Ventricular assist devices and sleep-disordered breathing.

Congestive heart failure is one of the leading causes of morbidity and mortality in the United States, and left ventricular assist devices have revolutionized treatment of end-stage heart failure. Given that sleep apnea results in significant morbidity in these patients with advanced heart failure, practicing sleep physicians need to have an understanding of left ventricular assist devices. In this review, we summarize what is known about ventricular assist devices as they relate to sleep medicine.

Neurologic complications of implantable devices.

Technologies for repairing cardiac structures or sustaining cardiac function with implantable devices have helped patients with an ever-expanding array of cardiac conditions. Patients are surviving and thriving with cardiac conditions that would formerly have been disabling or fatal. With the implantation of devices in the heart, however, comes the inevitable risk of neurological complications. This chapter focuses on devices implanted in the chambers or valves of the heart itself, including prosthetic heart valves, closure devices for patent foramen ovale, atrial appendage occluder devices, s

Impact of adverse events on ventricular assist device outcomes.

Left ventricular assist devices have been proven to be superior to medical therapy for advanced heart failure patients awaiting heart transplantation and viable alternatives to transplantation for destination therapy patients. Improvements in the design of ventricular assist devices have been rewarded by a decrease in adverse events and an increase in survival. Despite significant progress, even the latest generation left ventricular assist devices are burdened by a significant long-term adverse events profile that will increasingly challenge physicians as patients survive longer on implantabl

Assessment of Coagulation and Hemostasis Biomarkers in a Subset of Patients With Chronic Cardiovascular Disease.

Measurement of a single marker of coagulation may not provide a complete picture of hemostasis activation and fibrinolysis in patients with chronic cardiovascular diseases. We assessed retrospective orders of a panel which included prothrombin fragment 1.2 (PF1.2), thrombin: antithrombin complexes, fibrin monomers, and D-dimers in patients with heart assist devices, cardiomyopathies, atrial fibrillation and intracardiac thrombosis (based on ordering ICD-10 codes). During 1 year there were 117 panels from 81 patients. Fifty-six (69%) patients had heart assist devices, cardiomyopathy was present

Heart transplantation in perspective.

Heart disease remains one of the leading causes of death in the western world. In the 35 years since the first human heart transplants, cardiac transplantation has become established as the therapeutic option of choice in the management of terminal cardiac failure. Since 1981, the introduction of cyclosporin for immunosuppression has dramatically increased cardiac transplantation. However, several obstacles limit further utilization, including limited availability of donor hearts, limited ischemic time tolerated by donor hearts, and chronic rejection. Research is underway into donor heart pres

End-stage heart failure with multiple intracardiac thrombi: a rescue strategy.

The use of ventricular assist devices as a bridge to transplantation has become a widely used option for patients with end-stage heart failure. In contrast to total artificial hearts, ventricular assist devices support the failing heart by bypassing one or both ventricles. In certain cases (myocardial tumors, graft failure, transplant rejection, endocarditis, intracardiac thrombus formation), however, it may be advantageous to excise the heart and replace it with an artificial device. Total artificial hearts are intracorporeal devices designed for this purpose. Unfortunately, some patients are

Left ventricular assist device in pregnancy.

BACKGROUND: : Left ventricular assist devices have become increasingly common for the treatment of acute and chronic heart failure. It is unclear whether these devices can handle the increased stresses caused by pregnancy. CASE: : An 18-year-old woman received a left ventricular assist device after peripartum cardiomyopathy. The patient was not adherent to prescribed medical regimens until she returned 11 months later, at 33 weeks of gestation. She underwent anticoagulation and inpatient surveillance until a cesarean delivery at approximately 34 weeks (based on ultrasound estimates). CONCLUS

Left ventricular assist device as a bridge to partial left ventriculectomy.

In 1963, Dr Michael DeBakey successfully implanted the first left ventricular assist device. Throughout the 1970s, cardiovascular research aimed to achieve long-term circulatory support with such devices; later, as improved medical therapy decreased patients' chances of organ rejection or infection, transplantation became a viable alternative for the treatment of heart failure. At that point, left ventricular assist devices began to be used as bridges to transplantation. As left ventricular assist devices were used for longer periods of time, we realized that the ventricular function and overa

Anaesthesia for patients undergoing ventricular assist-device implantation.

In the last 10 years, implantation of ventricular-assist devices has become an interesting option as either bridge-to-transplantation or destination procedure for patients with end-stage congestive heart failure. In the future, the number of ventricular assist device implantations is expected to increase furthermore. In general, this patient cohort is associated with significant co-morbidities, for example, pulmonary hypertension, peripheral vascular disease and renal insufficiency. Anaesthetic management for implantation of ventricular assist devices can be challenging for cardiac anaesthesio

High-dose carvedilol therapy for mechanical circulatory assisted patients.

Left ventricular assist devices are increasingly being used as a "bridge to recovery" in the clinical arena. However, the percentage of patients who can be weaned off these devices is still low. Carvedilol is an anti-heart failure agent that is used to induce cardiac reverse remodeling, and its effect is reportedly dose-dependent. In this study, we conducted high-dose carvedilol therapy (80 mg daily) in five patients who wore left ventricular assist devices. Three of the five patients showed an increase in cardiac function after this therapy, and one patient was successfully weaned off his dev

Stroke epidemiology and outcomes in the modern era of left ventricular assist devices.

The care for the patients with end-stage heart failure has been revolutionized by the introduction of durable left ventricular assist devices, providing a substantial improvement in patient survival and quality of life and an alternative to heart transplantation. The newest devices have lower instances of mechanical dysfunction and associated pump thrombosis. Despite these improvements in complications, the use of continuous flow assist devices is still associated with high rates of thrombotic and hemorrhagic complications, most notably stroke in approximately 10% of continuous flow assist dev

Assist devices fail to reverse patterns of fetal gene expression despite beta-blockers.

BACKGROUND: Heart failure is associated with reversal to a fetal gene expression pattern of contractile and metabolic genes. Substantial recovery of ventricular function with assist devices is rare. Our goal was to evaluate the effects of assist devices on fetal gene expression and hypoxia inducible factor-1 alpha (HIF-1 alpha), a transcriptional factor in hypoxic signaling. METHODS: Human heart tissue was obtained from the left ventricular apex at the time of assist device implantation and again from the left ventricular free wall during cardiac transplantation. Non-failing tissue was obtain

[Driveline Wound Care: Key to the Long-Term Use of Implantable Ventricular Assist Devices].

Heart failure is a complicated chronic disease. Although numerous new medications are used to treat and improve the quality of life of patients with heart failure, the final treatment for terminal heart failure still relies heavily on heart transplants. However, as the number of heart donations is limited, many health institutions implant mechanical assist devices to perform heart functions temporarily. These implanted ventricular assist devices help reduce heart failure symptoms and enhance quality of life. However, driveline infection is one of the more severe complications associated with t
