We report the case of a male, small-for-gestational-age newborn who presented with failure to thrive, severe fluctuation of blood glucose concentrations, and increased serum concentrations of galactose. The infant responded well to a lactose-free diet supplemented with fructose, inulin and corn starch. The metabolic disorder disappeared within 6 months. The transient course, and results of a molecular analysis of the glucose transporter 2 (Glut2) gene seem to rule out Fanconi-Bickel syndrome.

Secondary hyperparathyroidism is a major contributor to skeletal fragility in chronic kidney disease, yet fracture risk remains high despite parathyroid hormone-lowering therapy. In a rat model of chronic kidney disease, Allen et al. showed that combining the prebiotic fiber inulin with a calcimimetic further suppresses elevated bone turnover while improving cortical bone architecture and mechanical properties. These findings highlight the potential of gut-targeted interventions to complement established parathyroid hormone-lowering strategies for improving skeletal health.

Renal extraction of gastrin in patients with normal clearance of inulin and p-aminohippuric acid was assessed by gastrin radioimmunoassay. In 23 fasting patients there was no significant difference in arterial gastrin (37.7 pg/ml serum +/- 25.1 S.D.) and the renal vein gastrin (35.9 +/- 24.6), resulting in a renal extraction ratio for gastrin not significantly different from zero (p greater than 0.5). The results in three patients indicated no extraction of gastrin for two consecutive periods after a protein-rich meal.

Much controversy exists over the performance of elderly living donor kidney transplantation. We report the safety of 2 cases of elderly living kidney donations in our hospital. CASE 1: An 82-year-old man was a living kidney donor for his 56-year-old son. The donor suffered from hypertension, but has successfully managed his blood pressure with only one medication.
