or deformity. Surgical treatments for these conditions include talonavicular joint (single) arthrodesis, talonavicular and subtalar (double) arthrodesis, or talonavicular, subtalar, and calcaneocuboid (triple) arthrodesis. This study evaluated the complication rate, revision surgery rate, and hardware removal rate for those treated with either single, double, or triple arthrodesis. METHODS: A retrospective review was conducted for patients who underwent single (Current Procedural Terminology [CPT] code …

Clinical Outcomes of AO/OTA Type C Distal Radius Fractures Treated With Volar Locking Plate Versus Fragment-Specific Plating.

PURPOSE: Distal radius fractures remain one of the most common adult fractures, with increasing operative fixation rates. Volar locking plat … nt-specific fixation was associated with longer operative times and higher rates of reoperation for hardware removal but did not appear to increase rates of major adverse postoperative clinical outcomes when compared with VLP. TYPE OF STUDY/LEVEL OF EVIDENCE: Therapeutic III.

Comparative Analysis of Isolated Nitinol Staple Fixation vs Staple Plus Screw Fixation In Midfoot and Hindfoot Arthrodesis.

BACKGROUND: Nitinol staples are increasingly used in orthopaedic surgery for arthrodesis because of their superelasticity properties, which … ignificantly higher joint revision rates compared with staples alone, most often due to symptomatic hardware removal. CONCLUSION: In our series, nitinol staples used in isolation or alongside static screws resulted in nearly identical fusion rates across the foot. However, the addition of a static screw to a nitinol staple significantly increases the revision sur …

The Rate of Major Complications Following Distal Radial Fractures Treated With One Specific Volar Locking Plate: A Retrospective Study of 1,597 Consecutive Cases in 1,564 Patients.

PURPOSE: Current knowledge of complication rates after volar plating of distal radius fractures mainly relies on studies of low to moderate numbers and various implants. This study sought to find the incidence of complications leading to reoperation in a sample of distal radius fractures treated with one specific volar locking plate (VLP). METHODS: We retrospectively evaluated 1,597 distal radius fractures in 1,564 patients operated with a VLP from January 2011 to December 2017 for complications leading to a reoperation. We considered any reoperation a major complication, except for carpal tu
