[Comments on emergency treatment of biliary lithiasis in patients over 75 years of age. Apropos of 157 cases].

Surgery remains the ideal emergency treatment for biliary lithiasis in elderly subjects despite perioperative morbidity and mortality. Minimally invasive techniques appear promising but require assessment. The aim of this work was to determine the usefulness of these techniques and evaluate outcome in a series of 157 patients over 75 years of age who were hospitalized in an emergency setting of complicated biliary lithiasis from January 1990 to December 1996. There were 103 women and 54 men, mean age 82 years. The patients' general status was evaluated according to the ASA classification; 66% …

[Ultrasonography and retrograde transpapillary cholangiopancreatography combined with sphincterotomy in the diagnosis of choledochal lithiasis. Our experience with 87 patients].

We retrospectively reviewed the US findings of 87 patients to assess US sensitivity in the detection of choledochal (CBD) stones. Endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (ES) was used as the diagnostic standard, except for 1 patient who underwent surgical exploration. First, the distal CBD was studied with transverse and parasagittal US scans through the head and uncinate portions of the pancreas. CBD stones usually appeared as hyperechoic lesions, most of them with acoustic shadowing. In some cases, the stone moves during real-time scanning, which

[Changes in the pattern of serum biliary acids and lipoprotein picture during therapy with biliary acids in cholesterol lithiasis].

Several studies are present in the literature about the efficacy of medical treatment with biliary acid, orally administered, on the symptomatic gallstone patients. Both this drugs act, with different mechanisms, on the same pathophysiological pathway, represented by the supersaturated bile in cholesterol. Aim of the present investigation was to study the possible modifications of serum bile acids pattern and of lipoproteins in a large sample of gallstone patients under long-term treatment with biliary acids. One hundred and twelve patients with radiolucent gallstones entered the study; 54 rec

[Intrahepatic biliary lithiasis : role of Ascaris infection in 150 cases. Experimental study on bile stone formation (author's transl)].

Biliary infection was the cause of intrahepatic biliary lithiasis in 150 patients operated upon over the last twenty years. Diagnosis was established during operation in the majority of cases, but cholangiography, or ultrasound and/or scintiscan examinations were required in a few patients. To determine the mechanism of formation of these biliary stones, a study was conducted in 30 rabbits divided into three groups, each forming a model of lithogenesis. Pathological examination of the liver demonstrated the presence of multiple abscesses, fibrosis, bile duct stenosis, and perihepatitis. The re…

Laparoscopic intercostal segment 7 resection for intrahepatic lithiasis due to low phospholipid-associated cholelithiasis syndrome. A case report.

INTRODUCTION: Low phospholipid-associated cholelithiasis (LPAC) syndrome is a rare biliary disorder caused by mutations in the ABCB4 gene, which encodes the MDR3 phosphatidylcholine transporter. It primarily affects young adults and may persist or recur following cholecystectomy. LPAC is characterized by intrahepatic lithiasis and recurrent biliary symptoms. PRESENTATION OF CASE: We report the case of a 27-year-old woman with a history of recurrent acute pancreatitis and prior laparoscopic cholecystectomy, who presented with ongoing episodes of biliary colic. Laboratory tests revealed leukocy…

Case Report: Carcinoma Ex-Pleomorphic Adenoma With Concurrent Submandibular Gland Lithiasis-A Rare Association With Potential Pathophysiological Implications.

This case report details the exceptionally rare co-occurrence of submandibular gland lithiasis and carcinoma ex-pleomorphic adenoma (CXPA), representing only the fourth documented instance worldwide. The patient, a 79-year-old male with a 10-year history of recurrent submandibular symptoms, was found to have both ductal and intraglandular stones alongside non-uniform glandular enlargement. Post-surgical analysis confirmed CXPA. The patient remained disease-free with good functional outcomes at 24-month follow-up. This case provides valuable insights, suggesting a potential pathophysiological l…

Transoral removal of submandibular hilar lithiasis: results on the salivary duct system, glandular parenchyma, and quality-of-life recovery.

OBJECTIVE(S): To confirm that hilar transoral submandibular sialolitectomy (TOSL) is the first treatment option for submandibular hilar lithiasis (SHL) in terms of glandular parenchyma recovery, salivary system restoration, and patient quality of life (QoL) improvement. METHODS: Depending on whether the stone was easily palpable, TOSL was carried out with or without sialendoscopy. For the first time in the literature, Magnetic Resonance Sialography (MR-Si) was performed before and after TOSL, to evaluate stone characteristics, glandular parenchyma status, hilum dilation and main duct recanali…

A Case of Biliary Intraepithelial Neoplasm in a Young Man Diagnosed by Laparoscopic Hepatectomy to Treat Recurrent Intrahepatic Lithiasis and Cholangitis.

INTRODUCTION: Biliary intraepithelial neoplasia (BilIN) is defined as a bile duct epithelial tumor with intraductal papillary neoplasia of the bile duct. BiIlN is a precancerous lesion of intrabiliary neoplasia. We performed laparoscopic hepatic resection for recurrent cholangitis due to intrahepatic lithiasis and diagnosed BilIN. This case suggests that it is necessary to consider the possibility of malignancy in cases of repeat cholangitis due to intrahepatic lithiasis. CASE PRESENTATION: A 34-year-old man developed cholecystitis due to gallstones at the age of 25 years and underwent laparo…

Complex Intrahepatic Lithiasis: A Case Report of Combined Treatment With Surgical Exploration of the Bilioenteric Anastomosis and Laser Lithotripsy by Cholangioscopy.

Intrahepatic lithiasis, or hepatolithiasis, is an endemic disease in southeast Asia, although, with immigration from Eastern countries, the incidence of this pathology is rising worldwide. The Latin American experience demonstrates morbidity and mortality compatible with other Western countries, but minimally invasive procedures are lacking. We demonstrate a case of a combined surgical and endoscopic approach for stone clearance. We present a case of a 47-year-old female patient with biliary enteric anastomosis to treat recurrent pyogenic cholangitis resulting from intrahepatic lithiasis. The …

S-Methyl-L-Ergothioneine to L-Ergothioneine Ratio in Urine Is a Marker of Cystine Lithiasis in a Cystinuria Mouse Model.

Cystinuria, a rare inherited aminoaciduria condition, is characterized by the hyperexcretion of cystine, ornithine, lysine, and arginine. Its main clinical manifestation is cystine stone formation in the urinary tract, being responsible for 1-2% total and 6-8% pediatric lithiasis. Cystinuria patients suffer from recurrent lithiasic episodes that might end in surgical interventions, progressive renal functional deterioration, and kidney loss. Cystinuria is monitored for the presence of urinary cystine stones by crystalluria, imaging techniques or urinary cystine capacity; all with limited predi…

Acute Urinary Retention Secondary to Urethral Lithiasis in a 4-Year-Old Boy: How We Managed This Rare Case.

Urethral stone impaction (USI) is an extremely rare cause of acute urinary retention (AUR) in pediatric urology. Few case reports are available, and no consensus guidelines currently exist for managing this condition. We describe our management of such a case and provide a review of the relevant literature. A 4-year-old boy with no prior urological history presented to our emergency department with abdominal pain lasting 8 days. An ultrasound performed 5 days earlier identified a 7-mm stone at the ureterovesical junction. Symptomatic treatment with paracetamol and non-steroidal anti-inflammato
