Tentorial incision in a lateral-medial direction with minimal retraction of the temporal lobe in the subtemporal transtentorial approach to the middle tentorial incisural space.

OBJECTIVE: The subtemporal transtentorial approach provides excellent exposure of the middle incisural space. A modification of the subtemporal transtentorial approach with use of a partial mastoidectomy is presented to avoid damage to the temporal lobe as a result of retraction as well as damage to venous structures. METHODS: Four patients, one with a superior cerebellar artery aneurysm, one with a metastatic tumor in the midbrain, one with a tentorial meningioma, and one with a tentorial schwannoma were treated with the present approach. After subtemporal craniotomy, all of the cortical bon…

Retraction pockets in chronic suppurative otitis media- our experience.

Retractions of the tympanic membrane constitute a large proportion of ear diseases causing concern to the otologist. The clinical features i … istent feature in most of our cases. This was subjectively assessed as a surgical finding in tympanomastoidectomy and objectively assessed by a high Resolution Computed Tomography of the temporal bone. We have found that a canal wall down mastoidectomy was the most effective in preventing the recurrence of retraction pockets. In a coutry like India, canal wall …

Critical Steps and Common Mistakes during Temporal Bone Dissection: A Survey among Residents and a Step-by-Step Guide Analysis.

BACKGROUND: Given that the temporal bone is one of the most complex regions of the human body, cadaveric dissection of this anatomical area represents the first necessary step for the learning and training of the young oto-surgeon in order to perform middle ear surgery, which includes the management of inflammatory pathology, hearing rehabilitation, and also cognitive decline prevention surgery. The primary objective of this study was to identify common mistakes and critical passages during the initial steps of temporal bone dissection, specifically cortical mastoidectomy and posterior tympano…

Acute bacterial meningitis caused by acute otitis media in adults: A series of 12 patients.

We conducted a retrospective chart review to characterize the outcomes of 12 patients-9 men and 3 women, aged 21 to 79 years (mean: 49)-who … 2 underwent surgery at a later stage because of a suspected defect in the mastoid bone. A cortical mastoidectomy was performed in 6 of the 7 surgical patients; the remaining patient underwent a canal-wall-down procedure. Ten patients experienced a full recovery, 1 died, and 1 had a poor neurologic outcome (vegetative state); both of the latter 2 patients were …

Ossiculoplasty in chronic otitis media: Surgical results and prognostic factors of surgical success.

BACKGROUND AND OBJECTIVES: The goal of ossiculoplasty is to improve hearing. Successful ossiculoplasty depends on several factors. This retr … uloplasties were carried out in one-stage surgery. In 38% of cases ossiculoplasty was combined with mastoidectomy; in the remaining 62% of cases, ossiculoplasty was performed without mastoidectomy. Ossiculoplasty was successfully achieved in 113 patients (74%). The presence of the stapes superstructure and normal mucosa were significant predictive factors of su …

Lateral Attic Wall Reconstruction with Glass Ionomer Bone Cement in the Management of Primary Acquired Attic Cholesteatoma in Children: A Preliminary Experience.

OBJECTIVE: To assess the effectiveness of glass ionomer bone cement (GIBC) in lateral attic wall reconstruction after primary acquired attic cholesteatoma surgery. MATERIALS AND METHODS: This prospective study was conducted on twenty children collected from the ENT outpatient clinics of a secondary and tertiary hospital. All patients presented with chronic suppurative otitis media with cholesteatoma of the primary acquired attic type. All patients underwent intact canal wall mastoidectomy (ICWM) with a transcanal atticotomy to address primary cholesteatoma involving the attic and the supratub…

[Lateral Sinus Thrombosis--A Rare Complication of an Acute Mastoiditis or Infected Cholesteatoma].

BACKGROUND: Lateral sinus thrombosis (LST) is a rare but threatening complication of an acute mastoiditis or infected cholesteatoma. Currently only very few papers are available in the literature dealing with the systematic investigation of patients with suspected LST. The purpose of the present study was to evaluate the clinical, intraoperative and therapeutic findings of patients with particular disease. PATIENTS AND METHODS: For this retrospective study the clinical records of 7 patients which were admitted for a suspected LST were evaluated. All patients underwent mastoidectomy with expos…

OBJECTIVE: This article reports the development of low-set ear following tympanomastoidectomy via endaural and/or postaural approaches. DESIGN: Description of 10 cases who had developed low-set ear following mastoid surgery undertaken to clear chronic suppurative otitis media. The surgical approach was via endaural or postaural incisions. SETTING: Tertiary care referral hospital. METHOD: The operative history and development of low-set ear complication are described. Presentation, treatment, and clinical course of otogenic lateral sinus thrombosis.

CONCLUSIONS: Otogenic lateral sinus thrombosis (OLST) is almost always associated with other complications of chronic otitis media (COM) and did not present a distinct clinical expression. In our experience, OLST exhibits a benign course if the underlying disease is controlled. Mastoidectomy in combination with broad-spectrum antibiotics provided effective treatment. Anticoagulation therapy is no longer used routinely. Recanalization is rare and is independent of the use of anticoagulants. OBJECTIVES: This study aimed to contribute to the understanding of the clinical aspects and evolution of…

Scedosporium apiospermum otomycosis.

We present a case of Scedosporium apiospermum otomycosis in an immunocompetent 62-year-old woman presumably inoculated as a child in Vietnam. She had life-long recurrent symptoms of chronic otomastoiditis and otorrhea. The organism was identified in an unexpected wound infection after a modified radical mastoidectomy done for chronic otitis media. Treatment consisted of local debridement of necrotic tissue, topical wound care, and long-term systemic antifungals.

Acute mastoiditis complicated with bezold abscess, sigmoid sinus thrombosis and occipital osteomyelitis in a child.

Acute otitis media complications are relative rare since the introduction of antibiotics. Still many controversies exist on the diagnosis and treatment of some of them. In this report we describe a pediatric case of upper cervical abscess (bezold abscess), sigmoid sinus thrombosis and occipital osteomyelitis following mastoiditis, presenting with persistent fever and otorrhea. The patient responded well to intravenous antibiotics and wall-intact mastoidectomy. We discuss methods of early diagnosis and appropriate treatment options.
