Risk of Perinatal and Maternal Morbidity and Mortality Among Pregnant Women With Epilepsy.

IMPORTANCE: Maternal epilepsy is associated with adverse pregnancy and neonatal outcomes. A better understanding of this condition and the associated risk of mortality and morbidity at the time of delivery could help reduce adverse outcomes. OBJECTIVE: To determine the risk of severe maternal and perinatal morbidity and mortality among women with epilepsy. DESIGN, SETTING, PARTICIPANTS: This prospective population-based register study in Denmark, Finland, Iceland, Norway, and Sweden took place between January 1, 1996, and December 31, 2017. Data analysis was performed from August 2022 to Nov

Maternal mortality and predictors of adverse outcome in patients with heart disease in pregnancy.

INTRODUCTION: Heart disease in pregnancy possesses a great haemodynamic challenge and is a known risk for increased maternal morbidity and m … of NYHA class, PAH, and LVEF, and the feto-maternal outcome was recorded and evaluated in terms of maternal mortality, fetal demise, the occurrence of major cardiac complication, and risk of preterm delivery. RESULTS: A total of three out of 29 (10.34%) maternal deaths were attributed to a cardiac cause. 5.45% of patients with heart disease had maternal mortality, …

The Role of Maternal Intimate Partner Violence Victimization on Neonatal Mortality in Ethiopia.

BACKGROUND: Previous studies have suggested that when intimate partner violence (IPV) tends to be persistent across a woman's life span, her newborn offspring have a higher risk of ill health and mortality. There is a high prevalence of both IPV and neonatal mortality in Ethiopia, but the issue of IPV has remained largely outside the focus of child survival programs in this country. One of the noticeable reasons is a lack of evidence regarding the effect of IPV on neonatal mortality. Therefore, this study investigated the effect of maternal IPV on neonatal mortality in Ethiopia. METHOD: This

A review of the preventability of maternal mortality in one hospital system in Louisiana, USA.

OBJECTIVE: To determine preventability of in-hospital maternal mortality in the Ochsner Health System (OHS) in the US state of Louisiana. METHODS: A retrospective study was undertaken of all known cases of in-hospital maternal death (during pregnancy or within 42 days of termination) that occurred within OHS facilities in 1995-2013. Associations between characteristics and mortality and preventability were investigated. Incidence rate ratios (IRRs) were calculated in view of varying reference values. RESULTS: Among 16 eligible deaths, 12 (75%) were deemed potentially preventable. The inciden…

Biocultural perspectives on maternal mortality and obstetrical death from the past to the present.

Global efforts to improve maternal health are the fifth focus goal of the Millennium Development Goals adopted by the international community in 2000. While maternal mortality is an epidemic, and the death of a woman in childbirth is tragic, certain assumptions that frame the risk of death for reproductive aged women continue to hinge on the anthropological theory of the "obstetric dilemma." According to this theory, a cost of hominin selection to bipedalism is the reduction of the pelvic girdle; in tension with increasing encephalization, this reduction results in cephalopelvic disproportion,…

Medical disease as a cause of maternal mortality: the pre-imminence of cardiovascular pathology.

Maternal mortality ratio in low- to middle-income countries (LMIC) is 14 times higher than in high-income countries. This is partially due to lack of antenatal care, unmet needs for family planning and education, as well as low rates of birth managed by skilled attendants. While direct causes of maternal death such as complications of hypertension, obstetric haemorrhage and sepsis remain the largest cause of maternal death in LMICs, cardiovascular disease emerges as an important contributor to maternal mortality in both developing countries and the developed world, hampering the achievement of…

Causes of maternal mortality in Ethiopia: a significant decline in abortion related death.

BACKGROUND: Although the common direct obstetric causes of maternal mortality are known from the literature, the contribution of each cause and the change in trend over decades is unknown in Ethiopia. The objective of this review was to assess the trend of proportion of maternal mortality due to the common direct causes. METHODS: This systematic review was done on eighteen health facility based maternal mortality studies conducted between 1980 and 2012 in Ethiopia. Emphasis was given to the proportion of maternal mortality due to direct causes and their case fatality rates. RESULTS: The summ…

Association of maternal height with child mortality, anthropometric failure, and anemia in India.

CONTEXT: Prior research on the determinants of child health has focused on contemporaneous risk factors such as maternal behaviors, dietary factors, and immediate environmental conditions. Research on intergenerational factors that might also predispose a child to increased health adversity remains limited. OBJECTIVE: To examine the association between maternal height and child mortality, anthropometric failure, and anemia. DESIGN, SETTING, AND POPULATION: We retrieved data from the 2005-2006 National Family Health Survey in India (released in 2008). The study population constitutes a nation

Racial disparity in infant and maternal mortality: confluence of infection, and microvascular dysfunction.

OBJECTIVES: Racial disparities in infant and maternal mortality have been attributed to the unique stresses faced by Black women in the United States, but the underlying pathophysiological pathways are poorly understood. This paper reviews the literature related to potential causes of racial disparities in infant and maternal mortality. METHODS: A review of the literature from 1966 to 2003 was conducted using a series of searches of Medline, obstetrical journals, and bibliographies. The review focused on potential contributing pathophysiological factors to infant and maternal mortality. RESU…

Child and maternal mortality during a period of conflict in Beira City, Mozambique.

BACKGROUND: Child mortality rates have been declining in most developing countries. We studied child and maternal mortality risk factors for child mortality in Beira city in July 1993, after a decade of conflict in Mozambique. METHODS: A community-based cluster sample survey of 4609 women of childbearing age was conducted. Indirect techniques were used to estimate child mortality ('children ever born' method and Preceding Birth Techniques (PBT) and maternal mortality (sisterhood method). Deaths among the most recent born child, born since July 1990, were classified as cases (n = 106), and two…

The effect of stay in a maternity waiting home on perinatal mortality in rural Zimbabwe.

A hospital-based cohort study was carried out in a district hospital in Zimbabwe to evaluate the effect of a maternity waiting home on perinatal mortality. Information on antenatal risk factors, use of antenatal care, access to the hospital and stage of labour on arrival was collected for each woman delivering at the hospital during the period 1989-1991 (n = 6438). Women who stayed in the maternity waiting home had a lower risk of perinatal death compared to women who came directly from home to the hospital during labour. The crude relative risk of perinatal death for the women coming from hom

Family planning issues relating to maternal and infant mortality in the United States.

Both maternal and infant death rates in the United States are much higher than in many developed countries. The interrelationships between abortions and maternal and infant mortality have been analyzed on the basis of data from the 1970s and 1980s. The legalization of abortions in 1973 resulted in a marked increase in legal abortions and marked reductions in maternal and infant mortality over the course of the 1970s. However, a wide variation in abortion rates and in the number of abortion facilities indicates that such facilities were not readily available to all segments of the population in

Effect on mortality of community-based maternity-care programme in rural Bangladesh.

Various community-based interventions have been proposed to improve maternity care, but hardly any studies have reported the effect of these measures on maternal mortality. In this study, the efficacy of a maternity-care programme to reduce maternal mortality has been evaluated in the context of a primary health-care project in rural Bangladesh. Trained midwives were posted in villages, and asked to attend as many home-deliveries as possible, detect and manage obstetric complications at onset, and accompany patients requiring referral for higher-level care to the project central maternity clin…
