Determinants of postoperative morbidity and mortality in children managed for typhoid intestinal perforation in Kano Nigeria.

BACKGROUND: Intestinal perforation is a serious but poorly understood complication of typhoid fever. This study aims to determine the patient factors associated with postoperative morbidity and mortality. METHODS: We retrospectively reviewed the records of all children presenting to our unit with typhoid intestinal perforation (TIP) between March 2009 and December 2013. The patients were grouped based on postoperative outcome status and were compared with respect to patient related variables, using chi square test. Multivariate analysis was performed using a binary logistic regression model.

Case Report: A Giant Gallstone Was Found at End of the Duodenum.

Gallstone ileus (GI) occurs due to the migration of gallstones into the intestinal tract through a bilioenteric fistula. The surgical approach may vary based on the size and location of the fistula causing obstruction. We present a case of recurrent calculous cholecystitis with acute abdominal pain and vomiting, which was attributed to an uncommon giant gallstone located at the distal end of the duodenum. Our management involved performing a one-stage enterolithotomy, cholecystectomy, and fistula repair. The patient had an uneventful postoperative recovery without any complications such as duo

Malignant colo-duodenal fistula; case report and review of the literature.

BACKGROUND: Colo-duodenal fistula is a rare complication of malignant and inflammatory bowel disease. Cases with malignant colo-duodenal fistulae can present with symptoms from the primary, from the fistula or from metastatic disease. The fistula often results in diarrhoea and vomiting with dramatic weight loss. Upper abdominal pain is usually present as is general malaise both from the presence of the disease and from the metabolic sequelae it causes. The diarrhoea relates to colonic bacterial contamination of the upper intestines rather than to a pure mechanical effect. Vomiting may be faecu

Metronidazole therapy for Crohn's disease and associated fistulae.

Because of reports suggesting the efficacy of metronidazole therapy in the healing of perianal fistulae in Crohn's disease, the effect of metronidazole on symptoms of Crohn's disease and associated perianal fistulae were evaluated in eight consecutive patients. The criterion for entry into the study was the presence of resistant fistulae in patients with Crohn's disease. The duration of intestinal Crohn's ranged from 1/2-30 years and fistulae from 1-14 years. All patients had ileocolitis or pancolitis. Two had enteroenteric fistulae in addition to perianal fistulae. All had had previous surger

Management of internal fistulas in Crohn's disease.

Internal fistulas occur in 5-10% of patients with Crohn's disease. The clinical presentation of each of the three main types of internal fistulas--enteroenteric, enterovaginal, and enterovesical fistulas--is important in determining the best management. Asymptomatic fistulas usually require no treatment, but fistulas that cause severe or persistent symptoms necessitate intervention. Previously regarded as a surgical condition requiring resection, some internal fistulas are amenable to a more conservative approach involving medical therapy, surgical repair, or both. So far, there have not been

The radiological features of chronic radiation enteritis.

The radiological findings, using a single-contrast barium infusion technique, are described in a series of 13 patients with chronic radiation enteritis. The signs include evidence of submucosal thickening, single or multiple stenoses, adhesions and sinus or fistula formation. A combination of these signs characterises the condition. This technique is particularly suited to the investigation of radiation enteritis because of its ability to distend maximally the small intestine. A cause, stenosis and/or adhesions, was demonstrated in the eight of the 13 patients presenting with intermittent smal

Effect of duodenal feeding on serum gastrin and acid response to pentagastrin in cats.

Five cats were provided with a gastric fistula (GF) and a Heidenhain pouch (HP), and the proximal small intestine was excluded and provided with a duodenal fistula. A protein-rich meal, introduced into the duodenum (duodenal feeding) caused no serum gastrin increase, which supports our previous conclusion that duodenal gastrin is of no significance for the postprandial acid secretion in cats. However, duodenal feeding potentiated the maximal GF and HP acid responses to pentagastrin, suggesting that the proximal small intestine is implicated in the postprandial acid stimulation in cats via a hu

Colonic duplication: an unusual cause of enterovesical fistula.

Intestinal duplications are rare, occurring in 1 in 4000 births. Even rarer are tubular hindgut duplications, with less than 75 cases reported in the literature to date. We report an uncommon case of an enterovesical fistula caused by a duplication of the sigmoid colon. This is the first reported case of a tubular colonic duplication presenting as a colovesical fistula in an adult. We discuss the presentation and shortcomings in the diagnosis and treatment of enterovesical fistulas and colonic duplications.

[The use of the synthetic silver coated prosthesis and penduculated omentum flap in the treatment of prostheses infection].

There is a methods of surgical treatment of secondary enteroaortal fistula after infection of vascular prostheses with the use silver prosthesis and great omentum. Secondary enteroaortal fistula is the most serious complication followed infection of vascular prosthesis, with high risk of death rate regardless of different operative methods. Patients is operated and the dracon prosthesis was exchanged to arterial homograft, which is more resistant to infections or subclavio-femoral bypasses. Removing of intestinal fistula included resection of duodenum and gastroenterotomy or duodenorrhaphy. Th…

Short- and Long-Term Intestinal Complications After Combined Pancreas-Kidney Transplantation.

BACKGROUND: Simultaneous pancreas-kidney (SPK) transplantation is the treatment of choice in patients with type 1 diabetes and end-stage renal disease, because it improves survival and quality of life. Currently, enteric exocrine drainage is the most commonly used method. Intestinal complications continue to be a major cause of posttransplant morbidity despite improvements in surgical technique. This study analyzed early and late intestinal complications related to SPK transplantation. MATERIALS AND METHODS: We performed a retrospective analysis of 100 adult patients undergoing SPK transplant

Bursectomy in gastric cancer surgery: surgical technique and operative safety.

Although there is little evidence that bursectomy has clinical benefit, its continuing practice imposes evaluation of bursectomy-related adverse effects, especially pancreatic fistula and intestinal obstruction. The aims of this study were to provide a detailed description of the technique of bursectomy as standardized by the authors and determine if extended surgery for gastric cancer with additional bursectomy can be performed safely in Western population. A total of 72 consecutive patients of median age 76.4 years and mean ASA score grade 2.1, who submitted to D2 or D2+ gastrectomy with add

[Endoscopic closure of a chronic colonic fistula using the over-the-scope clip (OTSC)].

HISTORY AND ADMISSION FINDINGS: A 64 year old male patient suffered from recurrent subphrenic abscesses following a complicated postoperative course after sigmoidectomy for chronic recurrent diverticulitis. Two previous attempts of abscess treatment by transcutaneous drainage had failed. Radiographic studies eventually showed a fistula of the descending colon leading to the abscess formation, which could be identified and confirmed endoscopically by installation of dye. TREATMENT AND COURSE: Endoscopic application of an "over-the-scope clip" (OTSC) onto the anchor-retracted enteric fistula or

Metabolic acidosis during urinary retention in a patient with an enterovesical fistula.

We report a patient known to have an enterovesical fistula who presented severe acute metabolic acidosis during an episode of urinary retention. The enterovesical fistula which had been intermittently symptomatic for 4 years, had developed after several intestinal surgical procedures and related intraperitoneal sepsis following resection of colon cancer 21 years previously. The patient who had a total colectomy and ileostomy, was admitted for hip replacement with the routine placement of a Foley bladder catheter. Three weeks post-operatively, the patient developed acute urinary retention follo

Iliac Artery-Uretero-Colonic Fistula Presenting as Gastrointestinal Hemorrhage and Hematuria: A Case Report.

Background: The experience with uretero-arterial fistulas has been limited. However, the aggressive treatment of pelvic tumors with surgical resection and radiotherapy, along with liberal use of ureteral catheters, has been attributed to an increase in their incidence. Unless they are promptly diagnosed and treated, uretero-arterial fistulas are associated with considerably high rates of morbidity and mortality. Urologists need maintain a high degree of suspicion for uretero-arterial fistula in high-risk patients. We herein present the clinical course of an iliac artery-uretero-colonic fistula

Live ascaris in urinary bladder: a case report.

INTRODUCTION: Ascaris in urinary bladder is an extremely rare phenomenon. It may occur after fistula formation between urinary and gastrointestinal tract or by retrograde migration of adult worm, and is associated with complications. CASE PRESENTATION: A 47-year-old Amhara woman from rural northwest Ethiopia presented with a complaint of difficulty to fully evacuate her bladder of 1 year duration. Ultrasonography showed thickened bladder wall with echo debris. There were also thickened bowel and fluid-filled loops of intestine adjacent to urinary bladder. On cystoscopy examination, there was
