PURPOSE: To demonstrate a novel technique for the management of dangling lenses with near complete zonular dehiscence.
METHODS: A capsule-sparing combined anterior and posterior surgical approach for dangling lenses with severe zonular dehiscence is described. The surgical record of a patient who underwent phacoemulsification with "dangling lens technique" and in-the-bag intraocular lens (IOL) implantation was evaluated.
RESULTS: This technique was able to preserve the lens capsule, allowing freedom of IOL choice and an optimally reconstructed anterior segment. Placement of sutured capsular

PURPOSE: To introduce a modified capsular hook technique for managing intraocular lens-capsular bag complex (IOLCBC) subluxation and compare its outcomes with conventional transscleral IOL fixation.
METHODS: A modified implantation technique using polypropylene suture hooks introduced from within the capsular bag, combined with transscleral flanged-node fixation was developed to preserve capsular integrity. Seventeen eyes with in-the-bag IOL-CBC subluxation were included: 10 treated with the modified capsular hook implantation technique and 7 with conventional transscleral fixation. Outcomes

INTRODUCTION: The Yamane technique is a transconjunctival, sutureless method for scleral fixation of intraocular lenses (IOL) in cases lacking capsular support. While widely adopted because of its efficacy and safety, rare late complications such as IOL subluxation have been reported. We describe a case of late-onset reverse pupillary block with recurrent IOL subluxation following Yamane fixation.
CASE PRESENTATION: We present a case of a 64-year-old male with a history of cataract surgery and ocular hypertension who underwent Yamane scleral IOL fixation in his left eye. Five years later, he

PURPOSE: To identify factors that determined the outcome of eyes that underwent pars plana vitrectomy for retained lens fragments after phacoemulsification.
METHODS: A retrospective review of the records of 63 eyes that underwent vitrectomy for retained lens fragments after phacoemulsification. Fragments were defined as nuclear if they contained any nucleus component or non-nuclear if they contained only cortex and epinucleus. Nuclear fragments were further divided into large (at least 1/3 the size of the nucleus) or small.
RESULTS: A total of 67% of eyes had retained nuclear fragments and 3

Synchysis scintillans is a vitreous condition in which multiple golden brown opacities are formed as a result of chronic vitreous hemorrhage. Anterior chamber synchysis scintillans was described in patients with afakia or lens subluxation. We report a case of a 63-year-old man with a history of left eye trauma and complete loss of vision, who presented for left eye discomfort. The slit lamp examination revealed crystals of synchysis scintillans and rare inflammatory cells in the anterior chamber, stromal iris atrophy, circumferential posterior iris synechiae, and complete lens opacity. Total r…

This report presents the fluocinolone-loaded intraocular lens (IOL) with scleral suspension (FLISS) technique as a surgical approach for managing macular edema (ME) in patients with compromised lens-iris diaphragm and dislocated IOL. A 74-year-old woman with a subluxated bag-IOL complex and chronic ME underwent pars plana vitrectomy. After IOL removal, Gore-Tex CV-8 sutures were used to secure a 0.19-mg fluocinolone acetonide (FAc) intravitreal implant (Iluvien) to the haptic of a Micro-pure IOL and subsequently fix the lens to the sclera. Three months later, visual acuity had improved from 20

Surgical treatment of ectopia lentis has traditionally been associated with a poor visual outcome and a high complication rate. We treated a series of nine children (15 eyes) whose visual acuity could not be improved with optimal optical phakic or aphakic correction with limbal lensectomy. Preoperative visual acuity after amblyopia treatment ranged from 20/60 to 20/200. Improvement in postoperative visual acuity was documented in all operated-on eyes, ranging from 20/20 to 20/50 during a follow-up period ranging from 8 months to 118 months (median, 33 months). The only complication was a child

BACKGROUND: Cohen syndrome is a rare autosomal recessive disorder characterized by facial anomalies with or without microcephaly, non-progressive intellectual disability, hypotonia, ocular abnormalities, and neutropenia. Due to its low prevalence and diverse presentations, much information about the disease, including ocular manifestations, is not yet fully understood. To date, there is a paucity of literature on Cohen syndrome, which is characterized by predominantly ocular manifestations and typical manifestations in multiple systems throughout the body.
CASE PRESENTATION: A 24-year-old mal

Persistent fetal vasculature (PFV) is a common congenital developmental anomaly of the eye which results from failure of the embryological primary vitreous and hyaloid vasculature to regress by the time of birth (Int Ophthalmol Clin 48: 53-62, 2008). Typically, it is divided into anterior, posterior or combined types and is characterized by the presence of a vascular stalk located between the optic disc and the posterior lens capsule (Int Ophthalmol Clin 48: 53-62, 2008). Although it has been reported to manifest itself differently, in our case it presented in a microphthalmic eye as anterior

OBJECTIVE: To evaluate the success rate and outcome of trans-corneal reduction (TCR) in dogs presenting with anterior lens luxation (ALL).
PROCEDURE: Medical records from dogs with ALL that underwent TCR were reviewed from 2020 to 2023. Eyes were classified as visual or potentially functional (PF) if menace response, dazzle, or indirect-PLR were positive. TCR was classified as successful if the replacement of the lens posterior to the iris was achieved.
RESULTS: Forty-one eyes (38 dogs) with a median age of 12 (range 5-16) years were included. The median follow-up was 116 (range 9-720) days.
