BACKGROUND: Crigler-Najjar syndrome is a rare metabolic disorder characterized by severe unconjugated hyperbilirubinemia resulting in defici … ional phototherapy was used in all patients. A fatal out come was observed in 28 case; they died of kernicterus. CONCLUSION: Crigler-Najjar syndrome is a serious disorder which, when not treated, ultimately leads to brain damage (bilirubin encephalopathy) and death. That's why we must promote prenatal diagnosis and genetic council especially because of the b …

Retrovirus-mediated expression of HUG Br1 in Crigler-Najjar syndrome type I human fibroblasts and correction of the genetic defect in Gunn rat hepatocytes.

Crigler-Najjar syndrome type I (CN-I) is a congenital hepatic metabolic deficiency in bilirubin UDP-glucuronosyltransferase activity which leads to profound jaundice and death from kernicterus. UGT1, the gene locus coding for multiple glucuronosyltransferase isoforms, has been well characterized and the cDNA for the most active form, HUG Br1, has been cloned. Recent advances in liver directed gene transfer suggest that this disease could be treated through gene therapy. As an initial step to correct the genetic defect in Crigler-Najjar type I, recombinant retroviruses were used to transduce an…

Liver Transplantation Outcomes in Crigler-Najjar Syndrome in Iran: A Single-Center Retrospective Cohort Study Over 20 Years.

BACKGROUND AND AIMS: Crigler-Najjar Syndrome (CNS) is a rare autosomal recessive disorder caused by uridine diphosphate glucuronosyltransferase (UGT1A1) deficiency, leading to unconjugated hyperbilirubinemia. Without treatment, patients are at high risk of kernicterus and irreversible neurological damage. Liver transplantation (LT) is the only curative option. This study aims to evaluate long-term outcomes of LT in CNS patients over a 20-year period in Iran. METHODS: In this retrospective cohort study, we analyzed 69 patients with CNS who underwent LT between 2000 and 2023 at Namazi Hospital …

Evaluation and treatment of neonatal hyperbilirubinemia.

Although neonatal jaundice is common, acute bilirubin encephalopathy and kernicterus (i.e., chronic bilirubin encephalopathy) are rare. Universal screening for neonatal hyperbilirubinemia is controversial. The American Academy of Pediatrics recommends universal screening with bilirubin levels or targeted screening based on risk factors. However, the U.S. Preventive Services Task Force and the American Academy of Family Physicians found insufficient evidence that screening improves outcomes. Universal screening may also increase rates of phototherapy, sometimes inappropriately. Younger gestatio…

Development and validation of serum bilirubin nomogram to predict the absence of risk for severe hyperbilirubinaemia before discharge: a prospective, multicenter study.

BACKGROUND: Early discharge of healthy late preterm and full term newborn infants has become common practice because of the current social and economic necessities. Severe jaundice, and even kernicterus, has developed in some term infants discharged early. This study was designed to elaborate a percentile-based hour specific total serum bilirubin (TSB) nomogram and to assess its ability to predict the absence of risk for subsequent non physiologic severe hyperbilirubinaemia before discharge. METHODS: A percentile-based hour-specific nomogram for TSB values was performed using TSB data of 1708…

Risk factors and clinical profiles in Turkish children with cerebral palsy: analysis of 625 cases.

The aim of the study was to investigate risk factors, clinical profiles and gross motor function levels of Turkish children with cerebral palsy (CP). A total of 625 consecutive children with CP, who were rehabilitated in the pediatrics rehabilitation clinic between 2000 and 2004 years, were included. Factors causing CP were investigated by interviewing the families and by scanning medical files. Risk factors were recorded as consanguineous marriage, maternal disorder, preterm birth, birth asphyxia, low birth weight, multiple pregnancy, neonatal convulsion, kernicterus, postnatal central nervou…

Evaluating the healing effects of docosahexaenoic acid in neonates with bilirubin-induced brain injury.

BACKGROUND: Neonatal brain injury due to bilirubin toxicity presents critical need for effective healing treatments. Docosahexaenoic acid (DHA), having neuroprotective properties, offers potential therapeutic benefits in promoting brain repair and recovery. OBJECTIVES: This study focused on evaluating the healing capabilities of DHA in neonatal brains damaged by bilirubin-induced injury, with particular attention to its role in enhancing brain tissue repair mechanisms. METHODS: Employing the bilirubin encephalopathy model in neonatal Sprague-Dawley rats and neuronal cell cultures, we investi

A clinical prediction rule for acute bilirubin encephalopathy in neonates with extreme hyperbilirubinemia: A retrospective cohort study.

To establish a clinical prediction rule for acute bilirubin encephalopathy (ABE) in term/near-term neonates with extreme hyperbilirubinemia.A retrospective cohort study was conducted between January 2015 and December 2018. Six hundred seventy-three out of 26,369 consecutive neonates with extreme hyperbilirubinemia were enrolled in this study. Data included demographic characteristics, total serum bilirubin (TSB), albumin, bilirubin/albumin ratio (B/A), direct antiglobulin test, glucose-6-phosphate deficiency, asphyxia, sepsis, acidosis. ABE was defined as a bilirubin induced neurological dysfu

Influence of hypoxia and ischemia preconditioning on bilirubin damage to astrocytes.

Hypoxia-ischemia in the perinatal period is a common cause of neurologic disability in children and is often associated with neonatal morbidity and mortality. Another frequent condition of the newborn is hyperbilirubinemia and it is well known that deposition of unconjugated bilirubin (UCB) in the central nervous system can damage nerve cells and cause encephalopathy. Interestingly, some studies report the onset of cerebral hypoxia-ischemia as a risk factor for UCB encephalopathy, since that condition often precedes neonatal hyperbilirubinemia. However, the cellular mechanisms triggered by hyp

[Developmental diseases of the central nervous system caused by neonatal hyperbilirubinemia].

Neuropsychological development was evaluated in 71 infants. Groups: G I, healthy full term infants; G II, premature infants; G III, sick newborns. In these groups, total bilirubin (TB), free bilirubin (FB) and bilirubin binding capacity (BBC) of albumin were determined during the neonatal period. TB and FB concentration were determined by Brodersen and Bartels method. Neurological evaluation was estimated by physical exam, EEG, ophthalmological and audiometric exam. Psychological evaluation was done by Brunet-Lezine test. Infants were subdivided in three groups in relation to the degree of enc

Use or abuse of phototherapy for physiological jaundice of newborn infants.

To investigate the need for and effects of phototherapy in full-term otherwise healthy babies with physiological jaundice, 40 consecutive babies with serum bilirubin levels of 250 mumol/l or more were assigned at random to two treatment groups. Phototherapy was started in the early group (n = 20) when serum bilirubin was 250 mumol/l and in the late group (n = 20) when serum bilirubin reached 320 mumol/l; however, only 3 of the late group required treatment. Phototherapy prevented a further rise in bilirubin in almost all treated babies, but the difference in peak bilirubin level between early
