Natural history of Barrett's esophagus.

The natural history of Barrett's esophagus (BE) is difficult to quantify because, by definition, it should describe the course of the condition if left untreated. Pragmatically, we assume that patients with BE will receive symptomatic treatment with acid suppression, usually a proton pump inhibitor, to treat their heartburn. This paper describes the development of complications of stricture, ulcer, dysplasia and adenocarcinoma from this standpoint. Controversies over the definition of BE and its implications in clinical practice are presented. The presence of intestinal metaplasia and its rele…

Natural history of Pott's kyphosis.

The natural history of Pott's kyphosis is different from that of other spinal deformities. After healing of the spinal infection, the post-tubercular kyphosis in adults is static but in children variable progression of the kyphosis is seen. The changes occurring in the spine of children, after the healing of the tubercular lesion, are more significant than the changes that occur during the active stage of infection. During growth, there is a decrease in deformity in 44 % of the children, an increase in deformity in 39 % of the children and no change in deformity in 17 % of the children. The cr…

Natural history of HCV infection.

BACKGROUND: There is much controversy surrounding the natural history of hepatitis C virus (HCV) infection. AIMS: The aim of this review was to review the natural history of HCV infection. METHODS: Published English literature was searched via pubmed and then reviewed. RESULTS: Approximately, 75-85% of HCV-infected persons will progress to chronic HCV infection. The rate of chronic HCV infection is affected by a person's age, gender, race, and viral immune response. Once chronic HCV infection develops, there are external and host factors that can increase the risk of progression of liver di…

Plague: history and contemporary analysis.

Plague has caused ravaging outbreaks, including the Justinian plague and the "black death" in the Middle Ages. The causative agents of these outbreaks have been confirmed using modern molecular tests. The vector of plague during pandemics remains the subject of controversy. Nowadays, plague must be suspected in all areas where plague is endemic in rodents when patients present with adenitis or with pneumonia with a bloody expectorate. Diagnosis is more difficult in the situation of the reemergence of plague, as in Algeria for example, told by the first physician involved in that outbreak (NM).

History of growth hormone therapy.

Although the importance of the pituitary gland for growth was recognized in late 19(th) century, Growth hormone (GH) therapy was made available for severely GH-deficient children and adolescents only in late 1950s. Use of GH for other conditions was limited because of the limited supply of human pituitary-derived hormone. With unlimited availability of recombinant human GH (rhGH), the scenario of GH treatment has been changed enormously. Currently there is ever increasing list of indications of GH treatment in children, adolescents, and adults.

Phosphodiesterase inhibitors: history of pharmacology.

The first pharmacological investigations of phosphodiesterase (PDE) inhibitors were developed with the clinical efficacies of drugs isolated from coffee, cacao and tea but only later their relevant ingredients were identified as xanthines that act as PDE. With its diuretic, inotropic and bronchodilating clinical efficacy, use of theophylline anticipated the clinical goals, which were later approached with the first-generation of weakly selective PDE inhibitors in the period from 1980 to 1990. Pharmacological and clinical research with these early compounds provided a vast pool of information r

Natural history of Danon disease.

BACKGROUND: Danon disease is a rare but serious cardiac and skeletal myopathy leading to substantial morbidity and early mortality due to arrhythmia and cardiomyopathy. The X-linked nature of inheritance accounts for reported differences in phenotypic severity between men and women. The rarity of Danon disease has limited understanding of the complete phenotype. Clinical estimates of ages of disease onset and survival based on gender have not been published. METHODS AND RESULTS: We present data on 82 patients with Danon disease from 36 families, the largest series to date. Men were severely a

Natural history of venous thromboembolism.

Venous thromboembolism (VTE) originates in systemic venous thrombosis and has different etiological mechanisms and natural history from arterial thrombosis. VTE typically originates as deep venous thrombosis in a lower extremity, where it may give rise to acute symptoms “upstream” from the obstructed vein, result in pulmonary embolism, and/or cause chronic venous obstruction. Pulmonary embolism may result in acute respiratory symptoms, cardiovascular collapse and, uncommonly, may also cause chronic disease.

[Natural history of viral hepatitis].

Acute hepatitis B is associated with a risk of evolution to chronic hepatitis in 90% of cases in the newborn, 50% in the childhood and 5 to 10% in adults. During the chronic phase, we usually distinguish 4 phases: the immunotolerance phase with a positive HBe antigen, ALT in the normal value, high viral load and the absence of necro-inflammation; then the phase of immune clearance associated with fluctuations of the viral load and ALT level, HBe antigen seroconversion and histological lesions of chronic hepatitis; at this phase, we distinguish the HBe antigen positive chronic hepatitis B and t

Natural history of syphilitic aortitis.

No large studies of cardiovascular syphilis at necropsy have been reported since 1964. We examined at necropsy 90 patients who had characteristic morphologic findings of syphilitic aortitis. None had ever undergone cardiovascular surgery. With the exception of 2 cases seen more recently, the hearts and aortas of the 90 patients were examined and categorized by one of us (W.C.R.) from 1966 to 1990. All 90 had extensive involvement of the tubular portion of the ascending aorta by the syphilitic process, which spared the sinuses of Valsalva in all but 4 patients. The aortic arch was also involved

History of peritoneal access development.

The first peritoneal accesses were devices that had been used in other fields (general surgery, urology, or gynecology): trocars, rubber catheters, and sump drains. In the period after World War II, numerous papers were published with various modifications of peritoneal dialysis. The majority of cases were treated with the continuous flow technique; rubber catheters for inflow and sump drains for outflow were commonly used. At the end of the 1940s, intermittent peritoneal dialysis started to be more frequently used. Severe complications of peritoneal accesses created incentive to design access

History of surfactant from 1980.

The first successful trial of surfactant treatment for respiratory distress syndrome (RDS) was reported in 1980. Since then there have been numerous randomised trials demonstrating first, the efficacy of surfactant treatment in reducing pulmonary air leaks and increasing survival and second, assessing various other aspects of therapy. These studies show that multiple doses may be needed if surfactant is used to treat established RDS but early or prophylactic treatment is superior for infants with gestational ages less than 30 weeks. Natural surfactants (containing proteins) are more effective

History of acoustic neurinoma surgery.

The purpose of this article is to provide insight into the development of surgery for acoustic neurinomas throughout the years. The significant contribution of surgical authorities such as Cushing, Dandy, and House are discussed. The advances in surgical techniques from the very first operations for acoustic tumors at the end of the 19th century until today are described, with special emphasis on the technological and diagnostic milestones that preceded each step of this development.

Resistin: molecular history and prognosis.

Obesity and diabetes have reached epidemic proportions worldwide. The antidiabetic thiazolidinedione (TZD) drugs are insulin-sensitizing agents now widely used in the treatment of type 2 diabetes. TZDs are ligands for the nuclear hormone receptor peroxisome proliferator activated receptor gamma, which is a master regulator of adipogenesis and adipocyte metabolism. The molecular mechanisms by which TZDs improve insulin sensitivity have not been fully identified. Here we consider a novel secreted factor first identified as a TZD-suppressible gene in mouse adipocytes, called resistin, and discuss

[The vestibular organ throughout history].

Our aim is to attempt to create a chronologically ordered and coherent corpus of the apparently scarce information that exists about the history of the vestibular organ, a component of the inner ear situated on both sides of the head in the petrous temporal bone. Its job, at least in humans, is to transmit sensory information about movements of the head to components of the central nervous system. Some of its more common disorders lead to syndromes that implicitly entail balance disorders, such as the case of the syndrome described by Prosper Meni re in the 19th century. Without ruling out the…
