Appropriateness of Thyroid Nodule Cancer Risk Assessment and Management Recommendations Provided by Large Language Models.

The study evaluates the appropriateness and reliability of thyroid nodule cancer risk assessment recommendations provided by large language models (LLMs) ChatGPT, Gemini, and Claude in alignment with clinical guidelines from the American Thyroid Association (ATA) and the National Comprehensive Cancer Network (NCCN). A team comprising a medical imaging informatics specialist and two radiologists developed 24 clinically relevant questions based on ATA and NCCN guidelines. The readability of AI-generated responses was evaluated using the Readability Scoring System. A total of 322 radiologists in …

New frontiers in radiologic interpretation: evaluating the effectiveness of large language models in pneumothorax diagnosis.

BACKGROUND: This study evaluates the diagnostic performance of three multimodal large language models (LLMs)-ChatGPT-4o, Gemini 2.0, and Claude 3.5-in identifying pneumothorax from chest radiographs. METHODS: In this retrospective analysis, 172 pneumothorax cases (148 patients aged >12 years, 24 patients aged ≤12 years) with both chest radiographs and confirmatory thoracic CT were included from a tertiary emergency department. Patients were categorized by age and pneumothorax size (small/large). Each radiograph was presented to all three LLMs accompanied by basic symptoms (dyspnea or chest pa…

Comparative analysis of large language models in providing patient information about keratoconus and contact lenses.

OBJECTIVE: To evaluate the accuracy, completeness, informational quality, and readability of responses generated by large language models (LLMs)-ChatGPT (OpenAI, USA), Gemini (Google, USA), and Copilot (Microsoft, USA)-to patient questions concerning keratoconus and contact lens use. METHODS: In this cross-sectional study, 32 questions across eight domains were posed to the free versions of each model. Two independent ophthalmologists rated accuracy (6-point Likert scale) and completeness (3-point Likert scale). Information quality was assessed using the DISCERN instrument, and readability wa…

Improving social determinants of health documentation in French electronic health records using large language models.

Social determinants of health (SDoH) significantly influence health outcomes, shaping disease progression, treatment adherence, and health disparities. However, their documentation in structured electronic health records (EHRs) is often incomplete or missing. This study presents an approach based on large language models (LLMs) for extracting 13 SDoH categories from French clinical notes. We trained Flan-T5-Large on annotated social history sections from clinical notes at Nantes University Hospital, France. We evaluated the model at two levels: (i) identification of SDoH categories and associa…

Resource-efficient fine-tuning of large vision-language models for multimodal perception in autonomous excavators.

Recent advances in large vision-language models (LVLMs) have transformed visual recognition research by enabling multimodal integration of images, text, and videos. This fusion supports a deeper and more context-aware understanding of visual environments. However, the application of LVLMs to multitask visual recognition in real-world construction scenarios remains underexplored. In this study, we present a resource-efficient framework for fine-tuning LVLMs tailored to autonomous excavator operations, with a focus on robust detection of humans and obstacles, as well as classification of weather

[GENOMIC CANCER TESTS AND MEDICAL DECISION-MAKING: HOW DO LARGE LANGUAGE MODELS HANDLE INFORMATION GAPS?].

Clinicians and patients are offered a variety of genomic tests intended to assess the prognosis of cancer. Among these is the "Decipher" test for prostate cancer. A 71-year-old patient with prostate cancer was referred for testing by his treating doctor. The test results suggested reclassification of the patient from the intermediate-risk group for death and disease complications to the high-risk group.

Evidence triangulator: using large language models to extract and synthesize causal evidence across study designs.

Health strategies increasingly emphasize both behavioural and biomedical interventions, yet the complex and often contradictory guidance on diet, behavior, and health outcomes complicates evidence-based decision-making. Evidence triangulation across diverse study designs is essential for balancing biases and establishing causality, but scalable, automated methods for achieving this are lacking. In this study, we assess the performance of large language models in extracting both ontological and methodological information from scientific literature to automate evidence triangulation. A two-step …

Evaluating the reliability of large language models for clinical data extraction in bladder cancer prognosis.

Advances in natural language processing (NLP) and machine learning could assist human users in clinical data extraction from unstructured electronic medical records (EMRs). This study investigates the accuracy and consistency of several Large Language Models (LLMs) - including Dolly, Vicuna, Llama, and GPT-4 - in extracting critical clinical information pertinent to bladder cancer survival prediction. Using EMRs from 163 bladder cancer patients, we assessed the impact on LLM performance by factors such as differences in the trained models, model evolution, input text length, and sequencing of …

Evaluating Large Language Models for Accuracy and Completeness of Vitiligo Patient Education: A Comparative Analysis.

BACKGROUND: Vitiligo causes significant psychological stress, creating a strong demand for accessible educational resources beyond clinical settings. This demand remains largely unmet. Large language models (LLMs) have the potential to bridge this gap by enhancing patient education. However, uncertainties exist regarding their ability to accurately address individualized patient inquiries and whether comprehension capabilities vary between LLMs. PURPOSE: This study aims to evaluate the applicability, accuracy, and potential limitations of OpenAI o1, DeepSeek-R1, and Grok 3 for vitiligo patien…

Large language models as assistance for glaucoma surgical cases: a ChatGPT vs. Google Gemini comparison.

PURPOSE: The aim of this study was to define the capability of ChatGPT-4 and Google Gemini in analyzing detailed glaucoma case descriptions and suggesting an accurate surgical plan. METHODS: Retrospective analysis of 60 medical records of surgical glaucoma was divided into "ordinary" (n = 40) and "challenging" (n = 20) scenarios. Case descriptions were entered into ChatGPT and Bard's interfaces with the question "What kind of surgery would you perform?" and repeated three times to analyze the answers' consistency. After collecting the answers, we assessed the level of agreement with the unifi

Enhancing readability of USFDA patient communications through large language models: a proof-of-concept study.

BACKGROUND: The US Food and Drug Administration (USFDA) communicates new drug safety concerns through drug safety communications (DSCs) and medication guides (MGs), which often challenge patients with average reading abilities due to their complexity. This study assesses whether large language models (LLMs) can enhance the readability of these materials. METHODS: We analyzed the latest DSCs and MGs, using ChatGPT 4.0© and Gemini© to simplify them to a sixth-grade reading level. Outputs were evaluated for readability, technical accuracy, and content inclusiveness. RESULTS: Original materials …
