MAFLD is associated with increased all-cause mortality in low cardiovascular-risk individuals but not in intermediate to high-risk individuals.

BACKGROUND AND AIMS: Metabolic-associated fatty liver disease (MAFLD) is increasingly recognized as a systematic disease rather than just a liver disease alone, which raises concerns about its long-term impact on different populations. This study aimed to clarify the effects of MAFLD on long-term outcomes among different cardiovascular risk-stratified populations. METHODS AND RESULTS: Eligible individuals in the Third National Health and Nutrition Examination Surveys (NHANES Ⅲ, 1988-1994) were enrolled. Participants were classified into low, intermediate, or high cardiovascular-risk populatio

Differences in the Oral Microbiome in Patients With Early Rheumatoid Arthritis and Individuals at Risk of Rheumatoid Arthritis Compared to Healthy Individuals.

OBJECTIVE: It has been suggested that rheumatoid arthritis (RA) may originate at the oral mucosa. The aim of the present study was to assess the oral microbiome and periodontal condition in patients with early RA and individuals at risk of developing RA compared to healthy controls. METHODS: Three groups were recruited (n = 50 participants per group): 1) patients with early RA (meeting the American College of Rheumatology/European Alliance of Associations for Rheumatology 2010 classification criteria), 2) individuals at risk of developing RA (those with arthralgia who were positive for RA-ass

Wrist Accelerometry for Physical Activity Measurement in Individuals With Spinal Cord Injury-A Need for Individually Calibrated Cut-Points.

OBJECTIVE: To create and compare individual and group-based cut-points for wrist accelerometry that correspond to moderate-to-vigorous physical activity (MVPA) in people with spinal cord injury (SCI). DESIGN: Participants completed a graded treadmill-wheeling test while being assessed for oxygen consumption, wrist-acceleration vector magnitude, and spoke acceleration. Oxygen consumption was converted to SCI metabolic equivalents (METs), and linear regression was applied to determine an individualized vector magnitude cut-point (counts per minute, VM-CPM) corresponding with MVPA (≥3 SCI METs).

Relation of sexual risks and prevention practices with individuals' stigmatising beliefs towards HIV infected individuals: an exploratory study.

OBJECTIVE: To investigate how an individual's stigmatising beliefs towards people living with HIV are related to his or her own sexual risk and protective behaviours. METHODS: A cross sectional survey was conducted to assess HIV related stigmatising beliefs, risk sexual behaviours, and preventive practices among sexually experienced rural to urban migrants aged 18-30 years in 2002 in Beijing and Nanjing, two large Chinese cities. RESULTS: Among 2153 migrants, 7.2% reported having had more than one sexual partner in the previous month, 9.9% had commercial sex partners, and 12.5% had an episod

Short- and long-term biological variation in analytes in urine of apparently healthy individuals.

Intra-individual, interindividual, and analytical components of variation for each of 10 urine analytes were estimated from results obtained on analysis of urine specimens from 10 apparently healthy young men. In spot urines, urea and osmolality showed strong individuality, but intra-individual variances were larger than interindividual variances for sodium, urate, phosphate, and glucose. Urea and osmolality displayed strong individuality in daily urines, whereas sodium (when results were expressed in output terms), urate, potassium, and total protein (when results were expressed in concentrat…

Blood pressure variability and cardiovascular events in high-risk individuals: individual patient data analysis of SPRINT and ACCORD trials.

BACKGROUND AND AIMS: Blood pressure variability (BPV) is associated with cardiovascular risk and has been shown to confer prognostic information independent of mean blood pressure (BP). However, the consistency of its incremental predictive value across different clinical settings and populations warrants further investigation. A patient-level pooled analysis of two large randomized trials (SPRINT and ACCORD) was conducted to clarify the association between BPV and major cardiovascular events (MCEs). METHODS: Visit-to-visit BPV was calculated from Month 3 onwards using multiple metrics (inclu

Noninvasive Interstitial Glucose Estimation Using Wearables and Machine Learning in Healthy Individuals and Individuals With Obesity: Observational Cohort Study.

BACKGROUND: Continuous glucose monitoring (CGM) can facilitate weight management and lower the risk of metabolic diseases by providing real-time feedback on glycemic responses, thereby enabling more informed lifestyle decisions. However, current CGM systems remain constrained by invasiveness, cost, and short sensor lifespan, limiting their practicality for guiding individualized postprandial low-glycemic diets. OBJECTIVE: Extending earlier proof-of-concept findings, this study aimed to validate an interstitial glucose (IG) machine learning algorithm in real-world environments using multimodal

Individual Cognitive Remediation Therapy and Individual Emotion Skills Training for Anorexia Nervosa: A Case Series Study in a Paediatric Ward.

INTRODUCTION: Adolescents with anorexia nervosa (AN) often present cognitive rigidity, impaired decision-making, and difficulties processing emotions, all of which can impede engagement in treatment. The marked rise in paediatric hospitalisations for AN during the COVID-19 pandemic highlights the urgent need for brief, targeted interventions in inpatient settings. METHODS: This case series explored the feasibility and preliminary effects of combining Cognitive Remediation Therapy (CRT) and Cognitive Remediation and Emotion Skills Training (CREST) for eight female adolescents (12-16 years) adm

HIV-1 late diagnosis: Strategies to overcome the misclassification of individuals acutely infected with HIV-1 as individuals diagnosed late.

OBJECTIVES: Late HIV diagnosis is associated with a higher impact on treatment outcomes and a potential for prolonged transmissibility of HIV-1 infection. The consensus definition for late HIV diagnosis is problematic. It was updated in 2022; however, this definition relies on information that might not be clinically available. This study aimed to assess late HIV diagnosis using alternative parameters, in addition to the definition of clusters of differentiation (CD4) cell count, namely, sequence ambiguity rate and estimated time of infection inferred through phylogenetic analysis. METHODS: C

Comparative analysis of inflammatory markers in HIV-positive individuals on antiretroviral therapy versus HIV-negative individuals in South Africa.

BACKGROUND: HIV is associated with chronic inflammation and immune activation, which can persist even in individuals on antiretroviral therapy (ART), increasing the risk of cardiovascular disease, neurocognitive disorders, and other inflammatory conditions. OBJECTIVE: This study comparatively investigates inflammatory markers among HIV-positive individuals receiving ART with those in HIV-negative individuals. METHODS: We analysed retrospective laboratory results, including viral load, C-reactive protein (CRP), and D-dimer, from 275 individuals (aged 0-39 years) treated at a tertiary hospital

High Mortality Rates Among Individuals Misdiagnosed With Tuberculosis: A Matched Retrospective Cohort Study of Individuals Diagnosed With Tuberculosis in Brazil.

We conducted a matched retrospective cohort study comparing mortality among individuals receiving a false-positive tuberculosis (TB) diagnosis (n = 3701) to individuals correctly diagnosed with TB (n = 8595) in Brazil from 2007 to 2016. Over an average 5.4-year follow-up period, we estimated a mortality rate ratio of 1.95 (95% confidence interval, 1.80-2.11) for individuals incorrectly diagnosed with TB compared to controls. The leading causes of death among the misdiagnosed were malignant neoplasms (40.9%) and respiratory system disorders (15.9%), conditions with symptoms similar to TB. Our f

Enhancing the Impact of Individualized Nutrition Therapy with Real-Time Continuous Glucose Monitoring Feedback in Overweight and Obese Individuals with Prediabetes.

BACKGROUND/OBJECTIVES: prediabetes is a significant risk factor for the development of type 2 diabetes, cardiovascular diseases, chronic kidney disease, and other complications. Early diagnosis of prediabetes, coupled with education on lifestyle changes that support blood glucose management, are crucial for the prevention or delay of type 2 diabetes and related complications. This study aimed to evaluate the impact of incorporating real-time feedback from continuous glucose monitoring (CGM) into individualized nutrition therapy (INT) on blood glucose control in individuals with prediabetes who

Visual fixation patterns to AAC displays are significantly correlated with motor selection for individuals with Down syndrome or individuals on the autism spectrum.

Eye tracking research technologies are often used to study how individuals attend visually to different types of AAC displays (e.g. visual scene displays, grid displays). The assumption is that efficiency of visual search may relate to efficiency of motor selection necessary for communication via aided AAC; however, this assumption has not received direct empirical study. We examined the relation between speed of visual search and speed of motor selection of symbols. Ten individuals on the autism spectrum (AS; Study 1) and nine with Down syndrome (DS; Study 2) participated in a search task usi

[Management of the Sars-Cov2 pandemic in France - Benefit-risk balance at the collective versus individual level individual scale in children].

According to the precautionary principle and facing the initial uncertainty of the potential seriousness of Covid-19, France has adopted collective measures understood as acceptable despite the deprivation of liberty and the known risks of long confinement on mental health. Such measures should be applied proportionately and cause the least possible harm. Among these, the closure of schools was decided by declination of those appearing in response plans to viral pandemics where children play a major role in the transmission of the disease (e.g. flu). In an unprecedented way, measures and const

Cigarette Smoking and Inflammation, Monocyte Activation, and Coagulation in HIV-Infected Individuals Receiving Antiretroviral Therapy, Compared With Uninfected Individuals.

Smoking may affect cardiovascular disease risk more strongly in human immunodeficiency virus (HIV)-infected individuals than HIV-uninfected individuals. We hypothesized that an interaction at the level of the immune system may contribute to this increased risk. We assessed soluble markers of inflammation (high-sensitivity C-reactive protein [hsCRP]), immune activation (soluble [s]CD14 and sCD163), and coagulation (D-dimer) in HIV-infected and uninfected never, former, and current smokers. Smoking was independently associated with higher hsCRP levels and lower sCD163 levels and was borderline s

Roles of inter-individual differences and intra-individual acute elevations in early smoking lapse in people with posttraumatic stress disorder.

Existing models of the role of posttraumatic stress disorder (PTSD) symptoms and smoking have almost exclusively examined mean symptom levels, rather than the acute elevations that might trigger smoking lapse immediately or increase risk of a smoking lapse in the next few hours. We examined ecological momentary assessments (EMA) of PTSD symptom clusters and smoking in the first week of a quit attempt in 52 people with PTSD. In multilevel models including PTSD symptom means, acute elevations, and lagged acute elevations together as simultaneous predictors of odds of smoking in the same models,

Reduction of immunity in HIV-infected individuals: can fibrosis induce hypoplasia in palatine and lingual tonsils of individuals with HIV infection?

The role of tonsils in oral immunity has been described. However, the pathogenesis of HIV infection in these organs is still unclear. The aim of this study is to perform histological and immunohistochemical analysis of the palatine and lingual tonsils of autopsied individuals with or without HIV infection. Twenty-six autopsied individuals with HIV infection (HI) (n=13) and without HIV infection (CO) (n=13) were selected. Palatine and lingual tonsil fragments were collected for histological and immunohistochemical analysis. We found in the HI group a higher frequency of hyaline degeneration in
