Surgical ultrasound-guided carpal tunnel release.

Ultrasound-guided surgery is a new trend stemming from the introduction of high-frequency linear probes and better quality screens. Surgical treatment of carpal tunnel syndrome is increasingly being performed under ultrasound guidance. Knowledge of musculoskeletal ultrasonography is obviously mandatory. Several types of cutting instruments (miniature knife, Gigli saw or hook) and surgical approaches (wrist or palm) have been described. Ultrasonography allows the wide-awake and local anesthesia with no tourniquet (WALANT) technique to be used in the context of ambulatory patient care. The pract

A rare case of a spontaneous neck hematoma in a patient with type 1 neurofibromatosis.

Neurofibromatosis type 1 (NF-1) is a genetic disorder that affects one in 3000 individuals. Although NF-1 notably involves nerves and connective tissue, vascular involvement in large series is estimated to range from 0.4% to 6.4%. Jugular vein involvement in these patients is rare. Spontaneous neck hematomas and hemorrhages are also unusual. We present a case of a NF-1 patient with a spontaneous neck hematoma with possible leakage from the left internal jugular vein, presenting as a lateral neck mass. The fragility of the vein wall and the surrounding tissue led patient to a severe intraoperat

Postoperative Acute Submandibular Sialadenitis after Neurosurgery: Two Case Reports and a Review of the Literature.

This article details two case reports and reviews the extant literature concerning acute postoperative submandibular sialadenitis occurring in the side contralateral to the operated site after neurosurgery. Although its precise pathogenesis remains to be elucidated, it is likely related to intraoperative compression of the submandibular gland and surrounding tissues, caused by head positioning and the endotracheal tube. Submandibular swelling rapidly deteriorates after surgery, and emergent airway protection is required in most cases in order to avoid fatal airway obstruction. To avoid serious

Arthroscopic management of anterior instability: pearls, pitfalls, and lessons learned.

Despite advances in the understanding of anterior shoulder instability, failure rates after open and arthroscopic surgery have been reported to be as high as 30%. In general, a successful operative outcome for patients with shoulder instability requires the surgeon to perform a complete preoperative evaluation, a thorough diagnostic arthroscopy to evaluate for concomitant co-pathology, and implement an effective postoperative therapy program tailored to the repair strategy. In addition to the Bankart lesion, the treating surgeon must be aware of other co-pathologies, such as the HAGL lesion, A

[Adenomatoid tumor of the testis with testicular pain: a case report].

A 30-year-old man who complained of sudden right scrotal pain visited our clinic. Physical examination revealed a palpable mass with tenderness on his right testis. An emergent operation was performed for acute scrotum. The intraoperative findings showed a small, white, elastic solid, a smooth surface tumor that originated from the tunica albuginea of the right testis. It seemed to be benign macroscopically and partial orchiectomy was performed. Soon after surgery, the pain disappeared. Pathohistological examination revealed an adenomatoid tumor of the testis which originated from the rete tes

[Nonocclusive ischemia of the right colon].

BACKGROUND: Nonocclusive disease (NOD) is known to be a common cause of ischemic colitis, which is frequently underestimated. MATERIAL AND METHODS: A computer-assisted search of radiological reports at our institute over a period of 18 months, describing ischemic colitis of the ascending colon with an unimpaired perfusion of the superior mesenteric artery, was performed. RESULTS: A retrospective analysis of the clinical and radiological data of 14 patients was performed. In ten cases colonic ischemia was confirmed clinically or intraoperatively. Most of our patients needed intravenous catech

Tumor-like cystic endosalpingiosis of the uterus with florid epithelial proliferation. A case report.

Mass-like cystic endosalpingiosis is very rare. The author reports such a lesion with histologic features of endosalpingiosis of the uterus with florid epithelial proliferation in a 51-year-old female who complained of lower abdominal pain. Preoperatively, it was considered to be left-sided ovarian cancer. Intraoperatively, a subserosal, sessile polypoid mass with multiple cysts in the uterine fundus measuring 12 x 6.5 x 5.5 cm was found. Histologically, it was composed of multiple cysts lined by benign-appearing tubal epithelium and bland smooth muscular and myofibromatous stroma. Part of the

Coconut atrium: transmural calcification of the entire left atrium.

Massive calcification of the left atrium usually spares the interatrial septum, which provides a cleavage plane for surgical access to the mitral valve. Endoatriectomy with mitral valve replacement is the currently accepted corrective procedure because it affords maximum exposure while decreasing the risk of embolization and intraoperative hemorrhage. We describe a case in which the entire left atrium, including the septum, was thickly calcified and resembled a coconut shell. This condition prevented surgical correction of severe mitral stenosis. To our knowledge, this is the most severe case

Vernix caseosa: an unusual cause of post-cesarean section peritonitis.

Surgeons should be aware of a very unusual cause of post-cesarean section peritonitis. This rare form of peritonitis is caused by the spillage of vernix caseosa material into the peritoneal cavity. The exact mechanism of the development of peritonitis in these few cases is unknown. Clinically, vernix caseosa peritonitis should be considered in the post-cesarean section acute abdomen after excluding the more common causes of peritonitis such as appendicitis and perforated viscus. Intraoperatively, the presence of cheese-like yellow patches limited to the serosal layer of visceral organs is the

Antegrade Jejunal Intussusception at the Bilopancreatic Limb Jejunojejunostomy After Laparoscopic Distal Gastrectomy With Roux-en-Y Reconstruction: A Case Report.

Jejunal intussusception is a rare but potentially serious complication of gastric surgery, typically involving the gastrojejunostomy or alimentary limbs. A 75-year-old woman underwent laparoscopic distal gastrectomy with D1+ lymphadenectomy and Roux-en-Y reconstruction for early-stage gastric cancer. Postoperatively, she developed intermittent vomiting every 10 days without identifiable triggers and was managed conservatively. However, transient jejunal intussusception involving the bilopancreatic limb was later identified at the jejunojejunal site. Although the intussusception resolved sponta

Surgery should not hurt: A look into the ergonomics of orthopedics.

Orthopedic surgery is among the most physically demanding medical specialties, exposing surgeons to prolonged static postures, repetitive high-force movements, and heavy equipment that significantly increase the risk of musculoskeletal injury. Historical perspectives reveal that ergonomic principles have been recognized since antiquity, yet modern surgical practice continues to neglect them, resulting in high rates of pain, disability, and even premature retirement among orthopedic surgeons. Recent survey data demonstrate that musculoskeletal disorders affect surgeons across all subspecialties

Hypertensive crisis during embolization unveils occult catecholamine-secreting glomus jugulare tumor: A case report and management protocol.

Glomus jugulare tumor (GJT) is a rare paraganglioma arising from neural crest cells. Although the majority of cases are non-functional, a minority of GJTs can secrete catecholamines. Such functional variants are frequently overlooked due to their rarity and may trigger perioperative crises. The present report documents a 53-year-old female patient with a giant occult secretory GJT (65 mm) presenting with prolonged cranial nerve deficits. Preoperative blood pressure was normal, but the patient developed hypertensive crisis (220/130 mmHg) during embolization. Biochemical tests confirmed catechol

Anesthetic management of pheochromocytoma complicated by Takotsubo syndrome: A case report about a dual challenge.

INTRODUCTION AND IMPORTANCE: Anesthetic management of pheochromocytoma is challenging due to the risk of severe hemodynamic instability. Recent evidence suggests an association with Takotsubo syndrome, complicating perioperative care. Awareness of this complication is crucial for optimizing management and improving outcomes. CASE PRESENTATION: We report a 42-year-old female undergoing preoperative evaluation for an adrenal mass, diagnosed as pheochromocytoma. Echocardiography revealed dilated cardiomyopathy consistent with Takotsubo syndrome. Preoperative optimization included alpha- and beta

Inflamed Amyand's Hernia in an Elderly Male: A Rare Surgical Surprise.

Amyand's hernia is a rare anatomic occurrence, and appendicitis in Amyand's hernia makes it exceptionally rare. We present such a rare case that was unexpectedly found during right inguinal hernia repair, highlighting the rarity and challenges in diagnosing this condition preoperatively, as well as the importance of awareness and proper management of this rare case. A 68-year-old male presented with right inguinal pain, tenderness, and swelling in the right inguinal region, initially admitted for right inguinal hernia repair. Surgery was postponed due to poor glycemic control. The patient was

A Fatal Case of a Ruptured Posterior Communicating Artery Aneurysm in a Patient With Suspected Klippel-Feil Syndrome.

Klippel-Feil syndrome (KFS) is a rare congenital disorder characterized by cervical vertebral fusion and often associated with multisystem anomalies. Although primarily skeletal, KFS has been linked to vascular abnormalities, including arterial dissections, agenesis, and aneurysms. We present the case of a 33-year-old previously healthy woman who presented with diffuse subarachnoid hemorrhage. Imaging revealed a ruptured 5.5 mm posterior communicating artery aneurysm arising from the left supraclinoid internal carotid artery, along with congenital cervical vertebral fusion consistent with undi

Accurate Machine Learning-based Monitoring of Anesthesia Depth with EEG Recording.

General anesthesia, pivotal for surgical procedures, requires precise depth monitoring to mitigate risks ranging from intraoperative awareness to postoperative cognitive impairments. Traditional assessment methods, relying on physiological indicators or behavioral responses, fall short of accurately capturing the nuanced states of unconsciousness. This study introduces a machine learning-based approach to decode anesthesia depth, leveraging EEG data across different anesthesia states induced by propofol and esketamine in rats. Our findings demonstrate the model's robust predictive accuracy, un…

Living Liver Donors With Harris Platelet Syndrome Related Thrombocytopenia: A Case Series.

Donor safety is of paramount importance for living-donor liver transplantation. Donor thrombocytopenia is viewed with caution and considered a relative contraindication for donations. Harris platelet syndrome (HPLS), a recently described cause of asymptomatic thrombocytopenia, is a subtype of inherited giant platelet disorders noted more commonly in the north-eastern part of India. Herein, we present our experience with living liver donors using HPLS. We retrospectively collected data from all patients with HPLS who underwent donor hepatectomies. Five patients were diagnosed as having HPLS. Th

[Consensus of Chinese Experts on Surgical Treatment of Lung Cancer in the Elderly
(2022 Edition)].

Lung cancer ranked the highest incidence rate and mortality among people over 60 years old in China. With the increasing social population and incidence of lung cancer, the treatment of elderly lung cancer patients has become a major concern. Due to the improvement of surgical techniques and the application of enhanced recovery after surgery in thoracic surgery, more elderly patients could tolerate surgical treatment. At the same time, with the improvement of health awareness and the popularization of early diagnosis and screening, more lung cancer can be detected at the early stage. However,

IgG4-related Pachymeningitis as a Cause of Spinal Epidural Compression: Can Intraoperative Frozen Sections Predict the Underlying Pathology?

At times, spinal lesions are part of systemic manifestations of autoimmune disease. Awareness regarding their clinicopathological spectrum, particularly the lesions which usually respond to steroids/immunotherapy, is essential to avoid unwanted surgical morbidity. We discuss a case of a young-man presenting with thoracic spinal epidural compressive lesion which was indeed a manifestation of IgG4-related hypertrophic pachymeningitis. The mass was firmly adherent to the dura and extended into left neural foramen/paravertebral space which precluded complete excision. Frozen sections suggested fib

Delayed Presentation of Gastric Volvulus with Gastric Outlet Obstruction after Laparoscopic Sleeve Gastrectomy.

BACKGROUND: Globally among bariatric procedures, SG popularity has grown significantly. New complications arise as a result of the rapid growth in the numbers of LSG. Once SG has been performed, the stomach is left with no fixations along the entire greater curvature, which may predispose to volvulus. Case Report. The 44-year-old patient had laparoscopic sleeve gastrectomy 2 years and 6 months before presentation in our emergent department complaining of episodic intolerance to oral intake for 3 months before presentation. Gastrografin swallow study showed gastric outlet obstruction. The patie
