Integrating Population Variants and Protein Structural Analysis to Improve Clinical Genetic Diagnosis and Treatment in Nephrogenic Diabetes Insipidus.

Congenital nephrogenic diabetes insipidus (NDI) is a rare genetic disorder characterized by renal inability to concentrate urine. We utilized a multicenter strategy to investigate the genotype and phenotype in a cohort of Chinese children clinically diagnosed with NDI from 2014 to 2019. Ten boys from nine families were identified with mutations in AVPR2 or AQP2 along with dehydration, polyuria-polydipsia, and severe hypernatremia. Genetic screening confirmed the diagnosis of seven additional relatives with partial or subclinical NDI. Protein structural analysis revealed a notable clustering of…

Abnormalities of water metabolism in the elderly.

Hypernatremia is a frequent problem at the extremes of age, but particularly so in elderly individuals. Changes in the physiological responses to water deprivation with increasing age may be of particular interest in understanding the pathogenesis of hypernatremia in the elderly. When comparing healthy elderly men to younger controls, there are differences in the response to water deprivation. In older men, there are deficits in both the intensity and threshold of the thirst response. The ability to concentrate the urine also declines with age. There is both a decline in glomerular filtration …

Attenuated vascular reactivity in dogs with anteroventral third ventricle lesions.

Lesion of the anteroventral portion of the third cerebral ventricle causes hypernatremia, adipsia, and attenuation of the pressor response to intravenous administration of angiotensin II and norepinephrine. In addition, these lesions prevent the development of several experimental models of hypertension. In this study, a lesion of the third cerebral ventricle region was made in 14 dogs. In seven dogs in which hypernatremia developed the lesions included the organum vasculosum of the lamina terminalis; seven animals in which the circumventricular organ was spared by the lesion remained normonat…

Causes of spontaneous subarachnoid hemorrhage in Thai infants and children. A study of 56 patients.

The authors report 56 cases of spontaneous subarachnoid hemorrhage in Thai infants and children. The causes were: hemorrhagic disorders in 22 cases, arteriovenous malformations and aneurysms in 18, gnathostomiasis in nine, bleeding tumors in four, hypernatremia in one, and undertermined causes in two cases. Coagulation studies and cerebrospinal fluid examination for eosinophils are recommended before further invasive studies in such cases.

[Electrolyte Emergencies. Selected Aspects].

The question of what factors determine the significance of abnormal electrolyte findings is discussed with reference to a number of examples of acute electrolyte emergencies (hyponatremia, hypernatremia, hypokalemia, hyperkalemia, metabolic alkalosis). In addition to the extent of the deviation from normal and the clinical manifestations already present in all cases, it is principally the rate of dysfunction which determines the action of a clinical or laboratory-determined electrolyte deviation.

Age-associated changes in the endocrine system.

This article explores age-related changes in hormone metabolism and production. Common signs and symptoms of endocrine diseases of the elderly may be masked by or exacerbate other disabilities, and the patient may present with atypical signs and symptoms. Glucose intolerance, diabetes, salt and water metabolism (hyponatremia and hypernatremia), thyroid disease, male reproductive function, and growth hormone are discussed. Clinical and research aspects are presented.

Hyperuricemia in diarrheal dehydration.

Serum uric acid levels were measured in 133 dehydrated patients admitted with diarrhea. Elevated levels were found in 80% of the patients; the highest was 38 mg/dL. Serum uric acid was correlated with serum urea nitrogen and the levels tended to be higher in patients with hypernatremia. The serum uric acid levels returned toward normal with rehydration, and no specific therapy was necessary.

Neonatal Hypernatremic Dehydration.

Newborn infants experience significant fluid shifts following delivery as a result of the contraction of extracellular water. Without prompt identification and treatment, significant weight loss can result in neonatal hypernatremic dehydration. This case study presents an infant with significant weight loss, dehydration, and electrolyte imbalances. The differential diagnosis for hypernatremia is presented along with clinical presentation, treatment, and discharge education recommendations.

Nephrogenic diabetes insipidus presenting with developmental delay and intracranial calcification.

A one-year-boy presented with constipation, fever, failure to thrive and developmental delay from the neonatal period. Investigations revealed persistent hypernatremia and deranged renal functions. Diagnostic work-up was suggestive of nephrogenic diabetes insipidus (NDI). Computerized tomography of head revealed calcification in the frontal, thalamic and basal ganglia region. The rare association of NDI and intracranial calcification is discussed.

A Stroke of Luck: Central Diabetes Insipidus Unmasked by a Heat Stroke.

Diabetes insipidus is a rarely encountered cause of hypernatremia, often presenting a diagnostic and therapeutic dilemma for the encountering physician. Patients are often asymptomatic for a number of years due to compensation of their polyuria with polydipsia, but may have dramatic presentations in situations where they lose access to hydration. Our case is of a 62-year-old woman who was found unconscious with signs and symptoms of a heat stroke, and later was found to have resistant hypernatremia that persisted despite extensive free water supplementation. She had dilute polyuria throughout …

[Study of diagnostic value of natremia in a cohort of patients with hypercalcemia].

INTRODUCTION: Hypercalcemia is a common pathological condition in clinical practice. The two most common causes are primary hyperparathyroid … usion of patients under treatment with loop diuretics. Conversely, there was no association between hypernatremia and cancer-related hypercalcemia. Among 151 patients with hypercalcemia, 16 presented hyponatremia and 7 with hypernatremia. SIADH was the main cause of hyponatremia. We performed univariate and multivariate logistic regression showing the associati …

Choice of fluid therapy in patients of craniopharyngioma in the perioperative period: A hospital-based preliminary study.

BACKGROUND: Electrolyte imbalance and acute diabetes insipidus (DI) are the most common complications in patients undergoing craniopharyngioma surgery. Improper management of water and electrolyte imbalance is common cause of morbidity and mortality. Data is sparse and controversial regarding the choice of fluid therapy in this population during perioperative period. METHODS: In this retrospective-prospective study involving 73 patients (58 retrospective), the type of fluid therapy was correlated with occurrence of hypernatremia, hyponatremia, DI, morbidity, and mortality. In the retrospectiv…

Transient diabetes insipidus after discontinuation of therapeutic vasopressin.

The use of vasopressin for the treatment of septic shock is increasing. Few reports of fluid and electrolyte complications of this therapy have been reported. A neurologically impaired, 53-year-old man with a history of syndrome of inappropriate antidiuretic hormone developed apparent transient diabetes insipidus and acute hypernatremia after being treated with vasopressin. He was treated for presumed septic shock with intravenous vasopressin 0.01-0.10 U/minute. His blood pressure did not improve with this therapy, and his course was complicated by hyponatremia during the vasopressin infusion.…

Early severe dehydration in young breast-fed newborn infants.

Six breast-fed infants living in a dry desert climate area presented at ages 4-11 days with severe dehydration. In all cases, dehydration wa … n breast milk was elevated in all five mothers in whom it was measured, and three babies had severe hypernatremia. Successful relactation was achieved in three cases with a decrease in the milk sodium concentration. These data demonstrate that severe dehydration in breast-fed infants may occur as early as the first week of life and may affect even infants of ex …

Metabolic disturbances after head injury: abnormalities of sodium and water balance with special reference to the effects of alcohol intoxication.

Eighty-eight patients with craniocerebral trauma were studied prospectively to assess the effects of the injury on sodium and water balance. Abnormalities of serum sodium and osmolality occurred in 11 of the 76 patients who were on the study more than 24 hours, and the incidence of these abnormalities was directly related to the severity of the craniocerebral injury. Hyponatremic hypo-osmolar states were as frequent as were hypernatremia and serum hyperosmolality. The major cause of the hyponatremia was inappropriate antidiuretic hormone secretion; hypernatremia was due to dehydration and occu…

Use of N-Acetylcysteine in Preterm Neonates with Enteral Feeding Intolerance and Intestinal Obstruction: A Case Series and Review of the Literature.

(1) Background: The use of N-acetylcysteine (NAC) to relieve meconium obstruction of prematurity in the first days of life has been reported, with NAC reducing the viscosity of luminal contents by cleaving the disulfide bonds of mucoproteins. However, its use in this population should be further explored since it has been associated with hypernatremia and transient increase in transaminases and bilirubin. (2) Methods: In this retrospective study, we included neonates admitted because of enteral feeding intolerance and intestinal obstruction from 2019 to 2021 who received NAC as a rescue therap…
