Familial variable immunodeficiency: autosomal dominant pattern of inheritance with variable expression of the defect(s).

In 1963, Rosen and Bougas reported the case of a woman with recurrent infection, marked elevation of 19S, and virtual absence of 7S gamma globulin. Recently, members of her family were found to have similar abnormalities: Ten of the 37 family members tested had elevated levels of serum IgM accompanied by a combined deficiency of IgG and IgA in three, and by a deficiency of either IgG or IgA in two. In five, an increase in IgM was the sole abnormality. Two children had deficiencies of IgG and IgA with normal serum levels of IgM. Ten of the 12 affected individuals had no IgD detectable by radial

Clinical heterogeneity among pediatric patients with autoimmune type 1 diabetes stratified by immunoglobulin deficiency.

BACKGROUND: Type 1 diabetes (T1D) may coexist with primary immunodeficiencies, indicating a shared genetic background. OBJECTIVE: To evalua … gM >2 standard deviations (SD) below age-adjusted mean. IgE deficiency was defined as IgE <2 kIU/L. IgA deficiency (IgAD) was defined as IgA >2 SD below age-adjusted mean irrespective of other immunoglobulin classes (absolute if <0.07 g/L, partial otherwise) and as selective IgAD when IgA >2 SD below age-adjusted mean with normal IgG and IgM (absolute if <0.07 g …

Disorders of Humoral Immunity in Children with IgG Subclass Deficiency and Recurrent Respiratory Infections.

Respiratory tract infections in children are one of the most common causes for medical consultations. When the infections are of recurring n … of children with IgG deficiencies, all of whom had normal IgE levels. There were 8.4% (33 cases) of IgA deficiency, 7.4% (29 cases) of IgM insufficiency, and 4.1% (16 cases) of CD19+ cells deficiency. The elevated level of CD19+ cells was found in 27.7% (109 out of the 394 children). Immunoglobulin deficiencies often coexisted with a deficiency in another immuno …

IgA modulates respiratory dysfunction as a sequela to pulmonary chlamydial infection as neonates.

Neonatal Chlamydia lung infections are associated with serious sequelae such as asthma and airway hyper-reactivity in children and adults. Our previous studies demonstrated the importance of Th-1 type cytokines, IL-12 and IFN-γ in protection against neonatal pulmonary chlamydial challenge; however, the role of the humoral arm of defense has not been elucidated. We hypothesized that B-cells and IgA, the major mucosal antibody, play a protective role in newborns against development of later life respiratory sequelae to Chlamydia infection. Our studies using neonatal mice revealed that all WT and

Selective immunoglobulin a deficiency in type 1 diabetes mellitus: a prevalence study in Western sicily (Italy).

BACKGROUND: The association between type 1 diabetes and immunoglobulin A deficiency (IgA-D) has long been recognized in many populations. The aim of this study was to assess the prevalence of IgA-D in patients with type 1 diabetes mellitus all coming from a defined geographical area and to investigate the clinical features of these subjects. METHODS: The records of 150 consecutive patients with type 1 diabetes mellitus referred in a period of one year were analyzed. A detailed history was obtained for each patient. Information was collected concerning age, gender, time of onset of diabetes, a

[IgA, an essential part of the immune system: selected issues].

Immunoglobulin class A is the main protein of the mucosal immune system. The daily production of this isotype exceeds the synthesis of all other classes of immunoglobulins. Because the mucosal surface area of the human organism is about 400 m2, the role of IgA in the defense of this surfaces from harmful agents from the external environment is extremely essential. Any changes connected with a lack or an overproduction of this immunoglobulin can manifest as different clinical diseases. In this paper some selected issues on the heterogeneity of the IgA are presented, its structure and significan

Hematopoyetic deficiency in polymeric immunoglobulin receptor attenuates experimental atherosclerosis.

BACKGROUND: Polymeric immunoglobulin receptor (PIGR) is a transmembrane protein involved in the transcytosis of polymeric IgA and IgM immunoglobulins. Recent data from our group described that systemic deficiency of Pigr reduces atherosclerosis development, which is associated with increased plasma IgA and IgM levels. PURPOSE: To analyse the role of Pigr deficiency in hematopoietic cells in an experimental model of atherosclerosis in mice. METHODS: We analyzed PIGR distribution in early atherosclerotic plaques compared to healthy aortic samples, as well as PIGR expression in macrophages deri

The four most common pediatric immunodeficiencies.

The four most common immunodeficiencies in pediatric patients are transient hypogamma-globulinemia of infancy, IgG subclass deficiency, impaired polysaccharide responsiveness (partial antibody deficiency) and selective IgA deficiency. Most of these patients have normal cellular immune systems, phagocyte function and complement levels. All four illnesses are characterized by recurrent bacterial respiratory infections such as purulent rhinitis, sinusitis, otitis and bronchitis. Except for some IgA-deficient patients, the molecular basis for these illnesses is not known, and indeed each syndrome …

Infections of the nose and paranasal sinuses in adult patients with immunodeficiency.

Twenty-two patients with common variable immunodeficiency (CVID) and 18 patients with selective IgA deficiency were examined with respect to previous and present infections of the upper respiratory tract (URT), especially of the nose and paranasal sinuses. Recurrent acute rhinosinusitis was common in both groups of patients, but the development of chronic rhinosinusitis was only found in patients with CVID, indicating the more severe nature of this condition compared with selective IgA deficiency. The infections of the URT occurred several years before the appearance of lower respiratory tract…

Human faecal immunoglobulins in healthy infants and children, and in some with diseases affecting the intestinal tract or the immune system.

IgA, IgG and IgM is faeces were quantified by single radial immunodiffusion using extracts of freeze-dried faeces. IgA in small specimens of … erative colitis in remission, contained IgG in markedly increased concentrations. Two patients with IgA deficiency had no detectable IgA in faeces, but had increased levels of faecal IgM which also agglutinated rabbit erythrocytes. Immunoglobulins were not demonstrated in faeces from three patients with agammaglobulinaemia. The findings indicate that faeces can …

Lack of Utility of Anti-tTG IgG to Diagnose Celiac Disease When Anti-tTG IgA Is Negative.

OBJECTIVES: Guidelines for diagnosing celiac disease (CD) recommend initial testing with a highly sensitive serologic test for anti-tissue transglutaminase immunoglobulin A antibodies (tTG IgA). When the probability of CD is high, IgA deficiency should be considered. The 2 approaches to address this include measuring "both tTG IgA and tTG IgG" or measuring "total IgA." We aim to assess the utility of an isolated positive tTG IgG result in diagnosing CD. METHODS: We conducted a retrospective review of patients undergoing serologic testing for CD from January 1997 to June 2014. Patients with po…

Dietary pectin remodels commensal bacterial surfaces to promote IgA-mediated protection from intestinal inflammation.

Diet-microbe interactions shape mucosal immunity, yet how dietary cues drive strain-specific immunoglobulin A (IgA) responses remains poorly understood. Here, we show that dietary pectin induces a microbiota-dependent high-IgA phenotype in gnotobiotic mice mono-colonized with Bacteroides uniformis (B. uniformis). This effect is mediated by a pectin-responsive rcsC-associated regulatory program that remodels the bacterial surface. Comparative lipidomics identifies the glycosphingolipid HexCer (34:0) as a dominant feature of the remodeled bacterial surface. HexCer is recognized by the C-type lec
