[The influence of epidural analgesia on the course of labor].

OBJECTIVE: The aim of the study was to analyze the influence of epidural analgesia (EA) on the course of labor. MATERIAL AND METHODS: The study group consisted of 191 women with EA ("walking anesthesia"). 209 women without EA were chosen for controls. All the patients delivered at the 1st Dept of Obstetrics and Gynecology Medical University of Warsaw, Poland. T-test and chi2 test were used for statistical analysis and p < 0.05 was considered statistically significant. RESULTS: There were no significant differences between the groups regarding age, parity weight, average birth weight of newbo

Time from pre-labor rupture of membrane at term to delivery in grand multiparous women.

OBJECTIVE: The effect of parity on labor progression following term pre-labor rupture of membranes (PROM) has been described, but detailed patterns among grand multiparous women remain unclear. Traditional labor models show accelerated labor with increasing parity, yet few studies have specifically examined PROM latency distributions or heterogeneity within high-parity groups. The aim of the present study was to characterize labor progression after term PROM across parity groups, with specific focus on identifying heterogeneity and distinct distributional patterns among grand multiparous women

Labor patterns of spontaneous first-stage labor in Chinese women with normal neonatal outcomes.

BACKGROUND: Friedman's standards, developed almost 50 years ago, may no longer align with the needs of today's obstetric population and current pregnancy management practices. This study aims to analyze contemporary labor patterns and estimate labor duration in China, focusing on first-stage labor data from Chinese parturients with a spontaneous onset of labor. METHODS: This retrospective observational study utilized data from electronic medical records of a tertiary hospital in Changsha, Hunan. Out of a total of 2,689 parturients, exclusions were made for multiple gestations, preterm, post-t

[Comparison of obstetric and perinatal results of childbirth vertical position vs. childbirth supine position].

BACKGROUND: Vertical position is an option to delivery to which several advantages have been attributed. This research exposes its related findings. OBJECTIVE: To compare obstetric and perinatal outcomes between supine and vertical position at delivery. PATIENTS AND METHOD: We performed a randomized double-blind study including healthy women assigned to the supine or upright posture (vertical) during labor with complications following the delivery in the puerperium stage. The variables evaluated were: blood loss, pain in the second period of labor and immediate postpartum, duration of the se

Association Between Active Phase of the Second Stage of Labour and Obstetrical Anal Sphincter Injury Risk: A Contemporary, Retrospective, Population-Based Cohort Study.

INTRODUCTION AND HYPOTHESIS: Although the association between the prolonged second stage of labour and obstetrical anal sphincter injury (OASI) risk has been established, the contribution of the duration of the active phase (time spent pushing) remains unclear. This study was aimed at estimating the association between the duration of the active phase of the second stage of labour and OASI risk among nulliparous pregnant persons. METHODS: We conducted a population-based, retrospective cohort study of nulliparous pregnant persons who gave birth to a singleton, non-anomalous, vertex foetus at ≥

Impacted fetal head extraction methods at second stage cesarean and subsequent preterm delivery: A multicenter study.

OBJECTIVE: Second-stage cesarean delivery (CD) is associated with subsequent preterm birth (PTB). It has been suggested that an increased risk of PTB after second-stage cesarean delivery could be linked to a higher chance of cervical injury due to the extension of the uterine incision. Previous studies have shown that reverse breech extraction is associated with lower rates of uterine incision extensions compared to the "push" method. We aimed to investigate the association between the method of fetal extraction during second-stage CD and the rate of spontaneous PTB (sPTB), as well as other ma

Evaluating fetal head dimension changes during labor using open magnetic resonance imaging.

AIM: Fetal skull molding is important for the adaptation of the head to the birth canal during vaginal delivery. Importantly, the fetal head must rotate around the maternal symphysis pubis. The goals of this analysis were to observe a human birth in real-time using an open magnetic resonance imaging (MRI) scanner and describe the fetal head configuration during expulsion. METHODS: Real-time cinematic MRI series (TSE single-shot sequence, TR 1600 ms, TE 150 ms) were acquired from the midsagittal plane of the maternal pelvis during the active second stage of labor at 37 weeks of gestation. Management of the second phase of labour: perineum protection techniques.

The obstetric experience alongside scientific evidences in literature indicate several management techniques during the expulsive period of labour to minimize obstetric complications. Among the various methods that can be used for the protection of the perineum during the expulsive phase, some are performed prepartum (perineum massage), while most are used during childbirth. Among the second group, progressively increasing importance is assumed by the manual techniques to protect the perineum (using the "hands-on" and "hands-off") and by episiotomy. These techniques, when used in accordance to

Risk of perineal damage is not a reason to discourage a sitting birthing position: a secondary analysis.

AIM: To examine the association between semi-sitting and sitting position at the time of birth and perineal damage amongst low-risk women in primary care. BACKGROUND: Evidence on the association between birthing positions and perineal trauma is not conclusive. Most studies did not distinguish between positions during the second stage of labour and position at the time of birth. Therefore, although birthing positions do not seem to affect the overall perineal trauma rate, an increase in trauma with upright position for birthing cannot be ruled out. METHODS: Secondary analysis was performed on

The symptom of stress incontinence caused by pregnancy or delivery in primiparas.

Three hundred five primiparas were interviewed repeatedly about stress incontinence before and during pregnancy and after delivery. Eleven (4%) had stress incontinence before pregnancy and 98 (32%) during pregnancy, whereas 21 (7%) developed it after delivery. According to the International Continence Society definition, the corresponding frequencies were one (0.3%), three (1%), and one (0.3%), respectively. Obstetric factors such as length of the second stage of labor, head circumference, birth weight, and episiotomy seemed to be associated with, whereas cesarean delivery seemed to protect ag

Third-stage uterotonic management after second-trimester induction of labour: a retrospective cohort study.

OBJECTIVES: Retained placenta is a potential complication of second-trimester induction of labour. There is limited consensus regarding the use of adjunctive medications to manage the third stage following second-trimester vaginal delivery. This study aimed to evaluate the effects of oxytocin versus misoprostol on third-stage duration and the need for dilation and curettage (D&C) among patients with and without mifepristone pretreatment. STUDY DESIGN: We conducted a retrospective cohort study of patients undergoing induction of labour between 14 + 0 and 23 + 6 weeks of gestation for any indic

Maternal Positioning in the Second Stage of Labour and its Relationship with Perineal Trauma: A Systematic Review and Network Meta-Analysis.

INTRODUCTION AND HYPOTHESIS: Perineal trauma is frequent after childbirth and may be influenced by maternal position. We aimed to synthesize pooled estimates comparing positions in the second stage of labour and their association with perineal tears and episiotomy, using a network meta-analysis (NMA) of randomized controlled trials (RCTs). METHODS: Systematic review and NMA of RCTs identified in Cochrane Library Plus, EMBASE, Scopus, PubMed, and ClinicalTrials through June 2024, without language or date restrictions. Outcomes were 1st-degree tears (involving skin and mucosa), 2nd-degree tears

Maternal positions and pushing techniques in a nonprescriptive environment.

An observational study was done on the positions and breathing techniques women will choose for second-stage labor when they are given the freedom and support to choose. In the 50 second stages and births observed, nine different positions were used in conjunction with three variations of expulsive breathing techniques. No adverse outcomes resulted from the nonprescriptive approach to birthing women. All outcome parameters were found to be within the range of normal. These findings support the acceptability of allowing women to respond to their birthing impulses. Further study is recommended t

[Paravertebral block for labor analgesia in a parturient with idiopathic thrombocytopenia].

A parturient with idiopathic thrombocytopenia received labor analgesia with bilateral paravertebral blocks, because epidural analgesia was contraindicated due to her low platelet count (69,000.microliter-1) even after intravenous administration of freeze-dried sulfonated human normal globulin (400 mg.kg-1.day-1) and transfusion of platelet (20 units.day-1). In spite of predonisolone (1 mg.kg-1.day-1) p.o., her platelet counts could not increase at early gestation period. Prior to the induction of the labor, two catheters were inserted into T 11 bilateral paravertebral spaces, then 0.2% ropivac

Misconceptions about neuraxial analgesia.

Neuraxial analgesia is the most effective treatment for labor pain. Although there are known risks and side effects, many problems are attributed to the technique without appropriate scientific assessment. Some observations, such as the increased use of forceps and prolonged second stage of labor, are associated with epidural analgesia, but there is not enough evidence to categorically state whether the technique caused the problems. These topics warrant continued investigation.

Factors associated with a maternal lower-limb neurological deficit after vaginal delivery.

INTRODUCTION: The etiology of lower-limb neurological deficit after vaginal delivery remains poorly understood. The objective herein was to identify factors associated with this maternal nerve injury after vaginal delivery. MATERIAL AND METHODS: A single-center, case-control (matching 1:4) study. Cases were women with a lower-limb neurological deficit that appeared immediately after vaginal delivery. Controls were randomly selected women who gave birth vaginally during the same period, without any deficit. Finally, to assess the rates of factors associated with these deficits, we studied them
