Attitudes of nurses toward life-sustaining treatment and its influencing factors: A mixed-methods study.

OBJECTIVE: This study aimed to identify factors influencing nurses' attitudes toward life-sustaining treatment (LST) in the Republic of Korea and to explore how perceptions of a good death and contextual factors shape these attitudes. METHODS: An explanatory sequential mixed-methods study was conducted at two tertiary hospitals from July to November 2024. In the quantitative phase, 104 nurses completed a cross-sectional survey, and multiple linear regression was used to identify factors associated with LST attitudes. In the qualitative phase, eight purposively sampled nurses participated in s

Singapore Paediatric Life Support Guidelines 2026.

The Singapore Paediatric Life Support Guidelines 2026 updates the 2021 guidelines and provides evidence-based, consensus recommendations for life support in paediatric patients (<18 years of age), excluding newborns. We reviewed treatment recommendations from the International Liaison Committee on Resuscitation Paediatric Life Support Task Force as well as resuscitation guidelines from the American Heart Association, the European Resuscitation Council and other relevant studies and reviews published between January 2021 and January 2026 to inform these paediatric life support guidelines. Digital support for chronic dyspnoea management in primary care: protocol for the BREATHE (Breathlessness Rapid Evaluation and Therapy) cluster randomised controlled trial.

INTRODUCTION: Chronic dyspnoea is a prevalent symptom, and primary care is ideally placed to identify and manage it. However, chronic dyspnoea is under-reported by patients and can be a diagnostic dilemma for practitioners. A fully automated system of patient screening, coupled with a clinical decision support system (CDSS) that uses a validated and evidence-based dyspnoea algorithm, may improve detection, diagnosis and management of the condition. There is currently no CDSS validated for chronic dyspnoea diagnosis and management in primary care in Australia. The objectives of this study are t

Early withdrawal of life support after resuscitation from cardiac arrest is common and may result in additional deaths.

AIM: "Early" withdrawal of life support therapies (eWLST) within the first 3 calendar days after resuscitation from cardiac arrest (CA) is discouraged. We evaluated a prospective multicenter registry of patients admitted to hospitals after resuscitation from CA to determine predictors of eWLST and estimate its impact on outcomes. METHODS: CA survivors enrolled from 2012-2017 in the International Cardiac Arrest Registry (INTCAR) were included. We developed a propensity score for eWLST and matched a cohort with similar probabilities of eWLST who received ongoing care. The incidence of good outc

Medical management of the suspected victim of bioterrorism: an algorithmic approach to the undifferentiated patient.

We have purposely expanded on the well-known ATLS paradigm to aid EHCPs in their approach to a potential bioterrorism event. By building on a process that is already familiar, we hope this will aid the EHCP to remember a systematic approach to such an incident. By following this ten-step process, we believe that all EHCPs, and especially those practicing at the first echelons of care in urgent care clinics and EDs, can approach the daunting problem of biological defense with a good deal more confidence. This same model advocated for bioterrorism also may apply to natural infectious disease epi

Decision-making: initiation and withdrawing life support in the asphyxiated infants in developing countries.

The issues of life support in the asphyxiated infant are not only whether cardiopulmonary resuscitation or CPR will be successful, but also whether if successful, the infant will be severely damaged. This is particularly important in the developing countries because the damaged infants may burden the society. The country has to allocate huge financial and human resources to look after them. When it comes to decisions in initiation and withdrawal of life support, there are differences between the East and the West. Physicians are searching for reliable predictors of outcome of term asphyxiated

[Heart and lung transplantation in acute organ failure].

Heart and lung transplantation represent a realistic therapeutical option in endstage cardiac and pulmonary diseases. Transplantation for acute organ failure, however, is limited due to the scarcity of suitable donors with respect to the shortage of time. Aside from pharmacological means a number of mechanical assist devices are available for bridging of those patients in acute myocardial or respiratory failure. Indications for acute organ transplantation are primary graft failure following heart or lung transplantation and acute myocardial insufficiency due to extensive myocardial infarction

Resuscitation and senility: a study of patients' opinions.

In the context of 'Do-not-resuscitate' (DNR) decisions, there is a lack of information in the UK on the opinions of patients and prospective patients. Written anonymous responses to questionnaires issued to 322 out-patient subjects showed that 97 per cent would opt for cardiopulmonary resuscitation (CPR) in their current state of health. In the hypothetical circumstance of having advanced senile dementia only 10 per cent would definitely want CPR, with 75 per cent preferring not to have CPR. There were no significant correlations between the responses and sex or age. Of 270 patients asked verb

[Empathy in palliative care of the elderly].

Empathy is fundamental to care, particularly geriatric and palliative care, as it responds to the complex, multidimensional needs of older adults. However, empathy can be risky, and requires ongoing training. Concrete strategies and techniques exist to facilitate the expression of empathy when caring for people at the end of life, and their loved ones, and to support the care team in preserving human dignity, even at the end of life.

The evaluation and treatment of men with asymptomatic prostate nodules in primary care: a decision analysis.

BACKGROUND: Whether to perform periodic rectal examinations in asymptomatic men as a screening test for prostatic cancer remains controversial. A randomized clinical trial that tests the efficacy of further evaluation and treatment of men who have been found to have asymptomatic prostate nodules may never be carried out. Decision analysis was therefore used to further investigate this clinical issue. METHODS: A decision tree was developed to model the decision of whether to biopsy an asymptomatic prostate nodule found by digital rectal examination in a 65-year-old man by his primary care phys

Promoting Reasons for Living Within Community Care Environments: A Scoping Review and Ninety-Four Reasons From Colombia.

INTRODUCTION: Suicidal behaviour continues to increase worldwide, which makes it necessary to strengthen care environments with strategies complementary to traditional ones, which validate the needs of the people who come to them. The objective was to determine the essential components of strategies that promote reasons to live in care services, and explore these reasons in community care settings in Colombia. METHODS: A Scoping Review was carried out. In-depth interviews were also conducted in community care settings with 94 people of multiple ethnicities, from the ages of five to 85, in var

Family Caregivers' Experiences with Dying and Bereavement of Individuals with Motor Neuron Disease in India.

Motor neuron disease (MND) is a progressive neurodegenerative disease. Ideal management plan in MND includes palliative care initiated from the time of diagnosis. At present, most of the neurodegenerative conditions are cared for at home. Neuropalliative care is an emerging concept in India and social workers are integral team members in this process. The primary aims of the study were to explore (a) the caregivers' experiences of the end-of-life stage, and (b) the sources of support for individuals and their caregivers with MND at the end-of-life stage. In-depth interviews were conducted with

Bereaved informal caregivers' experiences with a Canadian home hospice program: a qualitative study.

BACKGROUND: Most individuals wish to die at home, yet in Canada this is not always possible. Informal caregivers play a vital role to support individuals who wish to receive home-based end-of-life care and die at their own homes. Community-based home hospice programs aim to support both individuals at the end of life and their informal caregivers, however, informal caregivers' experiences within such programs, particularly in northern communities, remain underexplored. This study aimed to describe the experiences of bereaved informal caregivers who received support from a community-based home

Exploring the Impact of a Remote Monitoring System for Palliative and End-of-Life Care (CARE-PAC): Mixed Methods Feasibility Study.

BACKGROUND: In the United Kingdom, access to and the quality of palliative and end-of-life care (PEOLC) vary widely. In the final months of life, many patients face avoidable accident and emergency (A&E) visits and hospital admissions, driven by gaps in out-of-hours support and poorly coordinated care. This not only increases stress for patients and carers but also places avoidable strain and cost on the National Health Service (NHS). There is an urgent need for more compassionate, person-centered models that support people to remain at home, improve their quality of life (QoL), and reduce unn
