Reducing postoperative length of stay in low-risk caesarean section: a system-level quality improvement initiative.

A structured, system-level quality improvement (QI) initiative at Singapore General Hospital was undertaken to reduce postoperative length of stay (LOS) after uncomplicated lower segment caesarean section (LSCS) and improve performance on the national Value-Driven Care clinical quality index. This project specifically targeted an increase in postoperative day 3 (POD3) discharge rates towards the 88% internal benchmark without compromising safety or increasing readmissions.Uncomplicated LSCS cases between October 2023 and September 2025 were included, with complicated caesarean sections and sev…

Does Sugammadex Decrease Length of Stay in Bariatric Surgery? Experience from an MBSAQIP Comprehensive Bariatric Center.

BACKGROUND: Residual effects from general anesthesia can delay the time until oral intake and ambulation. Reversal with Sugammadex has been shown to improve outcomes and decrease post-operative nausea and vomiting. The benefits amongst the bariatric population have not been ascertained. We hypothesized that the use of Sugammadex for reversal will decrease the length of stay (LOS). METHODS AND PROCEDURES: This is a single center retrospective review of 961 patients who underwent elective bariatric surgery from January 2023 to January 2025. Inclusion criteria were any patient undergoing electiv…

Lean Psoas Muscle Area Is Associated with Length of Stay After Lower Limb Revascularization for CLTI.

Background: Chronic limb-threatening ischemia (CLTI) is associated with high morbidity and substantial healthcare utilization. Length of hospital stay (LOS) after lower limb revascularization is influenced by procedural complexity, but patient physiological reserve may also play a role. We evaluated whether CT-derived lean psoas muscle area (LPMA) is independently associated with LOS in patients undergoing revascularization for CLTI. Methods: We retrospectively analyzed 234 consecutive patients treated with endovascular, hybrid, or open revascularization for CLTI (Rutherford 4-5) between 2018

Length of Hospital Stay between Direct Anterior Approach and Posterior Approach in Primary Total Hip Arthroplasty.

PURPOSE: The surgical approach used in performance of total hip arthroplasty (THA) is critical in determining the postoperative recovery process. The direct anterior approach (DAA) is a minimally invasive technique that preserves soft tissue, while the posterior approach (PA) is a traditional method providing a more extensive view of the hip joint. Length of stay (LOS) is an essential metric in assessing healthcare efficiency and patient recovery. The aim of this article is to compare LOS in patients undergoing THA via the DAA versus the PA. MATERIALS AND METHODS: A retrospective matched pair…

Machine learning models for predicting extended length of stay and hospital charges in nontraumatic subarachnoid hemorrhage.

BACKGROUND: Nontraumatic subarachnoid hemorrhage (SAH) is a critical condition requiring prolonged hospitalization and significant healthcare costs. Identifying factors contributing to extended length of stay (LOS) and predicting associated hospital charges can optimize clinical decision-making and resource allocation. This study aimed to construct and validate machine learning (ML) models to predict extended LOS and total charges in SAH using a national database. METHODS: A retrospective cohort study was conducted using data from the National Inpatient Sample database, including 25,092 adult…

Clinical Decision Support to Reduce Hospital Length-of-Stay for Cancer Patients with Fever and Neutropenia.

Pediatric cancer patients with fever and neutropenia are at risk for bacterial sepsis, traditionally requiring extended hospital stays on antibiotics until neutrophil counts recover. According to a newly validated scoring system, a subset of these patients is at lower risk and eligible for early discharge and reduced intravenous (IV) antibiotic exposure.Reduce length-of-stay (LOS) for febrile neutropenic patients using clinical decision support (CDS) to identify low-risk patients.A CDS system was developed to (1) screen febrile neutropenic patients using a validated clinical decision rule, (2)

Mobility matters: a protocol to improve mobility and reduce length of stay in hospitalised older adults.

Functional decline in hospitalised older adults aged 65 and above is a significant clinical problem. Despite its adverse outcomes, the problem of not mobilising older adult patients in clinical settings remains. Existing evidence suggests that a mobility protocol can be effective in addressing this concern. The aims of this quality improvement project were to determine whether a nurse-driven, multidisciplinary collaborative mobility protocol would increase the daily out-of-bed episodes, improve mobility level and reduce hospital length of stay (LOS). A nurse-driven mobility protocol was implem…

Length of hospital stay and readmissions after major lower extremity amputation: a Danish nationwide registry study.

BACKGROUND AND PURPOSE: Major lower extremity amputation (MLEA) is associated with complications that may prolong length of hospital stay (LOS) and increase the risk of readmission. We primarily aimed to examine the LOS and risk of readmissions after MLEA in Denmark. Secondarily we investigated the time trends. METHODS: Using Danish National Patient Registry data, this observational study analyzed 11,205 first-time MLEAs (35% transtibial amputations, 65% transfemoral amputations) performed between January 1, 2010 and December 31, 2021. Total LOS included pre- and postoperative nights. The fir

Clinical and Operative Risk Factors Associated With Prolonged Length of Stay After Endoscopic Pituitary Adenoma Resection.

BACKGROUND AND OBJECTIVES: Pituitary adenomas (PAs) are the most common intrasellar tumor. Clinically relevant adenomas have a prevalence of 1 per 1000 in the general population. Transsphenoidal surgery (TSS) is the most common surgical treatment and is the first-line management for most PAs. Most patients fare well postoperatively, but a subset of patients experience a prolonged length of stay (PLOS). In this article, we aim to identify demographic and clinical factors associated with PLOS after TSS for PA. METHODS: Patients with sellar pathologies surgically treated at a single tertiary cen…

Uncommon Sociodemographic Factors Are Associated With Racial Disparities in Length of Stay Following Oncologic Elective Colectomy.

INTRODUCTION: Although outcome disparities by race have been identified in colorectal cancer, these patterns are challenging to explain using variables that are commonly available in databases. In a single institution serving a diverse community, length of stay (LOS) varies by race following elective oncologic colectomy. We investigated previously unexplored variables that may explain the relationship between race and LOS following elective resection of colorectal neoplasms. METHODS: Retrospective, single institution cohort study from January 2015 to December 2020 for adult patients undergoin…

Enhanced Recovery After Surgery and Postoperative Nausea and Length of Stay in Mastectomy Patients With Reconstruction.

INTRODUCTION: Surgery is a mainstay of curative breast cancer treatment and is associated with postoperative nausea and vomiting (PONV) negatively impacting the patient experience. Enhanced recovery after surgery (ERAS) protocols are a combination of evidence-based strategies applied to traditional perioperative practices with the goal to reduce postoperative complications. ERAS protocols have been traditionally underutilized in breast surgery. We investigated if the implementation of an ERAS protocol was associated with decreased rates of PONV as well as length of stay (LOS) in patients under…
