A rare case of hyperostosis frontalis interna in an 86-year-old Japanese female cadaver.

Hyperostosis frontalis interna (HFI) is a condition characterized by abnormal bone outgrowth on the inner surface of the frontal bone. Most HFI cases occur in post-menopausal elderly women. The pathology of HFI development is uncertain. The estimated incidence of HFI ranges from 5 to 12% in Western countries, but few cases have been reported in the Japanese population. Here, we report a case of HFI in an 86-year-old Japanese female cadaver. Macroscopically, the internal surface of the frontal bone exhibited bilateral nodular protrusion with sparing of the midline, while the external surface wa…

Back to the beginning: Identifying lesions of diffuse idiopathic skeletal hyperostosis prior to vertebral ankylosis.

OBJECTIVE: To better understand the pathogenesis of DISH, identifying early or pre-DISH lesions in the spine and investigating the relationship between spinal and extra-spinal manifestations of DISH. MATERIAL: 44 skeletonized individuals with DISH from the WM Bass Donated Skeletal Collection. METHODS: For each vertebra, location, extension, point of origin and appearance of vertebral outgrowths were recorded. The size of the enthesophytes at the olecranon process, patella and calcaneal tuberosity was measured with digital callipers. RESULTS: At either end of the DISH-ankylosed segment, isol

Polyostotic cortical hyperostosis in an 8-week-old cat with a 3-year follow-up.

A 2 month-old female cat, mixed breed, was referred for difficulty moving and severe enlargement of the mandible and limbs. Polyostotic cortical hyperostosis was diagnosed based on diagnostic imaging and histopathological changes of the mandible and limbs. Marked cortical bone thickening was detected on radiographs and CT scan images. The diaphyses of both radii and ulnae, together with the mandibular rami and bodies, were most severely affected. The many similarities shared with the human condition, Caffey's disease, are discussed.

Localization of the gene for hyperostosis cranialis interna to chromosome 8p21 with analysis of three candidate genes.

Hyperostosis cranialis interna (HCI) is a rare autosomal dominant disorder characterized by intracranial hyperostosis and osteosclerosis, which is confined to the skull, especially the calvarium and the skull base. The rest of the skeleton is not affected. Progressive bone overgrowth causes nerve entrapment that leads to recurrent facial nerve palsy, disturbance of the sense of smell, hearing and vision impairments, impairment of facial sensibility, and disturbance of balance due to vestibular areflexia. The treatment is symptomatic. Histomorphological investigations showed increased bone form…

Diffuse idiopathic skeletal hyperostosis (DISH): a review of radiographic features and report of four cases.

OBJECTIVE: To discuss the diagnostic criteria, clinical findings and radiographic findings of diffuse idiopathic skeletal hyperostosis (DISH). CLINICAL FEATURES: DISH is a disease that affects elderly persons and is characterized by a bone-forming tendency with prominent radiographic findings, including proliferation of bone throughout the ligaments and tendons of the spine and extremities. Clinical symptoms are often mild and include mild pain and stiffness. DISH can coexist with other articular diseases, such as degenerative joint disease and rheumatoid arthritis, or can be complicated with…

Fracture of the spine in patients with ankylosis due to diffuse skeletal hyperostosis: clinical and imaging findings.

OBJECTIVE: Only 12 patients with a fracture through a portion of the spine ankylosed by diffuse idiopathic skeletal hyperostosis have been reported. The purpose of this study was to determine the types of causative trauma, spinal sites at risk for fracture, complications mortality, diagnostic difficulties, and abnormalities identified only with special imaging studies in a group of 15 patients with this complication seen at our institution. MATERIALS AND METHODS: The study included 15 patients with diffuse idiopathic skeletal hyperostosis who had a spinal fracture through an area of ankylosed…

Hyperextension injuries of the thoracic spine in diffuse idiopathic skeletal hyperostosis. Report of four cases.

Four patients who had multisegmental ankylosis of the thoracic and lumbar spine due to diffuse idiopathic skeletal hyperostosis sustained a hyperextension fracture-dislocation. The patients had a mean age of sixty-four years (range, fifty-eight to sixty-nine years); all four patients were men. All injuries occurred between the seventh and eleventh thoracic vertebrae. All patients had intact neurological function at the time of admission to the hospital. Three patients were managed with posterior spinal arthrodesis with Cotrel-Dubousset segmental instrumentation; one patient was managed non-ope…

Diffuse idiopathic skeletal hyperostosis (DISH) and spondylosis deformans as predictors of cardiovascular diseases and cancer.

The associations between DISH (diffuse skeletal Hyperostosis) and spondylosis deformans on the one hand and cardiovascular disease and cancer on the other were studied in a follow-up investigation of 6 167 persons in Finland. Mean duration of follow-up investigation of 6 167 persons in Finland. Mean duration of follow-up was 6.3 years. Similar methods were employed in the baseline and follow-up examination. A finding of DISH or of spondylosis was based on a reading of lateral X-ray films. Age-adjusted incidence of hypertension, ECG findings suggesting coronary heart disease (CHD), CHD history,…

Variable Aβ tracer uptake in hyperostosis frontalis interna: implications for brain Aβ PET/CT interpretation.

INTRODUCTION: Hyperostosis frontalis interna (HFI) is a relatively common incidental finding on head computed tomography (CT). We report two patients with HFI identified on amyloid-beta (Aβ) positron emission tomography (PET)/CT who demonstrated distinct patterns of tracer uptake within the affected calvarium. METHODS: Both patients underwent 18F-florbetaben PET/CT imaging to assess eligibility for Aβ-targeting therapy. RESULTS: Patient 1 was an 85-year-old woman with amnestic mild cognitive impairment whose 18F-florbetaben PET/CT scan revealed a Centiloid value of 59 and marked tracer uptak…

Vocal Cord Paralysis Secondary to Diffuse Idiopathic Skeletal Hyperostosis: A Case Report and Literature Review.

Diffuse idiopathic skeletal hyperostosis (DISH) is a condition that causes excessive bone growth along the spinal ligaments. While often silent, it can lead to symptoms such as neck pain or difficulty swallowing and, in rare cases, vocal cord paralysis (VCP). We present a 61-year-old man with progressive hoarseness due to DISH-related VCP. Early recognition of DISH as a cause of VCP is crucial, as timely diagnosis can guide appropriate management, ranging from conservative observation to surgical intervention in severe cases.

MRI Imaging Appearance of Hyperostosis Frontalis Interna (HFI): A Case Report of Focal Benign Enhancement.

INTRODUCTION: Hyperostosis frontalis interna (HFI) is a common and often incidental finding seen on imaging. There is a significant paucity of radiology literature, particularly regarding the MRI imaging appearance of HFI. CASE PRESENTATION: We reported two cases of HFI on MRI, which showed focal enhancement. These were stable on long-term follow-up studies and thought to be most consistent with benign enhancement. CONCLUSION: Further studies are needed to elucidate the underlying pathogenesis; however, it is important to be aware that regions of HFI may demonstrate variable enhancement and …

RESOLUTION OF CALVARIAL HYPEROSTOSIS IN AFRICAN LION CUBS (PANTHERA LEO LEO) AFTER VITAMIN A SUPPLEMENTATION.

Two female (FL 1, FL 2) and one male (ML) 11-wk-old, intact, captive African lion cubs (Panthera leo leo) were presented with a history of mild vestibular signs. Initial serum vitamin A concentrations were low (140 nmol/L) for ML. Calvarial hyperostosis was confirmed using computed tomography (CT) of the head and cervical vertebrae in each cub. CT measurements were adapted in relation to the skull width. ML showed the most pronounced thickening of the tentorium cerebelli and occipital bone, represented by a tentorium cerebelli to skull width ratio (TCR) of 0.08 (FL 1: 0.06, FL 2: 0.05) and a b…

Life Expectancy Is Poor in Patients with Diffuse Idiopathic Skeletal Hyperostosis-Related Pyogenic Vertebral Osteomyelitis.

INTRODUCTION: Pyogenic vertebral osteomyelitis (PVO) is an uncommon but life-threatening infectious disease. Diffuse idiopathic skeletal hyperostosis (DISH) is an age-related disorder and sometimes problematic in terms of spinal instability or high mortality, especially in cases of DISH-related fracture. Meanwhile, no reports have focused on the impact of DISH on the clinical outcomes after treatment for PVO. We hypothesized that PVO occurring at DISH-related segments might contribute to poor clinical results or high mortality rates. The purpose of this study was to investigate the impact of D…

Surgical Treatment of Dysphagia Secondary to Anterior Cervical Osteophytes Due to Diffuse Idiopathic Skeletal Hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is an ossifying and ankylosing skeletal condition that can be associated with DISH-related dysphagia in the case of cervical involvement. In contrast to most cases of dysphagia, which are managed conservatively, DISH-related dysphagia can be discouraging due to the progressive nature of DISH. We report two cases of DISH-related dysphagia that were treated with the surgical removal of osteophytes via an anterolateral approach. We were able to remove osteophytes using the bottleneck point as an anatomical landmark between the vertebral body and the…
