Serum linezolid concentrations are reduced in critically ill patients with pulmonary infections: A prospective observational study.

RATIONALE: The concentration-time profile of linezolid varies considerably in critically ill patients. Question of interest is, if the site of infection influences linezolid serum concentrations. METHODS: 68 critically ill patients, treated with linezolid, were included. The concentration-time-profile for linezolid was determined using maximum a-posteriori predictions. A trough concentration (Cmin) between 2 and 10 mg/L was defined as the target. A generalized linear model (GLM) was established to evaluate potential covariates. RESULTS: The indications for linezolid therapy were in descendin…

Linezolid Administration to Critically Ill Patients: Intermittent or Continuous Infusion? A Systematic Literature Search and Review.

A judicious antibiotic therapy is one of the challenges in the therapy of critically ill patients with sepsis and septic shock. The pathophysiological changes in these patients significantly alter the antibiotic pharmacokinetics (PK) and pharmacodynamics (PD) with important consequences in reaching the therapeutic targets or the risk of side effects. The use of linezolid, an oxazolidinone antibiotic, in intensive care is such an example. The optimization of its therapeutic effects, administration in intermittent (II) or continuous infusion (CI) is gaining increased interest. In a systematic re…

An Unusual Presentation of Community-Acquired Methicillin-Resistant Staphylococcus aureus Infection in a Child Treated With Linezolid.

Methicillin-resistant Staphylococcus aureus (MRSA) infection is a major public health concern. MRSA isolates are classified into community-a … comycin but resistant to the other anti-MRSA antibiotics. The patient was successfully treated with linezolid after clinical deterioration with vancomycin. A literature review comparing vancomycin and linezolid in invasive MRSA infections among children indicated that linezolid has better lung and tissue penetration than vancomycin, and an early switch is w …

Update on prevalence and mechanisms of resistance to linezolid, tigecycline and daptomycin in enterococci in Europe: Towards a common nomenclature.

Vancomycin-resistant enterococci (VRE) are important nosocomial pathogens. Invasive VRE infections are difficult to treat since common therapeutic options including ampicillin and glycopeptides often fail. In vitro, most VRE remain susceptible to last-resort antibiotics such as linezolid, tigecycline and daptomycin. However, neither tigecycline nor linezolid act in a bactericidal manner, and daptomycin has proven activity only at high dosages licensed for treating enterococcal endocarditis. Despite these pharmacological and therapeutic limitations, reports on resistance to these last-resort dr…

Rapid and sensitive determination of the antibiotic linezolid in low plasma volumes by high-performance liquid chromatography.

A rapid and sensitive method for the determination of linezolid by high-performance liquid chromatography (HPLC) with UV detection (251 nm) is presented. Linezolid is an important antibiotic against severe infections caused by multi-resistant bacterial pathogens. Scientific efforts continue investigating its effectiveness in different conditions and patient populations including children and newborns. Because plasma samples in a pediatric setting or from animal models are usually collected in low volumes, there is a necessity for a reliable and precise analytical method that is reliable and pr…

Transcriptomic signatures predict severe linezolid-induced peripheral neuropathy and reveal pre-treatment immune and antioxidant dysregulation.

Linezolid is an essential drug for treating multidrug-resistant and extensively drug-resistant tuberculosis, yet its long-term use is frequently limited by peripheral neuropathy, which in severe cases may be irreversible. To investigate biological factors underlying susceptibility, we analyzed transcriptomic profiles of peripheral blood mononuclear cells from 51 MDR/XDR-TB patients before and after six months of linezolid treatment, stratifying individuals into asymptomatic, mild, and severe neuropathy groups. Patients who later developed severe neuropathy already exhibited distinct transcript…
