We describe the case of a patient with a lumbar disc herniation who underwent a course of side posture manipulation. Despite the appearance of an enormous central herniation on the CT scan, the patient improved considerable during only 2 wk of treatment. The disparity which so commonly exists between radiological and clinical findings is depicted in this case. Further, it is emphasized that manipulation has been shown to be an effective treatment for some patients with lumbar disc herniation. While complications of this form of treatment have been reported in the literature, such incidents are

OBJECTIVE: To review the case of a missed cervical spine fracture in a patient with neck pain.
CLINICAL FEATURES: A 21-year-old thatcher with neck pain presented to a chiropractic clinic after a fall from a roof 4 m high. The hospital radiographs were read as normal. The chiropractor suspected a Jefferson fracture of the atlas after evaluation of the same radiographs.
INTERVENTION AND OUTCOME: The chiropractor retook the anteroposterior open-mouth radiograph to confirm the suspected fracture. The patient was referred for further imaging and underwent neurosurgical treatment, after which he r

No data are available on the incidence of pelvic pain and organic dysfunction (PPOD) in patients suffering from low back pain. PPOD is not an uncommon finding in the low back pain patient. Women appear to be more frequently involved than men. The results of recent electrophysiologic investigations indicate that many patients with urological, bowel or anorectal dysfunction demonstrate evidence of denervation neuropathy in muscles innervated by the branches of the pudendal nerve. Six patients with low back pain meeting predetermined criteria, indicating the presence of PPOD as a result of suspec

OBJECTIVE: The purpose of this study was to determine if the addition of a self-acupressure pillow (SAP) to typical chiropractic treatment results in significantly greater improvement in tension-type and cervicogenic headache sufferers.
METHODS: A pragmatic randomized clinical trial was conducted in a chiropractic college teaching clinic. Thirty-four subjects, including tension-type and cervicogenic headache sufferers, 21 to 60 years of age, male or female, completed the study. Group A (n = 15) received typical chiropractic care only (manual therapy and exercises), and group B (n = 19) receiv

A review of the literature reveals strong evidence for both the mechanical model of disease production (structural) and the neurobiological model (functional). Outdated models which attempt to describe a scientific basis for chiropractic theory are inadequate and indeed harmful to the progress and acceptance of chiropractic. Pragmatic or empirical arguments that "Chiropractic works and that's what counts" have served a useful purpose, but now must be augmented by extant research findings. The "paradigm shift" is on. Research investigators around the world are focusing on the multiple component

Text neck describes an overuse injury of the cervical spine resulting from the repetitive stress of prolonged forward head flexion while looking down on a mobile screen. This case report describes a 24-year-old young man who presented with a 12-month history of head and neck pain and paresthesia of the right upper limb. The patient worked as a YouTuber and has been editing and posting videos on the website for three years. One year prior to referral for chiropractic assessment, the patient first visited his family physician for similar complaints. Based on cervical radiographs, the diagnosis o

OBJECTIVE: This report discusses a patient with a pathologic fracture of the C2 vertebra secondary to osteolytic metastasis from squamous cell carcinoma of the lung.
CLINICAL FEATURES: The patient was a 68-year-old man with a chief complaint of neck pain who was referred by his physician to a chiropractic office. The initial onset of neck pain began after a forceful sneeze that resulted in a sensation of "a twig snapping" in the neck. Radiographs revealed osteolytic destruction and pathologic fracture of the C2 spinous process.
INTERVENTION AND OUTCOME: The patient was referred back to his p

BACKGROUND: The term 'acute facet syndrome' is widely used and accepted amongst chiropractors, but poorly described in the literature, as most of the present literature relates to chronic facet joint pain. Therefore, research into the degree of consensus on the subject amongst a large group of chiropractic practitioners was seen to be a useful contribution.
METHODS: During the annual congress of The European Chiropractors Union (ECU) in 2008, the authors conducted a workshop involving volunteer chiropractors. Topics were decided upon in advance, and the participants were asked to form into gr

CONTEXT: Depression and other mood disorders have a significant impact on patients, affecting their daily functioning, physical health, and emotional well-being. While psychopharmacological medications and psychotherapy are the most common evidence-based treatments (supported by a significant amount of scientific research), there is growing research that supports the use of complementary therapies (nonmainstream approaches utilized together with conventional medicine), including manipulative or manual therapies.
OBJECTIVES: This systematic review contributes to the existing literature by eval

Subclavian steal syndrome caused by an acute thrombus is very rare. We present a case of cerebellar infarction with proximal subclavian artery thrombosis. A 56-year-old woman was admitted for sudden vertigo. One day prior to admission, she received a shoulder massage comprised of chiropractic manipulation. On examination, her left hand was pale and radial pulses were absent. Blood pressure was weak in the left arm. Downbeat nystagmus and a right falling tendency were observed. Brain MRI showed multiple acute infarctions in the left cerebellum. The findings of Doppler ultrasonography in the lef

Arnold-Chiari I malformation (Chiari I) is a congenital disorder characterized by caudal herniation of cerebellar tonsils through the foramen magnum. The symptoms and signs include headaches precipitated by coughing or exertion, dizziness, visual or oculomotor symptoms, dysphagia, trunk or extremity dysesthesias, ataxia, and drop attacks indicating cerebellar or cervical cord lesion. The symptoms may be provoked by increased intracranial pressure. The mean age of onset of symptoms is 25 years; consequently, previously unidentified Chiari I malformations occur in military personnel. Chiari I is

BACKGROUND: Spontaneous intracranial hypotension (SIH) results from spinal cerebrospinal fluid (CSF) leakage and typically presents with orthostatic headache. Spontaneous resolution without intervention is rare, and its underlying mechanism has not been established.
CASE DESCRIPTION: A 33-year-old female developed a SIH following chiropractic manipulation. The magnetic resonance imaging (MRI) demonstrated a ventral CSF leak at the Th2/3 level. During conservative management, she experienced acute back pain coinciding with the formation of an epidural hematoma at the same level; her headache r

We present the case of a 32-year-old woman with type III von Willebrand disease who, after a chiropractic manipulation, developed a large iliopsoas muscle hematoma with femoral neuropathy. Given the lack of recombinant factor VIII at our institution, we opted for a minimally invasive, interdisciplinary approach. The patient was clinically stable and fully adherent to the proposed interdisciplinary management plan. A percutaneous drainage was placed and supplemented with intralesional alteplase irrigations, which allowed for a progressive reduction of the hematoma, resolution of the neuropathy,

Spinal accessory nerve palsy (SANP) is rare and is commonly presented following iatrogenic injury. Their diagnosis is often missed on initial presentation. Injury following blunt trauma is rare, with few cases reported in literature describing blunt-associated SANP and their treatment and recovery. We present and discuss a case of SANP following an aggressive soft tissue adjustment by an uncertified individual that has been responsive to nonsurgical measures over 18 months. We also reviewed the related literature on similar cases that were presented as result of direct pressure on the nerve fr

BACKGROUND: Eagle syndrome (ES) was first described in 1937, to characterize elongation of the styloid process. It is rarely encountered by neurosurgeons but does present the potential for vascular sequelae and neurological complications.
DEMONSTRATIVE CASES: We discuss three patients with uncommon presentations of neurovascular compromise with uncommon symptomatology, secondary to ES. Their management ranged from retrospective diagnoses following self-limited events, antiplatelet therapy, and endovascular and surgical interventions.
DISCUSSION: While traumatic fractures, chiropractic manipu

A palsy of the long thoracic nerve of Bell is a cause of scapular winging that has been reported after trauma, surgery, infection, electrocution, chiropractic manipulation, exposure to toxins, and various sports-related injuries that include tennis, hockey, bowling, soccer, gymnastics, and weight lifting. Scapular winging can result from repetitive or sudden external biomechanical forces that may either exert compression or place extraordinary traction in the distribution of the long thoracic nerve. We describe an active duty Navy Airman who developed scapular winging secondary to traction to
