French protocol for the diagnosis and management of hereditary angioedema.

Recurrent isolated angioedema (AE) is a diagnostic and therapeutic challenge. Only a rigorous clinical approach can rule out the most obvious diagnoses. The recent international classification recognizes 7 subgroups: mast cells induced AE (spontaneous, allergic), Bradykinin mediated AE (Hereditary or acquired C1Inh; kallikrein-kinin mutations), HAE due to vascular endothelium dysfunction, drug induced AE and AE of unknow origin. Bradykinin-mediated hereditary angioedema (BK-HAE) are rare conditions. The currently accepted incidence of HAE-C1INH is approximately 1 in 50,000 inhabitants per year

Urine proteomic analysis reveals alterations in heme/hemoglobin and aminopeptidase metabolism during Lonomia obliqua venom-induced acute kidney injury.

AIMS: Accidental contact with the Lonomia obliqua caterpillar is a common event in southern Brazil. Envenomed victims present consumption coagulopathy, which can evolve to acute kidney injury (AKI). In the present study, we searched for AKI biomarkers and changes in molecular pathway signatures through urine proteomic analysis. METHODOLOGY: Male Wistar rats were injected with L. obliqua venom (1.5 mg/kg, via s.c.) or 0.9 % NaCl and distributed into metabolic cages. After 24 h, urine was obtained, and the set of differentially regulated proteins was analyzed by MudPIT technology in an OrbiTRAP

[High molecular weight kininogen in inflammation and angiogenesis: a review of its properties and therapeutic applications].

The plasma kallikrein-kinin system (KKS) participates in the pathogenesis of inflammatory reactions involved in cellular injury, coagulation, fibrinolysis, kinin formation, complement activation, cytokine secretion and release of proteases. It has been shown that KKS activation in the systemic inflammatory response syndrome results in decrease of its component plasma proteins. Similar changes have been documented in diabetes, sepsis, children with vasculitis, allograft rejection, disseminated intravascular coagulation, patients with recurrent pregnancy losses, hereditary angioedema, adult resp

Contact activation in low-density lipoprotein apheresis systems.

BACKGROUND: Anaphylactoid reactions due to contact activation have been observed in patients on ACE inhibitor therapy and hemodialysis with negatively charged dialysis membranes. Negatively charged surfaces are functional constituents of different LDL apheresis systems. Therefore, contact activation was investigated during LDL apheresis with three different systems: (i) heparin-induced extracorporeal LDL precipitation (HELP); (ii) dextran sulfate cellulose (DSC) columns; and (iii) modified polyacrylate gels (DALI) in a clinical setting. METHODS: 24 prevalent patients on regular LDL apheresis

Effect of invertebrate serine proteinase inhibitors on carrageenan-induced pleural exudation and bradykinin release.

The carrageenan model of pleurisy is described as temporal plasma exudation (1-5 h) with extensive neutrophil infiltration and release of proteinases into the pleural cavity. The aim of this work was to study the effects of serine proteinase inhibitors on the inflammatory process induced by administration of carrageenan to the rat pleural cavity and on release of kinins in pleural exudate. Pleurisy was induced by injecting carrageenan and serine proteinase inhibitors simultaneously into the pleural cavity. The proteinase inhibitors used were: aprotinin, a plasma kallikrein inhibitor; recombina

Posterior Ciliary Artery Contraction by Bradykinin Receptor Subtypes and Implications for Retinal Blood Flow Dysfunction.

BACKGROUND: Low retinal blood flow and/or vasospasm represent major risk factors for the development of glaucomatous optic neuropathy (GON), a potentially blinding eye disease. Bradykinin (BK), a nonapeptide, is endogenously produced and released, which can cause smooth muscles to contract and relax in different tissues depending on the physiological/pathological situation and the presence or absence of vascular tone. Several reports have shown the presence of BK receptor mRNAs, and in some cases, B1- and B2-receptor proteins, in ocular tissues, including the retina. However, the function of t

[Complications of closed chest injuries].

The role of predisposing factors in the development of complications in 1533 patients with closed traumas of the chest was studied as well as the dynamics of the state of the system of humoral regulation of microcirculation. The factors predisposing to the development of complications are the age older than 40, hospitalization later than 1 day from the trauma, double fractures of the ribs, injuries of lungs, hemothorax, concomitant diseases. In all the patients there was a subacute syndrome of disseminated intravascular coagulation, activation of the Kallikrein-Kinin system and inhibition of t

Anaphylactic and anaphylactoid reactions. A review.

Anaphylaxis is a life-threatening, immunologically mediated reaction. "Anaphylactoid" reactions produce the same clinical syndrome but are not immunologically mediated. Vasoactive mediators of these reactions include histamine, eosinophilic chemotactic factors of anaphylaxis, slow reacting substance of anaphylaxis (a mixture of three leukotrienes, one a potent coronary vasoconstrictor), kinins, and prostaglandins. Symptoms usually occur within 15 minutes of parenteral injection of the causative agent, but can be delayed as long as 2.5 hours. Because vasodilation occurs at the capillary and pos

Theophylline reduces the response to nasal challenge with antigen.

A nasal challenge model of allergic rhinitis was used to determine if pretreatment with oral theophylline reduces histamine release in vivo. Ten subjects were entered into a double-blind, cross-over trial. The results showed that both the physiologic response (sneezing) (p = 0.02) and the amount of mediators (histamine, kinins, toluene sulfonyl arginine methyl ester esterase activity) (p less than 0.01 for all) released into nasal secretions were significantly reduced after one week of pretreatment with theophylline. At the time of challenge, the serum concentrations of theophylline were betwe

Renovascular hypertension in bradykinin B2-receptor knockout mice.

We evaluated whether kinins exert a protective action against the development of two-kidney, one clip (2K1C) hypertension, a model characterized by an activated renin-angiotensin system in the ischemic kidney and increased expression of the bradykinin (BK) B2 receptor in the contralateral kidney. BK B2-receptor knockout (B2-/-), wild-type (B2+/+), and heterozygous (B2+/-) mice underwent clipping of the left renal artery, with the other kidney remaining untouched. Basal systolic blood pressure (SBP, via tail-cuff plethysmography) was higher in B2-/- mice than in B2+/- or B2+/+ mice (121+/-2 ver

Angioneurotic edemas of the upper aerodigestive tract after ACE-inhibitor treatment.

There are rare cases in which inhibitors of the angiotensin-converting enzyme can cause an angioneurotic edema of the upper aerodigestive tract. The pathomechanism of this side effect depends on an interaction of the drug with hormones regulating vascular permeability, such as the kallikrein kinin system and the prostaglandin system. Angioedema is characterized by subcutaneous or submucosal swellings, which usually affect the lips, soft palate, tongue and larynx. Pathomechanisms, differential diagnosis and treatment of ACE-inhibitor-induced edema of the upper aerodigestive tract are described

Inhibitors of angiotensin converting enzyme may rarely cause an angioneurotic oedema of the upper aerodigestive tract. The pathomechanism of this side effect depends on an interaction of the drug with hormones regulating the vascular resistance such as the kallikrein kinin system and the prostaglandin system. Anglo-oedema is characterised by subcutaneous or submucosal swelling, which preferably affects the lips, the soft palate, the tongue and the larynx. The des-Arg9-bradykinin/B1R axis: Hepatic damage in COVID-19.

Patients infected by the SARS-CoV-2 virus are commonly diagnosed with threatening liver conditions associated with drug-induced therapies and systemic viral action. RNA-Seq data from cells in bronchoalveolar lavage fluid from COVID-19 patients have pointed out dysregulation of kallikrein-kinin and renin-angiotensin systems as a possible mechanism that triggers multi-organ damage away from the leading site of virus infection. Therefore, we measured the plasma concentration of biologically active peptides from the kallikrein-kinin system, bradykinin and des-Arg9-bradykinin, and liver expression

Evaluation of Thr6-bradykinin purified from Polybia occidentalis wasp venom in the choline uptake of mammal cortices.

CONTEXT: Thr6-bradykinin is a peptide found in the venom of social and solitary wasps. This kinin, along with other bradykinin-like peptides, is known to cause irreversible paralysis in insects by presynaptic blockade of cholinergic transmission. However, this activity has never been tested in mammals. OBJECTIVE: As such, the objective of this study was to evaluate the effect of Thr6-bradykinin on the cholinergic system of rats. MATERIALS AND METHODS: The peptide was isolated from the venom of the Neotropical social wasp Polybia occidentalis Olivier (Vespidae). After correct identification a

Bradykinin B2 receptor contributes to the exaggerated muscle mechanoreflex in rats with femoral artery occlusion.

Static muscle contraction activates the exercise pressor reflex, which in turn increases sympathetic nerve activity (SNA) and blood pressure (BP). Bradykinin (BK) is considered as a muscle metabolite responsible for modulation of the sympathetic and cardiovascular responses to muscle contraction. Prior studies have suggested that kinin B2 receptor mediates the effects of BK on the reflex SNA and BP responses during stimulation of skeletal muscle afferents. In patients with peripheral artery disease and a rat model with femoral artery ligation, amplified SNA and BP responses to static exercise

Increased renal kallikrein excretion in SIADH after vincristine therapy.

This is a report about a 14-year old girl who, following chemotherapy for malignant teratoma, developed a clinical state of SIADH (syndrome of inappropriate secretion of antidiuretic hormone). The causative agent was most likely vincristine (VCR). The important feature in this case was that the urinary kallikrein activity was high when she was affected by SIADH and decreased when her hyponatremia improved. Sodium clearance was significantly correlated with the increase in urinary kallikrein activity. It is considered that the kallikrein-kinin system may in part participate in the excessive nat

[Renal kallikrein: variations in relation to sodium intake and experimental renovascular hypertension (author's transl)].

When rats are submitted to diets with variable sodium, concentration, one may observe a significant increase in the excretion of urinary kallikrein and in the kallikrein activity of the renal tissue in animals in chronic sodium depletion. During experimental renovascular hypertension induced in the rat by stenosis of the renal artery, one may note a significant rise in the excretion of urinary kallikrein; no change in this excretion is observed when the stenosis is associated with nephrectomy on the opposite side. It is possible to consider interrelations between the renin-angiotensin-aldoster

Intraoperative maintenance of tissue perfusion prevents ARDS. Adult Respiratory Distress Syndrome.

Patients undergoing prolonged, complex oncological surgery are at increased risk of developing the adult respiratory distress syndrome (ARDS) and other organ failures. Our hypothesis is that maintaining adequate tissue perfusion and oxygenation may prevent tissue hypoxia and acidosis in pulmonary, peripheral, and splanchnic microcirculations. Experimental evidence suggests that the hypoxic, acidotic endothelium stimulates the release of cytokines, kinins, and other mediators. We developed and tested an intraoperative protocol for surgical patients likely to develop ARDS and organ dysfunction;
