The relationship between mastoid pneumatization and the position of the sigmoid sinus.

Using high-resolution computed tomography, we measured the cross-sectional area of mastoid air cells and the shortest distance between the external auditory canal and the anterior edge of the sigmoid sinus (DIST), and then compared the right-left difference in 70 patients with unilateral chronic otitis media and 23 cases without middle ear disease. DIST was significantly short where there was poor mastoid pneumatization (P < 0.0001), regardless of whether it was the right or left ear. Furthermore, on the well-pneumatized temporal bone, the increase in size of the cross-sectional area was close…

[Latent, non-suppurative mastoiditis. Apropos of 62 cases].

The term masked mastoiditis defines a subclinical infectious inflammatory process of the mucosal lining and bony structures of the mastoid air cells with an intact tympanic membrane. The disease follows an apparently well treated recent acute otitis media. Probably due to an anaerobic colonizing flora, the developing bone infection is of low grade without pus formation. The clinical features of the disease are not overt as those in coalescent mastoiditis. The intact ear drum does not reflect the severity of bone eroding disease which is characterized by non-exudative but proliferative changes.…

[Anatomic characteristics of the mastoid in children. Pneumatization of the subfacial region].

Three specific features of the mastoid in the pediatric population appeared to us to be important in view of our per-operative findings on mastoidectomy for chronic suppurative otitis, cholesteatoma or middle ear effusion with chronic persistent mastoiditis in the older child. These included the position of the facial nerve, the superficial nature of its third part being well known, but also the existence of the Körner septum and especially the pneumatization of the sub-facial region which explains the similarity between certain infectious lesions of the retro-tympanic structures and infectiou…

Acute mastoiditis. Influence of antibiotic treatment on the bacterial spectrum.

The work was undertaken to investigate the spectrum of bacteria responsible for acute mastoiditis and to find out whether it is affected by intake of antibiotics prior to surgical treatment. The records were reviewed of 22 children with acute mastoiditis in whom mastoidectomy had been performed and bacterial cultures obtained. Eleven of the patients had had antibiotic treatment prior to admission (9 penicillin V and 2 erythromycin). Streptococcus pneumoniae was found in 8 of the purulent discharges: Haemophilus influenzae, Streptococcus beta-hemolyticus and Staphylococcus aureus in 2 each; Pro…

Granulocytic sarcoma (chloroma) initially seen as acute mastoiditis.

Granulocytic sarcoma (chloroma) is a localized tumefaction of immature granulocytes that is typically seen in association with myelogenous leukemia. The primitive cell population seen in biopsy material may be misinterpreted as histiocytic lymphoma or other sarcoma unless additional studies are performed. We saw a 36-year-old woman with promyelocytic leukemia in remission who had the signs and symptoms of an acute coalescent mastoiditis. Histologic examination of the surgical specimen, however, demonstrated a granulocytic sarcoma. Our case exemplifies some of the difficulties that may be encou…

Complications of mastoiditis with special emphasis on venous sinus thrombosis.

Two children with intracranial venous sinus thrombosis complicating mastoiditis prompted review of the experience with these disorders at the The Children's Memorial Hospital in Chicago over the past decade. Fifteen patients, 1.5 to 14 years of age, with mastoiditis were identified, including nine cases categorized as acute and six as chronic, the latter without fever but with chronic otorrhea. All six patients with chronic, but none with acute, mastoiditis were found to have cholesteatomas. Venous sinus thrombosis developed in four children and intracerebellar abscess in one child. Contrast-e…

Acute mastoiditis after a combined approach tympanoplasty operation.

Four of 112 ears on which a combined approach tympanoplasty (CAT) operation was performed had acute mastoiditis six to 24 months after operation. All four patients were younger than 13 years. This represents 12.5% of acute mastoiditis cases after CAT operations in this age group, or 3.5% of the total group. On reoperation, all four ears expressed pus under pressure in the mastoid cavity. There was also fibrosis and granulation tissue blocking the attic inlet. It is probable that the cause of the acute mastoiditis may be related to this partial or total stenosis. Two of the patients were found …

A dilated mastoid emissary vein as a potential cause of pulsatile tinnitus.

BACKGROUND: Pulsatile tinnitus (PT) is commonly attributed to vascular etiologies; however, up to half of cases remain idiopathic. OBJECTIVES: To investigate the association between the mastoid emissary vein (MEV) and PT and to identify MEV-related factors contributing to PT. MATERIALS AND METHODS: This retrospective study included 119 adults (237 ears; PT, 46 ears; non-PT, 191 ears) who underwent temporal bone computed tomography between January 2019 and July 2025. MEV presence, dilation, and morphological features were analyzed. RESULTS: MEV was identified in 73.9% of PT ears and 61.3% of…

Mastoid Signal Change and Acute Cerebral Venous Sinus Thrombosis: Unlocking the Puzzle.

Mastoid signal changes on magnetic resonance imaging in cerebral venous sinus thrombosis (CVT) patients are often misinterpreted as septic CVT. Studies have revealed mastoid signal changes in CVT patients. We conducted this study to investigate this relationship. Single-center, retrospective, observational study. Patients aged ≥18 years with venography-proven CVT were enrolled, excluding those with sepsis-induced CVT. Mastoid changes and clot burden score (CBS) were calculated. We screened and enrolled 128 and 124 CVT patients, respectively. Seventy per cent of patients had mastoid signal chan…

Implant-Retained Auricular Prosthesis: Is Mastoid Cell Perforation Associated With Implant Survival?

BACKGROUND: Implant-retained auricular prostheses are a well-established reconstructive option for patients with facial defects resulting from congenital anomalies, trauma, or oncologic resections. Most temporal implants are 3.0 to 4.0 mm in length. Longer implants could result in improved implant survival, but risk mastoid cell perforation. PURPOSE: The study purpose was to measure the association of implant mastoid cell perforation and implant survival. STUDY DESIGN, SETTING, SAMPLE: A retrospective cohort study was conducted, including all patients treated with temporal endosseous implant…

Management of Cholesteatoma: Surgical Options for Disease Limited to Middle Ear/Mastoid.

Surgery remains the mainstay of cholesteatoma management. Through advancement in technique and technology, the available surgical approaches have expanded to include not only the traditional procedures, but also endoscopic procedures, canal wall reconstruction procedures, mastoid obliteration, and retrograde mastoidotomy. Selection of management technique will depend on disease characteristics, patient factors, and surgeon preference.
