Corneal complications of vernal keratoconjunctivitis.

PURPOSE OF REVIEW: Vernal keratoconjunctivitis (VKC) is a severe bilateral chronic allergic inflammatory disease of the ocular surface. In most of the cases, the disease is limited to the tarsal conjunctiva and to the limbus. However, in the more severe cases, the cornea may be involved, leading to potentially sight threatening complications. Prompt recognition of these complications is crucial in the management of VKC, which is one of the most severe ocular allergic diseases. RECENT FINDINGS: A vicious cycle of inflammation occurs as a result of a set of reciprocal interactions between the c

Evaluation of the AlgerBrush II rotating burr as a tool for inducing ocular surface failure in the New Zealand White rabbit.

The New Zealand White rabbit has been widely used as a model of limbal stem cell deficiency (LSCD). Current techniques for experimental induction of LSCD utilize caustic chemicals, or organic solvents applied in conjunction with a surgical limbectomy. While generally successful in depleting epithelial progenitors, the depth and severity of injury is difficult to control using chemical-based methods. Moreover, the anterior chamber can be easily perforated while surgically excising the corneal limbus. In the interest of creating a safer and more defined LSCD model, we have therefore evaluated a

Squishy matters - Corneal mechanobiology in health and disease.

The cornea, as a dynamic and responsive tissue, constantly interacts with mechanical forces in order to maintain its structural integrity, barrier function, transparency and refractive power. Cells within the cornea sense and respond to various mechanical forces that fundamentally regulate their morphology and fate in development, homeostasis and pathophysiology. Corneal cells also dynamically regulate their extracellular matrix (ECM) with ensuing cell-ECM crosstalk as the matrix serves as a dynamic signaling reservoir providing biophysical and biochemical cues to corneal cells. Here we provid

Timeframe to Keratoplasty After Ocular Surface Stem Cell Transplantation.

PURPOSE: The aim of this study is to report the actual time delay between ocular surface stem cell transplantation (OSST) and keratoplasty at a high-volume center for limbal stem cell deficiency (LSCD). Understanding expected timelines and reasons for any delays or deferrals can allow for more accurate expectation-setting discussions with prospective patients. METHODS: A retrospective chart review was performed on all patients having undergone OSST between January 2013 and December 2022. Charts were reviewed to determine whether the need for keratoplasty was specified at the time of OSST plan

Nano-engineered mesenchymal stem cells as a platform for targeted and biologically regulated drug delivery in ocular surface inflammation.

Transplantation of human mesenchymal stem cells (hMSCs) has demonstrated safety and efficacy in the treatment of the inflamed and damaged ocular surface in patients with limbal stem cell deficiency (LSCD). However, this strategy alone is often insufficient to fully resolve clinical signs such as corneal opacity, neovascularization, ulceration, chronic inflammation, vision loss, and pain. The aim of this work was to nano-engineer hMSCs with fluorometholone-loaded nanostructured lipid carriers (FML-NLCs) to develop a combined cell-based drug delivery strategy capable of achieving sustained drug

Ocular Effects of Sulfur Mustard and Therapeutic Approaches.

Sulfur mustard (SM) is a strong blistering, highly reactive, lipophilic chemical war agent that causes injury in different organs including the skin, eyes, and respiratory tract. The Eyes are especially susceptible to the consequences of SM poisoning because of the aqueous and mucosal nature of conjunctiva and cornea. DNA alkylation and depletion of glutathione, are the most important mechanisms of SM action in the eye injuries. Acute clinical symptoms are including decrease in visual acuity, dryness, photophobia, blepharospasm, conjunctivitis, and complaints of foreign body sensation and sore

Blood-Based Treatments for Severe Dry Eye Disease: The Need of a Consensus.

The use of blood-based eye drops as therapy for various diseases of the ocular surface has become increasingly popular in ophthalmic practice during recent years. The rationale for their use is based on the promotion of cellular proliferation and migration thanks to the supply of metabolically active substances, in particular growth factors. Blood-derived eye drops have been used for the treatment of several ocular surface disorders, such as dry eye disease, corneal ulcer, persistent epithelial defect, neurotrophic keratitis, ocular surface burn, recurrent corneal erosion, and limbal stem-cell

Inflammation, vascularization and goblet cell differences in LSCD: Validating animal models of corneal alkali burns.

Limbal stem cell deficiency (LSCD) is one of the serious cause of visual impairment and blindness with loss of corneal clarity and vascularization. Factors such as ocular burns (acids, lime, thermal), genetic disorders or infections results in the loss of limbal stem cells leading to LSCD. Reliable animal models of LSCD are useful for understanding the pathophysiology and developing novel therapeutic approaches. The purpose of the present study was to validate small and large animal models of LSCD by immunohistochemcal, clinical and histopathological comparison with human. The animal models of

Ocular surface abnormalities in aniridia.

PURPOSE: The purpose of this study was to examine ocular surface abnormalities in aniridia. METHODS: We prospectively studied the ocular surface in nine consecutive patients (18 eyes) with virtually total aniridia. Of the 18 eyes, two eyes (two patients) were excluded from the study because severe surgical damage was likely to be the main cause of their corneal abnormalities. Six volunteers whose eye examinations yielded normal results served as control subjects. Detailed slit-lamp biomicroscopic examinations were performed to assess corneal findings and confirm the presence or absence of the

Induced Pluripotent Stem Cells in Corneal Regeneration: Biological Progress, Translational Barriers and Clinical Outlook.

Corneal blindness remains a major cause of visual impairment worldwide and may result from trauma, infectious keratitis, degenerative disorders, endothelial dysfunction, and limbal stem cell deficiency (LSCD). Although corneal transplantation remains the standard treatment for advanced disease, its effectiveness is limited by donor tissue shortage, immune-mediated rejection, postoperative complications, and progressive graft failure. These limitations have accelerated interest in regenerative approaches aimed at restoring native corneal structure and function. Induced pluripotent stem cells (i

Decoding cellular plasticity and niche regulation of limbal stem cells during corneal wound healing.

BACKGROUND: Dysfunction or deficiency of corneal epithelium results in vision impairment or blindness in severe cases. The rapid and effective regeneration of corneal epithelial cells relies on the limbal stem cells (LSCs). However, the molecular and functional responses of LSCs and their niche cells to injury remain elusive. METHODS: Single-cell RNA sequencing was performed on corneal tissues from normal mice and corneal epithelium defect models. Bioinformatics analysis was performed to confirm the distinct characteristics and cell fates of LSCs. Knockdown of Creb5 and OSM treatment experime

Descemet stripping endothelial keratoplasty following simple limbal epithelial transplantation for chemical injury-related endothelial dysfunction: two case reports.

INTRODUCTION AND IMPORTANCE: Ocular chemical injury primarily results in corneal epithelial damage and limbal stem cell deficiency (LSCD), leading to delayed epithelial healing and recurrent epithelial breakdowns. Simple limbal epithelial transplantation (SLET) is the most promising surgical modality for the treatment of unilateral LSCD, helping restore and stabilize the ocular surface. However, in some instances, chemical injury can also result in corneal endothelial damage. This under-recognized entity can mimic SLET failure, leading to delayed or inappropriate management.
