Uses of maxillary sinus lateral wall bony window in an open window sinus lift procedure: literature review.

AIM: To review all of the possible uses for maxillary sinus lateral wall bony window in an open maxillary sinus lift procedure and to evaluate the influence of each method to the rate of sinus membrane perforations. METHODS: A systematic literature review was performed of randomized control studies in English identified in MEDLINE (PubMed) and Cochrane online databases, published between 2007.09.01 and 2017.09.01. Surgeries had to be performed in vivo, for patients over 18 years old. A study had to have at least 10 sinus lifting procedures, had to detail how the bony window was used and had t

Maxillomandibular advancement is a successful treatment for obstructive sleep apnoea: a systematic review and meta-analysis.

The primary objective of this review was to establish the effectiveness of maxillomandibular advancement (MMA) as a successful treatment modality in improving airway patency in patients with obstructive sleep apnoea (OSA). A systematic and detailed search was performed using PubMed Central, covering the period January 2000 to December 2015, with well-defined selection criteria. The authors independently conducted the study selection, data extraction, and assessed the risk of bias of the included studies. Twenty studies met the inclusion criteria. The outcome measures studied were the apnoea-hy

Long-term use of Le Fort I osteotomy for the management of nasopharyngeal rhinosporidiosis: A case series.

Rhinosporidiosis is a rare, chronic, granulomatous infection of the mucous membranes that mainly involves the nose and nasopharynx; it occasionally involves the pharynx, conjunctiva, larynx, trachea and, rarely, the skin. The characteristic clinical features of this disease include the formation of painless polyps in the nasal mucosa or the nasopharynx that bleed easily on touch. At our center, excision of the lesion with a Le Fort I osteotomy is carried out in patients (1) in whom two or more previous attempts at excision of biopsy-proven rhinosporidiosis arising from the nasal mucosa was car

Cephalometric pharyngeal changes after Le Fort I osteotomy in patients with unilateral cleft lip and palate.

Pharyngeal changes after Le Fort I osteotomy were evaluated cephalometrically in 37 patients (27 M, 10 F; mean age 23.8 years) with unilateral cleft lip and palate (UCLP). Seven patients had previously undergone velopharyngeal (VPP) flap surgery to improve speech. One year postoperatively the patients without previous VPP showed a significant change (55%-85% of the surgical change) in the upper and lower sagittal depth of the nasopharyngeal airway, but not in the depth of oropharyngeal airway, length of soft palate, or position of hyoid bone. No significant changes were observed between 6 mont

Staged reconstruction of panfacial trauma involving a comminuted mandibular fracture: a case report.

Comminuted mandibular fractures (CMFs) associated with panfacial trauma are challenging because restoration of skeletal continuity does not necessarily re-establish functional occlusion or facial form. We report a staged multidisciplinary rehabilitation in a 34-year-old man who sustained severe craniofacial injuries in an industrial explosion, including a CMF, multiple midfacial fractures, and extensive anterior maxillary destruction. Initial treatment consisted of hemorrhage control, top-down open reduction and internal fixation, and simultaneous orbital floor reconstruction. Because severe m

Vascular complications related to Le Fort I osteotomy: a scoping review.

BACKGROUND: Le Fort I (LFI) osteotomy represents a secure procedure employed for correcting facial deformities and may be coupled with other facial osteotomies. However, notwithstanding professional proficiency, this technique is not exempt from issues or morbidities. Therefore, the objective of this study was to relate the types of vascular complications associated with LFI. MATERIAL AND METHODS: A scoping review was conducted with published articles up to April 2024 sourced from PubMed, LILACS, EMBASE, Scopus, Web of Science, Cochrane Library and Google Scholar. The analyzed data comprised:

Consequences and management of nasal airway obstruction in the dentofacial deformity patient.

INTRODUCTION: At the time of orthognathic surgery the maxillofacial surgeon has an opportunity to further contribute to a patient's quality of life by simultaneously addressing long-standing breathing and sinus drainage difficulties that may coexist with the jaw deformity. Breathing difficulties may range from isolated symptomatic nasal airway obstruction to obstructive sleep apnea syndrome and should not be overlooked. The impact of the combined occurrence of nasal obstruction and maxillary deformity has been recognized and discussed in the literature. RECENT FINDINGS: Studies have establish

Treatment of Severe Open Bite Malocclusion with Four-Piece Segmental Horseshoe Le Fort I Osteotomy.

Appropriate operations in severe anterior open bite (AOB) cases are extremely complicated to perform because of the multiple surgical procedures involved, the difficulty of predicting posttreatment aesthetics, and the high relapse rate. We herein report a 16-year-old girl with skeletal Class II, severe AOB malocclusion, and crowding with short roots, and aesthetic and functional problems. Four-piece segmental Le Fort I osteotomy with horseshoe osteotomy was performed for maxillary intrusion, and bilateral sagittal split ramus osteotomy (SSRO) and genioplasty were performed for mandibular advan

A Simple Method to Retract the Descending Palatine Neurovascular Bundle During Osseous Removal of Posterior Interferences in Le Fort I Osteotomy.

For accurate repositioning of the maxilla in Le Fort I osteotomy, bone removal around the descending palatine neurovascular bundle and maxillary tuberosity is frequently required. Such task can be challenging due to the high vascularity of the region, which is the most common site of hemorrhage during removal of bony interferences. Although a rare occurrence, ligation of the descending palatine artery in cases of intraoperative vascular damage can be related to the development of aseptic necrosis. This article reports a simple technique for retraction and protection of the descending palatine

Zygomatic osteotomy for improving aesthetic results in orthognathic surgery: a report of two cases.

Patients with anteroposterior maxillary deficiency and mandibular prognathism associated with a long-face pattern exhibit reduced anterior and lateral projection of the zygomaticomaxillary complex. Several techniques have been combined with orthognathic surgery to enhance the volume and definition of this region, such as zygomatic osteotomy. Two female patients, aged 22 (Case A) and 29 (Case B), with class II (Case A) and III (Case B) malocclusion, respectively, complained about facial aesthetics due to a long face and lack of malar projection. Le Fort I osteotomy to advance the maxilla, bilat

Automated condylar seating assessment using a deep learning-based three-step approach.

OBJECTIVES: In orthognatic surgery, one of the primary determinants for reliable three-dimensional virtual surgery planning (3D VSP) and an accurate transfer of 3D VSP to the patient in the operation room is the condylar seating. Incorrectly seated condyles would primarily affect the accuracy of maxillary-first bimaxillary osteotomies as the maxillary repositioning is dependent on the positioning of the mandible in the cone-beam computed tomography (CBCT) scan. This study aimed to develop and validate a novel tool by utilizing a deep learning algorithm that automatically evaluates the condylar

Skeletal deformities and surgical procedures in orthognathic surgery patients: a 10-Year retrospective analysis of 1095 cases.

BACKGROUND: Orthognathic surgery is a well-established method for correcting skeletal dentofacial deformities. Despite its widespread application, large-scale data describing deformity prevalence and the distribution of surgical approaches remain limited. This study aimed to determine the prevalence and patterns of skeletal deformities and orthognathic surgical procedures in patients who underwent orthognathic surgery. METHODS: In this retrospective study, the radiographic records of 1095 patients (663 females, 432 males; aged 16-65 years; mean age: 23.07 ± 5.6 years) treated between 2014 and

Piezo-surgical pterygoid plate horizontal osteotomy (PS-PPHO): a novel alternative to the classic vertical pterygomaxillary split in LeFort I osteotomy - clinical experience in a tertiary center.

BACKGROUND: The pterygomaxillary disjunction remains one of the most technically demanding and risk-prone steps in Le Fort I osteotomy, with potential for uncontrolled pterygoid fractures and severe hemorrhage. To enhance safety and control, we introduced the piezo-surgical pterygoid plate horizontal osteotomy (PS-PPHO), a novel alternative to the conventional blind vertical pterygo-maxillary split. METHODS: A retrospective observational cohort study was conducted at a tertiary referral center between 2008 and 2020, including 243 consecutive patients who underwent Le Fort I osteotomy or surgi

[Surgical-associated expansion of the jaws with asymmetric deformations].

One of the most common abnormalities in the development of the jaw is the narrowing of the alveolar process of the maxilla. In childhood, the elimination of such a deformation is quite common. But with age, the only effective option is the inclusion of a surgical procedure in the rehabilitation plan. The medial and parasagittal osteotomy of the maxilla for the purpose of its expansion by a distraction device has a number of important nuances which can significantly affect the result. These include: localization of sagittal osteotomy of the palate and alveolar process; the direction of cutting
