[Pregnancy and lithiasis of the upper urinary tract. Clinical aspects and therapeutic management].

Upper urinary tract calculi can be difficult to diagnose and to treat. One has to be aware that there is a risk for the mother which is often not recognized in the long term. The authors describe 17 cases of pregnant women aged between 20 and 33 who were treated for calculi in the Urological Service in Poitiers. They were diagnosed at different stages of pregnancy and a few had a previous urological history. The women presented in different ways, several of them with urinary colic and 10 with urinary colic and fever. Urinary tract infection and septicaemia also occurred. Six patients passed th

Advanced Management of Complex Transplant Lithiasis: Low Lithotomy and Boot Stirrups Technique.

BACKGROUND Transplant lithiasis may be rare but poses significant risk to the renal graft function of the recipient. Immediate management is necessitated upon first detection, to prevent further complications. CASE REPORT We report 2 cases of transplant lithiasis that were not treated immediately upon first detection. The first patient was a 42-year-old man that received a living-donor kidney from his mother, which was complicated by ureterovesical anastomotic stenosis, BK polyomavirus infection, and oliguria. He had a renal stone and 4 ureteric stones in his right allograft. The second patien

Treatment of ureteropelvic junction obstruction in patients with renal calculi via laparoscopic pyeloplasty and flexible vacuum-assisted ureteral access sheath ureteroscopy: a multicenter retrospective observational study.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) is a common obstructive disease of the urinary tract. UPJO patients commonly exhibit coexistent renal calculi. The main aim of therapy is to relieve the obstruction and remove the stones at the same time. METHODS: This retrospective study included 110 patients diagnosed with UPJO coexisting with multiple renal calculi at Shanxi Bethune Hospital and the First Hospital of Shanxi Medical University between March 2016 and January 2022. Patients were divided according to the methods used for dealing with UPJO and renal calculi. In Group A, pati

Which is the best treatment of pediatric upper urinary tract stones among extracorporeal shockwave lithotripsy, percutaneous nephrolithotomy and retrograde intrarenal surgery: a systematic review.

BACKGROUND: Although the indications of minimally invasive treatments for pediatric urolithiasis are similar to those in adults, it is still crucial to make the right treatment decision due to the special considerations of children. This review aims to evaluate the efficacy and safety of extracorporeal shockwave lithotripsy (SWL), percutaneous nephrolithotomy (PCNL), and retrograde intrarenal surgery (RIRS) in the management of pediatric upper urinary tract stones. METHODS: EMBASE, PubMed, and the Cochrane Library were searched from their first available date to March 2018. The studies that m

Risks of flexible ureterorenoscopy: pathophysiology and prevention.

Currently, indications for flexible ureterorenoscopy (fURS) are expanding, mainly due to technological advancements. Although data from clinical series definitely presents fURS as a safe procedure, serious complications including sepsis and ureteral lesions do occur. These complications seem to be a result of the unique elements of fURS, ureteral access and irrigation, pushing normal upper urinary tract physiology into pathophysiological processes, including intrarenal/pyelo-veneous backflow and ureteral contractions, potentially resulting in septic, haemorrhagic and ureteral lesional complica

Efficacy of extracorporeal shock wave lithotripsy for isolated lower caliceal stones in children compared with stones in other renal locations.

OBJECTIVES: To evaluate the efficacy of extracorporeal shock wave lithotripsy (ESWL) monotherapy for isolated lower caliceal calculi in a pediatric age group and compare it with that for isolated middle/upper caliceal and renal pelvic calculi. METHODS: We retrospectively reviewed the data of 151 renal units in 126 children treated with ESWL for isolated caliceal and renal pelvic stones from March 1992 to February 2004. The stones were localized in the lower, middle/upper calices, and renal pelvis in 50, 26, and 75 renal units, respectively. The results were compared with respect to renal loca

"Mirror pain" as an unusual presentation of renal colic.

Although most patients suffering from pain related to upper urinary tract stones feel the discomfort on the same side as the stone, rarely it is perceived on the opposite side. We sought to identify the prevalence of this clinical scenario and to review possible explanations. The charts and x-rays of all patients with unilateral, symptomatic, radiologically identifiable upper urinary tract stones, seen at an outpatient clinic between June 1993 and August 1996, were reviewed retrospectively in terms of the side of the discomfort in comparison to the side of the stone. Three of 631 patients pres

Verrucous carcinoma of the renal pelvis: case presentation and review of the literature.

Squamous cell carcinoma (SCC) of the renal pelvis is an uncommon tumour that has occasionally been associated with horseshoe kidney. The verrucous form of well-differentiated SCC has not been described previously at this site. We describe such a tumour in a 41-year-old man, who presented with gross haematuria and recurrent pyelonephritis caused by staghorn calculi within a horseshoe kidney. Histology showed extensive keratinising squamous metaplasia of the pelvic urothelium with an area of verrucous acanthosis and underlying invasion of the pelvic smooth muscle by broad tongues of squamous epi

Xanthogranulomatous pyelonephritis masquerading as occult malignancy.

Xanthogranulomatous pyelonephritis (XGP) can present with weight loss, anemia, leukemoid reaction, and generalized debility; there may be no signs or symptoms referable to the urinary tract. Confusion between XGP and renal adenocarcinoma is well recognized, but other malignancies can also be simulated. Case histories of patients with proved XGP whose clinical presentations suggested occult malignancies are recorded. Proteus urinary tract infection, calculi, and a nonvisualizing kidney on intravenous pyelogram should suggest the correct diagnosis. The pathology, bacteriology, diagnosis, and tre

Modification of tubeless percutaneous nephrolithotomy with a ureteropelvic junction stent.

INTRODUCTION: To introduce the ureteropelvic junction stent as a safe and effective modification to tubeless percutaneous nephrolithotomy for select patients to maintain antegrade access to the collecting system. MATERIALS AND METHODS: From April 2014 to December 2015, 31 patients underwent modified tubeless percutaneous nephrolithotomy with ureteropelvic junction (UPJ) stent left in situ and an extraction string coming out the nephrostomy tract. Primary study endpoints included complications, emergency department visits, or re-admissions. Secondary endpoints were perioperative parameters inc

Emphysematous pyelonephritis in a diabetic patient with pelvic-ureteric stone.

Emphysematous pyelonephritis is a rare but life threatening infection of kidney that is characterized by gas formation within or around the kidney. We describe a case of emphysematous pyelonephritis as a consequence of inadequately treated pelvic-ureteric stone in a diabetic man and complement this with a brief review of the literature pertaining in particular to the management of this condition.

CT scanning in the diagnosis and management of radiolucent urinary calculi.

The diagnosis of non-opaque calculi can be difficult. In 4 patients with protracted symptoms, 2 had renal pelvic filling defects, 1 a possible filling defect and 1 complete obstruction of the ureter at L4. Non-opaque calculi were readily diagnosed by CT scanning. Three patients have successfully undergone surgery (2 open, 1 percutaneous) and 1 awaits percutaneous nephrolithotomy. Of two stones available for analysis, one was composed of uric acid and the other calcium and phosphate. We recommend CT scanning as the investigation of choice for unexplained filling defects in the renal pelvis and

Kidney stones: natural remedies and lifestyle modifications to alleviate their burden.

BACKGROUND: Kidney stones (KSs), in fact, have been considered one of the most ancient and prevalent medical conditions that impact a significant number of human beings all around the world. Such stones can range greatly in size and can be detected in any part of the urinary system, including the kidneys, ureters, or bladder itself. The development of stones is caused by the mineral's crystallization, which then interacts with each other and adheres together. Kidney stone formation can represent a prime medical condition for which there are numerous therapies available, among them natural ones

Nephrolithiasis in a giant Pacific octopus Enteroctopus dofleini.

Managed care of cephalopods can be complicated by numerous factors including infectious and non-infectious disease, environmental stressors, and anatomic and physiological changes associated with senescence. The current report describes a unique case of nephrolithiasis in a senescent, >2 yr old female Pacific octopus Enteroctopus dofleini housed in a public aquarium. Clinical signs included generalized external pallor, inappetence progressing to anorexia, lethargy, and a slow healing mantle abrasion over a period of a year. Due to the animal's declining condition, humane euthanasia was elected

Evaluation of management of patients with postoperative permanent hypoparathyroidism. How close are we to the targets?

BACKGROUND: Postoperative hypoparathyroidism (PO-HypoPT) is a complication usually seen after thyroid surgery. PO-HypoPT, which lasts longer than 6 months is defined permanently. The aim of this study was to evaluate how close permanent PO-HypoPT patients can approach target values. METHODS: One hundred seven patients who were followed-up with permanent diagnosis of PO-HypoPT between 2016-2020 were included in the study. The study protocol includes serum albumin corrected total calcium (Alb-sCa), phosphate (P), Ca-P product, and 24 h urine calcium measurements. Laboratory measurements of the
