A rare case of ocular onchocerciasis in India.

Onchocerca volvulus is a spirurid nematode that mainly affects the rural poor of Sub-Saharan Africa, Yemen and parts of Central and South Africa. River blindness, caused by Onchocerca volvulus, is considered to be the second commonest infectious cause of blindness worldwide. We report a rare case of ocular onchocerciasis where a live adult worm was extracted from the eye of a patient from a nonendemic region. The worm was identified as Onchocerca volvulus based on morphological features. The patient was treated with Ivermectin (0.2 mg/kg). At six months follow-up she had complete remission of

Polio Eradication in India: Myth or Reality.

BACKGROUND: Poliomyelitis has been eradicated from large parts of the world. In South East Asian Region, India and Pakistan remain the only countries where active transmission of the disease persists. A decade-long initiative to eradicate the disease is in progress in India. Initial results were encouraging, with the number of fresh polio decreasing till 2001. The year 2002, however, witnessed a setback, with significant rise in cases of poliomyelitis. METHODS AND RESULTS: The eradication measures have been reviewed. The corrective measures appear to be succeeding. Various facets of eradicati…

Continuing challenge of infectious diseases in India.

In India, the range and burden of infectious diseases are enormous. The administrative responsibilities of the health system are shared between the central (federal) and state governments. Control of diseases and outbreaks is the responsibility of the central Ministry of Health, which lacks a formal public health department for this purpose. Tuberculosis, malaria, filariasis, visceral leishmaniasis, leprosy, HIV infection, and childhood cluster of vaccine-preventable diseases are given priority for control through centrally managed vertical programmes. Control of HIV infection and leprosy, but…

Glaucoma in India: estimated burden of disease.

Over the last decade the prevalence of glaucoma has been reported by the Vellore Eye Survey, Andhra Pradesh Eye Disease Study, Aravind Comprehensive Eye Survey, Chennai Glaucoma Study, and West Bengal Glaucoma Study. There have been some differences largely because of methodologic variations. We use the reported age and gender stratified prevalence estimates from these studies and the Indian population census estimates to calculate the number of persons with glaucoma or at risk of the disease in the country. On the basis of the available data, we estimate that there are approximately 11.2 mill…

Antimicrobial susceptibility of Salmonella Typhi in India.

BACKGROUND: Typhoid fever continues to remain a major public health problem, especially in regions such as Gulbarga, due to poor sanitation and personal hygiene. Gulbarga region is often prone to enteric fever outbreaks and is an endemic region of typhoid fever. Enteric fever caused by Salmonella Typhi has not been adequately explored in this region. METHODOLOGY: A total of 95 isolates of S. Typhi collected from different clinical and environmental sources were tested for antimicrobial susceptibility according to the CLSI guidelines. MIC of resistant isolates to various antibiotics was perfor

Human trichinosis in remotes of Uttarakhand, India.

Trichinosis is a parasitic zoonosis caused by ingestion of infected meat containing larvae of Trichinella, more prevalent in developing countries. Although infection with Trichinella is globally distributed, it has been documented only rarely in India. The reports are available where Trichinella larvae were found from animals in India but, to our knowledge, only one human case has been reported from India (Punjab), so far. This is the first report of small multiple outbreaks of human trichinosis in India (2009-2011). Epidemiological, clinical, and laboratory findings of trichinosis patients be…

Sexually transmitted diseases in children in India.

Sexually transmitted diseases (STDs) in children are not uncommon in India, though systematic epidemiological studies to determine the exact prevalence are not available. STDs in children can be acquired via sexual route or, uncommonly, via non-sexual route such as accidental inoculation by a diseased individual. Neonatal infections are almost always acquired intrauterine or during delivery. Voluntary indulgence in sexual activity is also an important factor in acquisition of STDs in childhood. Sexual abuse and sex trafficking remain the important problems in India. Surveys indicate that nearl…

Histopathologic risk factors in retinoblastoma in India.

CONTEXT: The presence of histopathologic risk factors is associated with development of metastasis in a patient with retinoblastoma. Adjuvant chemotherapy administered to such patients decreases the risk of metastasis. OBJECTIVE: To analyze the incidence of histopathologic risk factors in our patient population and the clinical predictors of such risk factors. DESIGN: This is a retrospective case series, with review of clinical data and histopathologic slides, in 142 consecutive eyes enucleated for retinoblastoma between 1996 and 2002. RESULTS: Histopathologic risk factors were present in 5

Skewed sex ratio at birth in India.

Scientists commonly use world average data on sex ratio at birth for India for want of dependable ones. Here an attempt is made to redress the problem to some extent. It is shown that this ratio has been high in India since the 1950s. The ratio has been strikingly high, even prior to the time of inception of prenatal sex identification technologies. The ratio shows a rising trend due to several biological reasons. In addition, it has been rising sharply for a couple of decades due to some socio-medical factors. The natural sex ratio at birth in India is noticeably higher than the world average…

Women and substance use in India and Bangladesh.

Given the feminization of the HIV epidemic in India and Bangladesh, understanding substance-use-related concerns among women is important for effective HIV prevention. This review is based on published research, reports (2000-2005), and primary data from treatment centers for drug-using women. We identify four main themes: (a) opioid use and injecting drug use in women, (b) alcohol use in sex work settings, (c) sexual transmission of HIV from male-injecting drug users (IDUs) to their regular female sex partners, and (d) sexual violence among female partners of substance-using men. We urge for …

Epidemiology of typhoid and paratyphoid fever in India.

Enteric fever (typhoid and paratyphoid fever) is a major human bacterial infection. Although the disease is not common in industrialised countries, it remains an important and persistent health problem in developing nations. Hospital-based studies and outbreak reports from India indicate that enteric fever is a major public health problem in this country, with Salmonella enterica serovar Typhi (S. Typhi) the most common aetiologic agent but with an apparently increasing number of cases due to S. Paratyphi A (SPA). Because risk factors such as poor sanitation, lack of a safe drinking water supp…

Community mental health in India: A rethink.

BACKGROUND: Community care of the chronic mentally ill has always been prevalent in India, largely due to family involvement and unavailability of institutions. In the 80s, a few mental health clinics became operational in some parts of the country. The Schizophrenia Research Foundation (SCARF), an NGO in Chennai had established a community clinic in 1989 in Thiruporur, which was functional till 1999. During this period various programmes such as training of the primary health center staff, setting up a referral system, setting up of a Citizen's Group, and self-employment schemes were initiate…

Pathogenic bacteria in an orthopaedic hospital in India.

BACKGROUND: The problem of changes in pathogenic microbiological flora and the emergence of bacterial resistance has created major problems in the management of orthopaedic diseases and fractures. Due to the use of implants for open reduction and internal fixation, which are foreign bodies to the body, orthopaedic trauma surgery is at grave risk of microbiological contamination and infection. METHODOLOGY: With new microbiological agents in vogue, we have conducted a retrospective study to determine the pattern of bacterial infection, sensitivity to various antimicrobial agents, and their rela

Overview of opportunistic fungal infections in India.

In recent years fungi have been flourishing in immunocompromised patients of tertiary care centers. The data on the burden of opportunistic mycoses in India is not clear though the climate in this country is well suited for a wide variety of fungal infections. There are very few good diagnostic mycology laboratories and clinicians are still not aware of the emerging trends. Within the limited data available, an increased incidence of invasive candidiasis, aspergillosis, and zygomycosis are reported. The emergence of fungal rhinosinusitis, penicilliosis marneffei and zygomycosis due to Apophyso…
