STUDY DESIGN: A retrospective clinical study.
OBJECTIVE: To analyze and evaluate the clinical outcomes of cerebrospinal fluid (CSF) leak after anterior decompression for cervical ossification of the posterior longitudinal ligament (OPLL) with or without dural ossification (DO).
SUMMARY OF BACKGROUND DATA: Anterior decompression can be highly efficacious in the treatment of OPLL. However, in some cases of OPLL, there often exists DO and fusion with the posterior longitudinal ligament, which may increase the chance for CSF leak during an anterior decompression surgery.

UNLABELLED: Sympathetic symptoms associated with cervical disorders, such as vertigo, headache, dizziness, etc., are common clinical disorders bewildering both clinicians and patients. In our clinical practice we observed that sympathetic symptoms associated with cervical disorders were apparently relieved in some patients after undergoing routine anterior cervical decompression and fusion plus posterior longitudinal ligament (PLL) resection. This study was designed to investigate the sympathetic nerve innervations in the cervical PLL and its potential correlation with cervical sympathetic sym

Our objective was to evaluate the clinical efficacy and feasibility of posterior decompression with transforaminal thoracic interbody fusion (PTTIF) for thoracic myelopathy caused by ossification of the ligamentum flavum (OLF) and ossification of the posterior longitudinal ligament (OPLL) at the same level. Between March 2004 and December 2008, 13 patients (five men and eight women, average age: 56years, range: 39-72years) who underwent PTTIF for concurrent OLF and OPLL were studied retrospectively. The clinical efficacy, operative time, blood loss, sagittal alignment and complications were in

Many studies have reported successful outcomes less than 10 years after anterior cruciate ligament (ACL) reconstruction. However, longer-term outcomes have not been analyzed. We assessd outcomes 24 years after anterior cruciate ligament reconstruction with iliotibial tract and compared them with shorter-term results in the same patients. Between 1979 and 1981, 45 patients underwent combined intra- and extra-articular ACL reconstruction with iliotibial tract. Follow-up evaluations of these patients were performed at 6, 13, and 24 years after surgery, which included manual and instrumental laxit

A retrospective study of the outcomes of multilevel anterior decompression and interbody fusion for cervical myelopathy due to ossification of the posterior longitudinal ligament (OPLL) was performed to both investigate the long-term results and assess the cause of late deterioration. Twenty-seven patients (mean age, 58.1 years) underwent this procedure and were followed for at least 5 years. The severity of the clinical symptoms was described using the scoring system for cervical myelopathy proposed by the Japanese Orthopaedic Association (JOA score). The average preoperative JOA score was 7.

BACKGROUND: Multilevel anterior cervical corpectomy with fusion (ACF) offers direct resection of spondylostenosis and ossification of the posterior longitudinal ligament (OPLL) with immediate stabilization. Ideal candidates for multilevel ACF include younger patients (<65 years of age), or older individuals (>65 years of age) with loss or reversal of the cervical lordosis (kyphosis).
METHODS: Sixty-five patients, averaging 56 years of age and including 40 males and 25 females, with multilevel MR- and CT-documented spondylostenosis and OPLL were studied. Preoperatively, patients exhibited mode

Spinal neuromas related to chronic irritation or remote trauma may arise from unrecognized repetitive minor injury to the spinal cord or nerve roots, yet detailed descriptions with precise preoperative imaging findings are rare. We report a 59-year-old man with a 6-month history of severe dysesthesia and numbness in the neck and left upper limb, followed by progressive weakness over 2 months. He had undergone splenectomy after a traffic accident 23 years earlier. Computed tomography revealed ossification of the posterior longitudinal ligament, osteophytes, and a disc herniation at the left C5/

OBJECTIVE: Surgical management of nonambulatory patients with beak-type thoracic ossification of the posterior longitudinal ligament (T-OPLL) remains challenging, and the optimal extent of decompression is unexplored. This study aimed to evaluate neurological outcomes and perioperative characteristics of a selective posterior decompression with fusion (PDF)-first strategy, with circumferential decompression (CD) performed only when indirect decompression was deemed insufficient based on intraoperative assessment.
METHODS: Nonambulatory patients with beak-type T-OPLL who underwent thoracic spi

IntroductionCervical laminoplasty using hydroxyapatite spacers is a surgical procedure widely performed for managing spondylotic myelopathy and ossification of the posterior longitudinal ligament (OPLL). Despite the procedure's efficacy, the mechanisms underlying bone formation in OPLL remain poorly understood, and postoperative ectopic ossification may still occur. After laminoplasty, we compared bone bridge formation between adjacent hydroxyapatite spacers in patients with cervical spondylotic myelopathy and OPLL.Materials and methodsA retrospective analysis was conducted on 30 patients who

BACKGROUND: Intraoperative hypotension (IOH) is a recognized contributor to changes in motor evoked potential (MEP) during spinal surgeries. Additionally, it is essential to precisely define IOH across different surgical phases. However, there is limited data on optimal IOH thresholds for predicting MEP changes in thoracic ossification of the posterior longitudinal ligament (OPLL) and/or ossification of the ligamentum flavum (OLF) surgery. We aim to determine the IOH thresholds for predicting MEP changes during surgical treatment for OPLL and/or OLF based on different surgical phase.

BACKGROUND: There is currently no established index for evaluating the adequacy of the surgical area during laminoplasty. This study aims to propose a novel concept of the drifting angle of the spinal cord (DA-SC) at the cephalic and caudal junction of surgical and nonsurgical laminae during laminoplasty, and to assess its clinical relevance.
METHODS: A retrospective review of clinical and radiological data was conducted on 296 patients who underwent laminoplasty. Patients were classified into four groups based on preoperative DA-SC of cephalic and caudal junction of surgical and nonsurgical

BACKGROUND: Ossification of the posterior longitudinal ligament (OPLL) combined with dural ossification (DO) significantly increases the technical difficulty and risk of complications in spinal canal decompression surgery. The pathological mechanisms underlying DO remain incompletely elucidated, and standardized imaging diagnostic criteria are currently lacking. CASE PRESENTATION: This study reports a 59-year-old female diagnosed with mixed-type cervical spondylopathy, OPLL at C5/6 and C6/7 levels, and hypertension. Preoperative computed tomography (CT) revealed flat-type OPLL at C5/6 and spik

Cervical ossification of the posterior longitudinal ligament (OPLL) causes spinal cord compression due to spinal canal stenosis. Traditional anterior approaches such as anterior cervical discectomy and fusion (ACDF) and anterior cervical corpectomy and fusion (ACCF) have limitations, including restricted decompression range, extensive vertebral resection trauma, or insufficient stability. This study aimed to evaluate the efficacy and safety of a novel surgical technique-Anterior Cervical V-Slot Decompression and Fusion (ACVDF)-for treating long-segment OPLL. A retrospective analysis was conduc

OBJECT: With the development of new technologies, the surgical algorithm for ossification of posterior longitudinal ligament (OPLL) in the cervical spine also needs to be updated. The aim of this study is to elucidate a new surgical classification algorithm to guide the choice of surgical approaches for cervical OPLL based on its location and extent. In this algorithm, anterior controllable antedisplacement and fusion (ACAF) will be used as a new surgical option.
METHODS: This is a single-centered, retrospective, cohort study utilizing a novel algorithm based on the following three criteria:
