PF4-dependent P-selectin expression assay in comparison to the heparin-induced platelet activation assay for the diagnosis of heparin-induced thrombocytopenia.

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is a severe complication characterized by thrombocytopenia and thrombosis. The presence of antibodies against heparin/platelet factor 4 (PF4) complexes is a key indicator for HIT. Diagnostic laboratory testing generally includes detection of anti-heparin/PF4 and/or functional testing such as the heparin-induced platelet activation assay (HIPAA). However, current functional tests are time-consuming and require specific technical skills. The PF4-dependent P-selectin expression assay (PEA) is a flow cytometry-based test that evaluates activation …

Successful Use of Cangrelor and Heparin for Cardiopulmonary Bypass in a Patient With Heparin-Induced Thrombocytopenia and End-Stage Renal Disease: A Case Report.

Heparin is the only well-established anticoagulant medication for cardiopulmonary bypass making selecting an alternative anticoagulant challenging in patients with heparin-induced thrombocytopenia. Other anticoagulant medications can cause significant postoperative bleeding, especially in patients with end-stage renal disease. We present a case of a 63-year-old woman requiring aortic valve replacement with a history of heparin-induced thrombocytopenia and end-stage renal disease. Cangrelor and heparin were successfully used during cardiopulmonary bypass, offering an option for anticoagulation …

Redo sternotomy for cardiac reoperations using peripheral heparin-bonded cardiopulmonary bypass circuits without systemic heparinization: technique and results.

OBJECTIVE: Cardiac reoperations are challenging and time-consuming and incur a high incidence of perioperative complications because of injuries to cardiac structures, bleeding, and hemodynamic instability. Some centers are using extracorporeal circulation with heparinization at the time of resternotomy, but it leads to prolonged anticoagulation, platelet dysfunction, fibrinolysis, coagulopathy, and morbidity. The authors routinely perform resternotomy in complex surgery with the support of heparinless cardiopulmonary bypass with heparin-bonded circuits (HBCs). The authors describe their techn…

Low-molecular-weight heparin and calcium heparin in thrombosis prophylaxis in patients with percutaneous arterial and venous ports for colorectal liver metastases.

STUDY OBJECTIVE: The evaluation of low-molecular-weight heparin use to prevent arterial and venous thrombosis in patients with indwelling arterial Port-a-Cath implants. METHODS: From 1996 to March 2003 we placed 370 indwelling hepatic arterial catheters with a minimally invasive approach. The left distal subclavian artery was approached from beneath the left clavicle, then an angiographic study of the tumoral vascular district was performed and the gastroduodenal artery was occluded by an embolus. A polyurethane catheter was introduced distally into the hepatic artery and connected to a reser…

Low-Molecular-Weight Heparin Versus Unfractionated Heparin for Venous Thromboembolism Prophylaxis in Adult Traumatic Brain Injury: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) is associated with a high risk of venous thromboembolism (VTE), yet the optimal strategy for thromboprophylaxis remains uncertain. Low-molecular-weight heparin (LMWH) and unfractionated heparin (UFH) are commonly used agents in clinical practice, but their comparative effects in adult TBI have not been formally synthesized. METHODS: Medical databases were searched through March 2026 for studies comparing LMWH and UFH for prophylaxis in adult patients with TBI. Randomized trials and cohort studies reporting at least one thromboembolic, he…

Unfractionated Heparin and Low-Molecular-Weight Heparin in Sepsis: Anti-Inflammatory and Anticoagulant Effects Versus No Anticoagulation-A Propensity Score-Matched Analysis.

BACKGROUND: Given the controversy surrounding anticoagulation therapy in sepsis, this study evaluated the efficacy of unfractionated heparin (UFH) and low-molecular-weight heparin (LMWH) compared with no anticoagulation in sepsis management, considering their potential benefits beyond anticoagulation. METHODS: In this retrospective cohort study, 1362 sepsis patients were stratified into no anticoagulation, UFH, or LMWH groups. Propensity score matching balanced baseline characteristics. UFH was administered intravenously and LMWH subcutaneously at standard doses. Twenty-two biomarkers were as…
