Fourteen patients were evaluated who had an arthroscopic removal of the symptomatic mediopatellar plica of the knee in this retrospective study. The main complaint of the patients was the pain at the medial side of the knee. Before the operation only in 3 cases was the diagnosis the pathological plica. The arthroscopy was performed in a routine way. The plica was consider pathological, and removed, when it was thickened. Osteochondritis dissecans is a disorder in which a fragment of cartilage and subchondral bone separates from an articular surface. The etiology is uncertain, although trauma and ischemia have been implicated. The knee is most commonly affected, but the elbow and ankle may also be involved. Patients typically present during their adolescent or early adult years with nonspecific knee pain and swelling that worsens with activity. The diagnosis is confirmed by radiographic findings. Management decisions are based on the patient's age and the stability, location, and size of the lesion.

Approximately one third of serious runners will incur an injury in a given year, and approximately one third of the injuries will involve the knee. Biomechanical studies on running reveal the tremendous cyclic forces to which the knee is subjected. Traumatic open dislocation of the knee is an infrequent, severe injury associated with extensive ligamentous damage and a high incidence of vascular and neurologic involvement. Eighteen patients with 19 open knee dislocations were treated at the affiliated hospitals of the University of Pennsylvania and Louisiana State University Medical Center during an 18-year period. Imaging of the knee is one of the main indications of MRI in musculo-skeletal diseases. Some of the most recent papers dealing with MR imaging of the knee are discussed in this review article. Seventy-nine arthroscopies of the knee were performed between January 1989 and March 1992. There were sixty-one males and eighteen females. The age range was from 12 to 78 years and the mean age was thirty-four years. Fifty patients had torn menisci. The menisci were removed arthroscopically in forty cases, arthrotomy being needed in the other ten cases. Medial tibial plateaux excised during 46 unicompartmental arthroplasties for osteoarthritis were collected and photographed. The anterior cruciate ligament was intact in all joints. In every case the cartilage and bone erosion was centred anteriorly on the plateau and the posterior cartilage was intact. Arthroscopic synovectomy for chronic hemophilic arthropathy has been performed since June of 1980, giving a 4 to 8 year follow-up of eight knees. The procedure was very successful in decreasing the incidence of hemarthroses. Only one patient had a recurrence of bleeding with synovitis. This publication consists of two parts, the first of which is concerned with the definition of arthrolysis as a mere soft tissue procedure. This is delineated from arthroplasty. In the second part, arthrolysis results are presented. We have investigated a prospective series of 100 acute traumatic haemarthroses of the knee in a general non-athletic population. All the patients had normal radiographs and an aspiration which confirmed the haemarthrosis, before undergoing ligament testing and an arthroscopic examination under anaesthesia. Only one patient had no serious pathology; in the other 99 a total of 193 lesions were recorded. Thirty knees had only one isolated lesion, 69 had combined lesions. This study confirms the serious nature of an acute traumatic haemarthrosis of the knee, even in non-athletic patients.

Congenital dislocation of the knee is a very rare condition, the incidence in Scandinavia hitherto not being known with certainty. Thirteen patients treated during the years 1960 to 1983 with 19 affected knees were reviewed and followed up. Findings of muscular imbalances in nine of these cases and of spinal abnormalities in four cases strongly indicate neuromuscular imbalance as an aetiological factor. Eighty-one patients with 116 osteoarthritic knees were seen by the author at the University College Hospital (UCH), Ibadan in six years. Below the age of 50 the disease was significantly commoner among Nigerians than Caucasians; 39.5% of our patients were less than 50 years old, compared with 1.1% of Caucasians. Spontaneous osteonecrosis is a common cause of knee pain in older patients, but the diagnosis is often overlooked. Sudden knee pain in older women, with marked joint line tenderness and a decreased range of motion, should alert the physician to the diagnosis. Two to three weeks after the onset of symptoms, plain radiographs will usually be normal but bone scan will be markedly positive. Treatment is initially conservative. Surgical intervention (either osteotomy or arthroplasty) is reserved for patients who develop a large radiolucent lesion in the subchondral femoral condyle.

Congenital knee dislocation presents with a fixed hyperextension deformity and anterior displacement of the tibia on the femur. Seventeen patients with a dislocation of 27 knees and no other neuromuscular syndrome have been reviewed for treatment outcome. Almost one-half of the patients had associated congenital hip dysplasia. Early closed treatment obtained a successful reduction and satisfactory knee motion. Open reduction was required for failure of closed reduction. The Pavlik harness allowed for treatment of the congenital knee dislocation and hip dysplasia simultaneously.
