[Panic disorder and transcranial magnetic stimulation].

Transcranial magnetic stimulation (TMS) has been tried in some Anxiety Disorders (Obsessive-compulsive disorder and Posttraumatic Stress Disorder) with different results. We present a pilot study including three Panic Disorder patients. The subjects who were enrolled had a history of the disease for at least 1 year and they had unsuccessfully followed psychotherapy and pharmacological treatment. The patients received 10 sessions during two weeks; each session lasted 30 trains of 60 seconds at a frequency of 1 Hz, on the right dorsolateral prefrontal cortex, at 110% of the motor threshold. All

Magnetic resonance angiography: the state of the art.

Three-dimensional contrast magnetic resonance angiography has rapidly advanced over recent years. It is now a highly accurate and safe method of diagnosing vascular abnormalities of the thoracic, abdominal and peripheral vessels. We describe techniques for the examination of the thoracic and abdominal aorta, the renal arteries and the lower limb vessels together with strategies to improve their diagnostic accuracy.

Magnetic resonance imaging of the foot and ankle.

Magnetic resonance (MR) imaging of the foot and ankle is playing an increasingly important role in the diagnosis of a wide range of foot and ankle abnormalities, as well as in planning for their surgical treatment. For an optimal MR study of the foot and ankle, it is necessary to obtain high-resolution, small-field-of-view images using a variety of pulse sequences. The most common indication for MR imaging of the foot and ankle is for the evaluation of tendon and bone abnormalities, such as osteomyelitis, occult fractures, and partial and complete tears of the Achilles, tibialis posterior, and

[Magnetic resonance tomography in oncology diagnostics].

MR is well suited for imaging in patients with malignant disease. It is the most sensitive and most specific method in the detection of skeletal metastases. It clearly demonstrates the extent of primary bone tumours and also reveals skip metastases and invasion into neighbouring joints, hence it is an important procedure when surgery of a bone tumour is planned. In case of a soft tissue tumour, MR is the preferable imaging modality because it demonstrates the anatomy and reveals the different tissue constituents. MR is the best method to show tumour manifestations in the central nervous system

Magnetic resonance spectroscopy of brain in epilepsy.

The current applications of magnetic resonance spectroscopy (MRS) in the clinical management of epileptic patients are reviewed. A major contribution of MRS to epilepsy is its ability to determine lateralisation before surgical resection of the diseased brain region. Phosphorus-31 and proton single-voxel MRS identify abnormalities in high-energy metabolism, neuronal function and neurotransmitter levels, but information can only be obtained from restricted regions of the brain. Spectroscopic imaging techniques (also known as chemical shift imaging) provide a metabolic mapping of the whole brain

Choledochocele imaged with magnetic resonance cholangiography.

Choledochal cysts are rare developmental malformations of the biliary tree. Percutaneous and endoscopic ultrasound, as well as endoscopic retrograde cholangiopancreatography, are recommended diagnostic tools. Magnetic resonance cholangiography may also contribute to the workup and treatment plan of patients with choledochal cysts. We report a 25-yr-old white woman with episodic nausea and moderate epigastric discomfort. Magnetic resonance cholangiography showed a rather spherical, cyst-like, fluid-filled structure in continuity with the prepapillary segment of the common bile duct, thus making

Magnetic resonance study of the pancreatic duct.

AIM: to prospectively determine the diagnostic accuracy of magnetic resonance cholangiopancreatography (MRCP) in the evaluation of the normal and diseased pancreatic duct. METHODS: patients seen during a 6-month period with a diagnosis of biliary tract or pancreatic disease underwent endoscopic retrograde cholangiopancreatography (ERCP) after a previous MRCP. The pancreatic duct was evaluated with both techniques in 37 patients. RESULTS: the pancreatic duct appeared normal in ERCP in 27 patients, and also appeared normal in MRCP in 25 of these patients (specificity 93%). ERCP showed moderate

Endosonography and endoscopic magnetic resonance imaging.

Endosonography is an important modality for the diagnosis and staging of oesophageal, gastric, colorectal and pancreatobiliary malignancy. It is also recognized as a reliable method for the evaluation of submucosal tumours of the gastrointestinal tract, for differentiating benign lesions from giant gastric folds, and for the localization of pancreatic endocrine tumours. The latest development, that of endosonographic fine-needle aspiration, provides for the cytological diagnosis of gastrointestinal tumours. This new technique may also be used for endoscopic therapy. High-frequency probes can b

Fast magnetic resonance imaging of the heart.

Fast MR imaging techniques have multiple applications for evaluation of cardiac disease. Cine MRI and MR tagging have been shown to be highly accurate and reproducible in evaluating regional and global myocardial function. Segmented k-space cine MRI and echo-planar imaging (EPI) can considerably improve time efficiency and thereby the clinical utility of these techniques. Double IR fast spin-echo sequences enable breath-hold acquisition of T2 weighted MRI with good suppression of the blood signal. Myocardial perfusion can be assessed with fast dynamic MRI after administration of contrast media

Filariasis of the ankle: magnetic resonance imaging.

Filariasis is a world health problem that is frequently seen in tropical and subtropical countries. In endemic areas, the clinical spectrum of extremity swelling, lymphangitis, or elephantiasis is usually recognized as filariasis. In the United States, diagnosis of the disease may be more difficult because of lack of familiarity with this infection. We present a case of filaremic arthropathy of the ankle joint and the magnetic resonance imaging (MRI) findings of this disease. It is the first reported case of MRI findings in a human patient. MRI has been done on animal models with filariasis, a

Magnetic resonance imaging in oncology: an overview.

This review discusses a growing area of imaging in oncology. Traditionally, the primary role of magnetic resonance imaging (MRI) has been in the investigation of neurological diseases and in the diagnosis of musculoskeletal abnormalities. With the increasing availability of MRI systems and with the advances in technology, the role of this modality out with these areas is rapidly expanding. This review outlines the areas where MRI has a specific role in the imaging of patients with cancer. In particular, emphasis is placed on areas outside of the central nervous and musculoskeletal systems. Thi

Magnetic resonance imaging features of allografts.

OBJECTIVE: To investigate the magnetic resonance imaging (MRI) features of allografts at various time intervals after surgery in patients with osteoarticular allografts. DESIGN AND PATIENTS: Sixteen patients who were treated with osteoarticular allografts and who were followed over time with MRI studies as part of their long-term follow-up were retrospectively selected for this study. T1-weighted images were obtained both before and after gadolinium administration along with T2-weighted images. All images were reviewed by an experienced musculoskeletal radiologist, with two other experienced

[Magnetic resonance imaging in musculoskeletal tuberculosis].

The incidence of tuberculosis in Denmark has increased over the last years. Two cases of primary osteoarticular tuberculosis are described: tuberculous ostitis of the scapula and surrounding soft tissue, and tuberculosis of the elbow. We have emphasized the appearance on magnetic resonance imaging (MRI), as the role of this modality has only been sporadically described for osteoarticular tuberculosis in areas other than the vertebral column. The cases show that MRI can contribute to a more precise determination of the extent of osteoarticular tuberculosis and its soft tissue involvement.

Magnetic resonance imaging of a mediastinal ependymoma.

Ependymomas typically occur in the brain and spinal cord, and represent 5% and 16% of primary intracranial and spinal cord tumors respectively. Ependymomas arising outside the central nervous systems are uncommon and when they do occur are usually seen in the sacrococcygeal region. Other reported sites include the pelvis, spinal nerve roots, and lung. An extraspinal mediastinal ependymoma is a rare occurrence, with only two previously reported cases to our knowledge. In both instances, the lesion arose in the posterior mediastinum, and were imaged with either chest radiographs alone or chest r

Magnetic resonance imaging of the shoulder in abduction.

To simulate the painful arm position in subacromial impingement syndrome, magnetic resonance imaging of the shoulder was done with the arms at the sides and at 80 degrees abduction in two groups of patients. Group A had 20 shoulders in 20 patients with subacromial impingement syndrome and Group B had 19 shoulders in 19 patients without impingement syndrome. When arm position was changed from adduction to abduction, an increase in signal intensity of the rotator cuff tendon was seen more often in Group A (45%) than in Group B (26%). Group A shoulders showed encroachment of the acromion or the a

Optic neuritis: imaging with magnetization transfer.

OBJECTIVE: Our objective was to prospectively examine the optic nerves in patients with clinically severe unilateral optic neuritis, using routine spin-echo and magnetization transfer MR imaging. SUBJECTS AND METHODS: For 39 patients with such lesions, we calculated the magnetization transfer ratio along the involved intraorbital optic nerve and along the asymptomatic contralateral optic nerve in a mirror-image location. Magnetization transfer ratios were correlated with the imaging findings on routine spin-echo MR imaging. RESULTS: Magnetization transfer ratios were decreased in 33 of the 3
