GIST. Duodenopancreatectomía. Tumor neuroendocrino no funcionante.

Zollinger Ellison syndrome is an unusual entity. This termn is used to describe the clinical manifestations of a gastrin-synthesizing neoplasm. Gastrinomas occur mainly in the duodenum and pancreas. Primary gastrinomas are rarely found in other intra-abdominal sites, such as the ovary, bile ducts, spleen or kidney, or even more unusual in extra-abdominal locations. Several studies provide strong evidence that gastrinomas can also occur in the lymph nodes. However, the existence of primary lymph node gastrinomas is controversial.

A Case Report of Successful Kidney Transplantation in a Patient With Chronic Myelogenous Leukemia (CML) Who Has Been in Remission for 15 Years on Imatinib.

For chronic myeloid leukemia (CML), a Philadelphia chromosome-positive myeloproliferative neoplasm, the introduction of tyrosine kinase inhibitors has transformed CML from a lethal disease into a manageable chronic disease with a close-to-normal life expectancy. Active malignancy is an absolute contraindication to kidney transplantation. However, it is controversial whether kidney transplantation can be safely performed in patients with a history of CML who are in remission. We describe the clinical course of a 64-year-old male patient with chronic kidney disease from diabetic nephropathy (DMN

Trends in disability (2001-2019), chronic medical conditions (1996-2020), and mortality (1995-2020) in Japanese older adults: analyses based on national datasets.

BACKGROUND: The overall health status of older adults in Japan has reportedly improved. However, it is unclear whether such improvement has occurred in the baby boomer generation, the oldest of whom turned 65 in 2015. In addition, the health status of oldest-old adults (aged > 85 years) was not examined extensively in previous studies. This study aimed to characterize trends in disability and chronic medical conditions in adults aged 65-89 years which includes the baby boomer generation. METHODS: From the Comprehensive Survey of Living Conditions (2001-2019), Patient Survey (1996-2020), and V

SATB2 Expression in Human Tumors: A Tissue Microarray Study on More Than 15 000 Tumors.

CONTEXT.—: Special AT-rich sequence-binding protein 2 (SATB2) induces local chromatin loops to facilitate transcription. SATB2 immunostaining is commonly used as a marker for colorectal adenocarcinoma and osteosarcoma. OBJECTIVE.—: To extend our knowledge on the diagnostic value of SATB2 analysis in a comprehensive set of human tumors. DESIGN.—: Tissue microarrays with 15 012 samples from 120 tumor types and 608 samples of 76 different normal tissues were analyzed. RESULTS.—: SATB2 positivity was found in 89 of 120 different tumor types (74%), including 59 of 120 (49%) with at least 1 moder

Tumors of the parathyroid gland and circulating parathyroid hormone-related protein associated with persistent hypercalcemia.

Neoplasms of the parathyroid glands are uncommon in all species of laboratory and domestic animals, but occur in low incidence in rats, Syrian hamsters, and dogs and rarely in mice. Proliferative lesions of the parathyroid gland include hyperplasia (diffuse and focal), adenomas, and carcinomas. The tumors may be functional or nonfunctional. Trophic atrophy of remaining parathyroid tissue is present around functional tumors. Humoral hypercalcemia of malignancy (HHM) is a syndrome that occurs in human and animal patients with certain malignant neoplasms and is characterized by hypercalcemia, hyp

Mechanisms of epithelial development and neoplasia in the metanephric kidney.

Recent studies on the mechanisms of normal epithelial development in the kidney, and on the aetiology of renal neoplasms, are converging to reveal remarkably close relationships between the phenotypes and behaviours of normally-developing and neoplastic cells. Normal renal epithelia arise from two sources; those of the collecting duct system develop by arborisation of an initially-unbranched ureteric bud, in a manner similar to the development of other glandular organs, while epithelial nephrons develop via an unusual mesenchyme-to-epithelial transition. Both types of development require contr

Comparative genomic hybridization study of perivascular epithelioid cell tumor: molecular genetic evidence of perivascular epithelioid cell tumor as a distinctive neoplasm.

Perivascular epithelioid cell tumor (PEComa) is a neoplasm composed chiefly of HMB-45-positive epithelioid cells with clear to granular cytoplasm and a perivascular distribution. Such tumors have been reported in different organs under a variety of designations. The cytogenetic features of these neoplasms have not been well studied. We collected 9 tumors (5 of kidney, 1 of prostate, 1 of urinary bladder, 1 of the pelvic cavity soft tissue, and 1 of uterus) from 8 patients, including one patient with tuberous sclerosis complex. The paraffin blocks of tumor tissue were submitted for comparative

Sulfated HNK-1 epitope in developing and mature kidney: a new marker for thin ascending loop of Henle and tubular injury in acute tubular necrosis.

The HNK-1 carbohydrate epitope is a 3-sulfo-glucuronyl residue attached to lactosamine structures on glycoproteins, proteoglycans, or glycolipids mostly expressed in the nervous system. Here, using monoclonal antibodies against the sulfated HNK-1 carbohydrate epitope, we first examined its distribution in developing and adult kidneys, then its expression in kidneys with tubular necrosis and renal neoplasms. This HNK-1 epitope was expressed in the human, rabbit, and rat, but not mouse kidney. It was detected within a subset of epithelial cells in the renal vesicle and in comma- and S-shaped bod

A Case of Myeloproliferative Neoplasm-Related Glomerulopathy Secondary to Primary Myelofibrosis With Acute Leukaemic Transformation.

Glomerular diseases associated with myeloproliferative neoplasms (MPN) are rare, and most often present with proteinuria and kidney impairment. Its natural history is not well described, although it has been associated with poor prognosis in described cases. Here, we present a case of MPN-related focal segmental glomerulosclerosis (FSGS) secondary to primary myelofibrosis (PMF) and describe its progression with transformation of PMF to leukaemia. A 52-year-old gentleman was referred for lower limb swelling on a background of primary myelofibrosis requiring splenectomy 3 months prior. Kidney fu

Familial pneumothoraces - Birt-Hogg-Dubé syndrome. Differentiation with other cystic lung diseases.

Birt-Hogg-Dubé syndrome (BHDS) is a rare, genetic, autosomal dominant disease caused by mutation in a folliculin gene. This syndrome is characterised by three main symptoms: benign lesions originating from hair follicles, variously shaped cysts in the lungs, and various types of benign and malignant kidney neoplasms. In our article we are going to present cases of two sisters with BHDS. In the case of the first sister skin lesions were accompanied by lung abnormalities. The second sister, however, presented with recurrent pneumothoraces associated with variously shaped lung cysts located mainl…

Cholesterol granuloma: a rare benign rib tumor.

Rib tumors are uncommon, constituting only 5% to 10% of all bony neoplasms. Cholesterol granuloma is also rare and is described as inflammatory granulation that occurs in response to the deposition of cholesterol crystals. Cholesterol granulomas are found most commonly in the paranasal sinuses or temporal bones, but there are also rare reports of their occurrence in the peritoneum, parotid gland, lymph nodes, thyroglossal duct, kidney, liver, and spleen. Involvement of the ribs has rarely been described previously. We report a rare case of cholesterol granuloma involving the second rib of a 38

Thymoma and solid-organ transplantation.

Solid-organ-transplant recipients are at high risk of developing cancer, including thoracic malignancies. Herein, we report on two cases of thymoma in solid-organ recipients. One case was a female kidney-transplant recipient who had a type B2, Masaoka stage IVA thymoma that was treated by extended surgical resection followed by postoperative radiation therapy. The second case was type A Masaoka II thymoma that occurred after a liver transplant. The role of EBV in the oncogenesis of thymoma is controversial. We suggest that viruses may induce neoplasms in this specific population, as observed i

The Imaging Features of Tuberous Sclerosis in African Children:A Pictorial Essay.

Tuberous sclerosis (TS) is a neurocutaneous syndrome with an autosomal dominant inheritance characterized by widespread hamartomas and benign neoplasms distributed in several organs throughout the body. The mean age of presentation is 5 years and the imaging features tend to vary with age. The most commonly affected organs include the brain, skin, retina, kidney, bones, heart and lung. The role of imaging diagnosis and management cannot be overemphasized. The aim of this paper is to highlight the specific imaging features of TS in children of African descent which is crucial to timely diagnosi

Imaging of Metastases in the Chest: Mechanisms of Spread and Potential Pitfalls.

Pulmonary and pleural metastases are routinely identified on thoracic computed tomography. Pulmonary metastases are the most common pulmonary neoplasms and commonly originate from primary malignancies of the lung, breast, colon, pancreas, stomach, skin (ie, melanoma), head and neck, and kidney. Metastatic disease to the lungs may occur via 3 routes of spread: hematogenous, lymphatic, and endobronchial. Pleural metastases most commonly originate from primary malignancies of the lung and breast. Mechanisms of pleural metastatic involvement include hematogenous spread, direct invasion from a neig

[The place of ultrasound diagnosis in internal medicine].

The applications of ultrasound diagnosis within the framework of general medicine are discussed on the basis of case reports. Ultrasonic equipment with manual moving of the probe is used in this department for the investigation of the abdomen in cases of occult neoplasms and metastases to differentiate between cysts and solid tumours and to determine the situation and size of parenchymatous organs like liver and kidney and spleen. Special fields of application are: follow up of patients after renal transplantation, investigation of systemic haematological diseases such as lymphogranuloma, or t

Renal sinus angiomyolipoma: A rare case.

Angiomyolipoma (AML) consists of <10% of all renal tumors and is the most common benign mesenchymal neoplasm of the kidney. It arises from the renal cortex and extends toward perirenal fat. Fat-poor AML as well as those arising from rare locations may pose diagnostic difficulties. In our case, we report a rare presentation of this tumor arising from the renal sinus extending toward the pelvis thereby leading to an alternative diagnosis of transitional cell carcinoma of renal pelvis.

Two unusual cases of adult intussusception.

Adult intussusception is very rare. We report 2 unusual cases, a 58-year-old man with a transverse colo-colonic intussusception caused by a malignant sessile polyp that also had an asymptomatic synchronous neoplasm of the kidney, and an 18-year-old female with an ileocecolic intussusception caused by acute appendicitis. This report stresses the point that intussusception in adults may represent an underlying malignancy. The age of the patient and the anatomic location of the intussusception provide significant input as to the etiology and hence the most appropriate surgical procedure.

[Apparent spontaneous regression of an inoperable kidney cancer].

In 1959, a then 66-year-old woman was undergoing an operation of a renal "tumor". Because of inoperability, only a biopsy was performed. Histologically, a non differentiated hypernephroid carcinoma was diagnosed. 20 years later, in 1979, the patient died of biventriculary cardiac insufficiency. In the autopsy a retroperitoneal "tumor", affecting the kidney, was found and for the present (without knowledge of the anamnesis) diagnosed as a liposarcoma. In knowledge of history, duration and behaviour of the disease and histopathology of the lesion, the "tumor" in question showed to be a twice mis

A case of lysozyme-induced nephropathy with improved renal function following treatment for chronic myelomonocytic leukemia.

Chronic myelomonocytic leukemia (CMML) is a myelodysplastic/myeloproliferative neoplasm characterized by persistent peripheral blood monocytosis. Although renal involvement can occur in patients with CMML, it may be overlooked. In addition, renal impairment is not included among the current criteria for initiating treatment for CMML.We report the case of a 75-year-old man with lysozyme-induced nephropathy that was initially considered to be diabetic nephropathy. He had a 20-year history of diabetes mellitus and presented to our hospital with weight loss and progressive anemia. After further ev

Dedifferentiated liposarcoma with extensive cystic change causing significant diagnostic challenges: report of two cases and review of the literature.

BACKGROUND: Retroperitoneal dedifferentiated liposarcoma is a rare, aggressive malignancy, characterized by high rates of recurrences and the potential for metastasis. On imaging, these tumors typically present as a solid mass with lipomatous and non-lipomatous components. Cystic changes of dedifferentiated liposarcomas is exceedingly rare and might pose significant diagnostic challenges, with only a few cases reported in the literature. REPORT OF 2 CASES: We here present two cases of retroperitoneal dedifferentiated liposarcoma with a rare cystic presentation in two female patients aged 51 an
