COVID-19 in corrections: Quarantine of incarcerated people.

Carceral settings in the United States have been the source of many single site COVID-19 outbreaks. Quarantine is a strategy used to mitigate the spread of COVID-19 in correctional settings, and specific quarantine practices differ state to state. To better understand how states are using quarantine in prisons, we reviewed each state's definition of quarantine and compared each state's definition to the Centers for Disease Control's (CDC) definition and recommendations for quarantine in jails and prisons. Most prison systems, 45 of 53, define quarantine, but definitions vary widely. No state p

Incarcerated Rectal Procidentia: A Case Report and Review.

Rectal procidentia is an uncommon perineal disease that is rare in males. There is no specific medical role in treatment of rectal procidentia and surgical intervention is the treatment of choice. Various surgical approaches have been performed, but there is no consensus on which procedure is most effective in terms of patient condition, recurrence rate, bowel function, and risk. This case presentation of a healthy male patient with experience of uncomplicated reducible rectal prolapse and a history of chronic constipation. Presented with complicated rectal prolapse in the presence of incarcer

Hematemesis Secondary to Complex Incarcerated Pantaloon Hernia.

Hematemesis with concomitant small bowel obstruction is an uncommon emergency department presentation. We report the case of a patient who p … l and the need for rapid surgical intervention can be difficult. Hematemesis concurrent with hernia incarceration may be suggestive of impending complete bowel obstruction and ischemia.

Care for Incarcerated Patients Hospitalized with COVID-19.

The COVID-19 pandemic has reshaped health care delivery for all patients but has distinctly affected the most marginalized people in society. Incarcerated patients are both more likely to be infected and more likely to die from COVID-19. There is a paucity of guidance for the care of incarcerated patients hospitalized with COVID-19. This article will discuss how patient privacy, adequate communication, and advance care planning are rights that incarcerated patients may not experience during this pandemic. We highlight the role of compassionate release and note how COVID-19 may affect this pros

Kidney Disease among People Who Are Incarcerated.

CKD affects 15% of US adults and is associated with higher morbidity and mortality. CKD disproportionately affects certain populations, including racial and ethnic minorities and individuals from disadvantaged socioeconomic backgrounds. These groups are also disproportionately affected by incarceration and barriers to accessing health services. Incarceration represents an opportunity to link marginalized individuals to CKD care. Despite a legal obligation to provide a community standard of care including the screening and treatment of individuals with CKD, there is little evidence to suggest s…

A fistulating incarcerated incisional hernia: a case report.

An incisional hernia is usually a defect in the scar of an abdominal surgery. The natural history is intestinal obstruction with the risk of strangulation. We report a case of a long-term conservative management of an incisional hernia with an abdominal corset. This resulted in fistulation from pressure necrosis that required an en-bloc excision of the incarcerated fistulating bowel with the hernia sac. The defect was managed using the Jenkin's 'mass closure' technique with no recurrence of the hernia.

Anisakiasis in a Canadian patient with incarcerated epigastric hernia.

Anisakiasis is human zoonotic parasitic infection caused by a nematode parasite called Anisakis. This infection is usually reported in Asian countries where consumption of raw seafood is common. Very few cases have been reported in North America. We present the case of a female Canadian patient with an Anisakis larvae in an incarcerated ventral hernia. Cases of Anisakis infections are exceedingly rare in western countries, with very few previous reports describing extra-gastrointestinal cases. Diagnosis is often difficult since the symptoms of anisakiasis are not pathognomonic. As the larvae c

Incarcerated Individuals' Perspectives on Living with Serious Illness.

The object of this study was to elicit patient perspectives on the experience of living with serious illness while incarcerated. The study was conducted at the Rhode Island Adult Corrections Institutions (ACI) in both the men's medium security and women's (all levels) facilities in June of 2016. Semi-structured interviews were conducted with participants, who met study criteria for serious illness. Interviews were coded following the template organizing style. Eighteen participants were enrolled, 13 males and five females with majority Caucasian (n=11) and ages 40-59 (n=9). Incarcerated indivi

Multiple Bladder Calculi in an Incarcerated Genitourinary Prolapse.

Background: Genitourinary prolapse is a common entity in women rarely needing emergency intervention. However, in the rare event of multiple vesicle calculi causing acute incarceration and obstructive uropathy surgical intervention should not be delayed. Case Presentation: We herein present case of a 65-year-old woman who presented with irreducible urogenital prolapse and eventually found to have multiple vesicle calculi with bilateral hydro-ureteronephrosis. The patient underwent an initial suprapubic cystolithotomy with subsequent hysterectomy and prolapse repair. Conclusion: Multiple vesicl…

Reducing Hospital Readmissions Among Incarcerated Patients.

In many health care settings, hospital readmissions represent poor quality and increased costs. The California Department of Corrections and Rehabilitation includes reduced readmission rates among key performance indicators for the quality of care provided to inmates. More than 25% of the institutions continue to face challenges in minimizing these occurrences. Despite historic deficiencies, the California Substance Abuse Treatment Facility and State Prison at Corcoran reduced 30-day readmissions over a 2-year period. The use of a mnemonic algorithm along with intervention strategies was effec

Cervical cancer screening among incarcerated women.

BACKGROUND: Women with a history of incarceration bear a disproportionate burden of cervical disease and have special characteristics that affect their intent and/or ability to adhere to cervical screening and follow-up recommendations. The goal of this study was to identify factors associated with cervical cancer screening and screening outcomes among incarcerated women. METHODS: We applied a framework of predisposing factors, enabling factors and population-specific characteristics that could impact screening behaviors and outcomes for this population. We used bivariate chi-square tests and…

Characterization of drug overdoses in an Ohio incarcerated population.

CONTEXT: Literature exists about drug overdose following release of recently incarcerated individuals and mortality among prisoners in the United States, but little information exists about drug overdoses in the imprisoned population. OBJECTIVE: This study aims to quantify and describe prisoner medication overdose requiring inpatient admission or assessment at a tertiary care facility and to assess the associated hospital charges. MATERIALS AND METHODS: A single-center, retrospective cohort study was conducted on all Ohio Department of Rehabilitation and Correction inmates who presented to T

A rare case of appendicitis incarcerated in an inguinal hernia.

Amyand's hernia was coined after Claudius Amyand (1660-1740), who was the first to describe the presence of a perforated appendix in a hernial sac and also was the first to perform a successful appendectomy in 1735. It is an exceptionally rare condition in which the hernia itself contains the appendix, which may not necessarily be inflamed. The presence of an inflamed appendix further contributes to the rarity of this case. We report a case of acute appendicitis brought on by its incarceration in the inguinal hernia.

Health issues of incarcerated women in the United States.

Health care within jails and prisons in the United States is typically insufficient to meet the medical and psychological needs of female inmates. Health services are often of low quality, especially in the areas of reproductive medicine. Mental illness, substance abuse, a trauma history, and sexual victimization while incarcerated can predict a more difficult adjustment to a correctional environment. Incarcerated women who are able to maintain contact with family members, especially children, can have a better prison adjustment. Recommendations are made to improve the types and quality of hea

The Unmet Contraceptive Need of Incarcerated Women in Ontario.

OBJECTIVE: Studies from the United States have shown that women in correctional facilities have higher rates of unintended pregnancy and unm … y intention as unplanned or ambivalent. Of women who were at risk for unintended pregnancy prior to incarceration, 80% were not using a reliable form of contraception. CONCLUSION: Incarcerated women in Ontario have higher rates of unintended pregnancy and unmet need for contraception than does the general population. The provision of family planning services d …

History of Sex Exchange in Women with a History of Incarceration.

Sex exchange among incarcerated women is not well-described in the literature. Sex exchange can lead to numerous adverse health outcomes, especially when combined with individual factors (e.g., depression and homelessness) and larger systemic inequalities. The purpose of this study was to explore factors associated with having a history of sex exchange among a sample of incarcerated women. Of 257 women surveyed in this study, 68 women (26.5%) reported a history of sex exchange. In multivariate logistic regression analysis, physical abuse history (p=.05, OR 2.20), history of two or more sexuall
