Initiation of adjuvant hormone therapy by Medicaid insured women with nonmetastatic breast cancer.

Hormone therapy is the mainstay of adjuvant treatment for hormone receptor positive (HR-positive) nonmetastatic breast cancer. We evaluated adjuvant hormone therapy (AHT) initiation among Medicaid-insured women aged 21-64 years with stage I-III HR-positive breast cancer. We used multivariable logistic regression to identify independent predictors of AHT initiation. Within 1 year of diagnosis, 68% (1049/1538) initiated AHT; by 18 months, 80% (1168/1461) initiated AHT. In multivariable analysis, women less likely to initiate AHT had more comorbidity (≥ 2 vs none: adjusted odds ratio (AOR) = 0.55…

Increased olanzapine discontinuation and health care resource utilization following a Medicaid policy change.

OBJECTIVE: To assess the short-term impact of Florida Medicaid's policy change on olanzapine discontinuation and health care resource utilization among olanzapine-treated patients with schizophrenia or bipolar diagnoses. The announced policy change, effective on July 11, 2005, but rescinded on September 9, 2005, reclassified olanzapine as nonpreferred and gave physicians 60 days to change antipsychotics for current users. METHOD: Prescription patterns, health care resource utilization, and Medicaid payments were compared between patients using olanzapine on July 11, 2005, and matched prior-ye…

Retrospective assessment of Medicaid step-therapy prior authorization policy for atypical antipsychotic medications.

BACKGROUND: Antipsychotic medications account for more prescription expenditures in Medicaid than any other therapeutic category. This has made them an attractive target for states hoping to curtail rising expenditures. OBJECTIVE: The objective of this study was to document the effects of a step-therapy prior authorization (PA) policy for atypical antipsychotic medications on: (1) Medicaid prescription expenditures among all Medicaid beneficiaries and (2) prescription and health service expenditures among patients with schizophrenia. METHODS: Prescription, inpatient, outpatient, and long-ter…

Prevalence of Co-occurring Mental, Neurodevelopmental and Neurological Conditions in Medicaid Beneficiaries With Autism.

PURPOSE: As more children with autism enter adulthood, updated data on co-occurring conditions throughout the lifespan are needed. We examined mental, neurodevelopmental and neurological (MNN) conditions among people with and without autism overall and by demographics. METHODS: Using Medicaid claims data for beneficiaries aged ≥ 1 year enrolled during 2020, we identified autism and MNN diagnoses using ICD-10 codes. Adjusted prevalence ratios (aPRs) in beneficiaries with versus without autism were calculated using log-binomial models. RESULTS: Among 993,965 beneficiaries with autism, attentio…

Doulas as Facilitators to Black Birthing People's Needs in Pennsylvania in the Context of Medicaid Coverage.

INTRODUCTION: Longstanding racial inequities in maternal mortality require tailored approaches to improving outcomes for Black birthing people. Doulas provide continuous emotional, physical, and informational support and have been shown to improve perinatal outcomes, particularly for Black birthing people. In response, some states have implemented Medicaid doula benefits. In January 2025, Pennsylvania-where ∼35% of births are Medicaid-covered-launched such a policy, allowing certified doulas to enroll as Medicaid providers. METHODS: As part of a larger mixed-methods study on the effects of Me…

Foster Care Status and the Timing of Diagnosis of Fetal Alcohol Syndrome in a Medicaid Sample.

OBJECTIVES: Fetal alcohol syndrome (FAS) is frequently undiagnosed or misdiagnosed, particularly among populations at elevated risk of the condition. We examined the prevalence of FAS among children aged 0 to 5 years enrolled in Medicaid, described characteristics of affected children, and evaluated diagnostic timing between children in foster care and children not in foster care. METHODS: We conducted a retrospective analysis of Medicaid Transformed Analytic Files for children born from 2015 through 2017 with FAS diagnoses (n = 771), following each birth cohort for 5 years. We used descripti…

Beyond insurance coverage: the impact of Medicaid expansion on colorectal cancer surgery in the Midwest.

PURPOSE: The Affordable Care Act's Medicaid expansion increased insurance coverage nationally; its impact on colorectal cancer surgery access and outcomes remains unclear, particularly in rural regions with structural barriers. This study evaluated whether Medicaid Expansion was associated with changes in colorectal cancer surgery utilization, access to elective surgery, and short-term outcomes within 8 Midwestern states. METHODS: A population-based difference-in-differences analysis used State Inpatient Databases from 2010 to 2022, including 4 expansion and 4 late or nonexpansion states. Adu…

Development and validation of an algorithm to predict stillbirth gestational age in Medicaid billing records.

With Medicaid covering half of US pregnancies, Medicaid Analytic eXtract (MAX) provides a valuable data source to enrich understanding about stillbirth etiologies. We developed and validated a claims-based algorithm to predict gestational age (GA) at stillbirth. We linked the stillbirths identified in MAX 1999-2013 to Florida fetal death records (FDRs) to obtain clinical estimates of GA (n = 825). We tested several algorithms, including using a fixed median GA, median GA at the time of specific prenatal screening tests, and expanded versions considering additional predictors of stillbirth, inc…

The Influence of Structural Factors and Telemedicine on Missed Appointments Among Medicaid-Insured Patients.

BACKGROUND: Missed clinic appointments disproportionately affect Medicaid-insured patients and residents of socioeconomically deprived neighborhoods. The role of the recent telemedicine expansion in reducing these disparities is unclear. We analyzed the relationship between census tract (CT) poverty level, residential segregation, missed appointments, and the role of telemedicine. METHODS: This retrospective cohort study used electronic health records (EHR) data merged with population datasets and restricted to adult patients (≥ 18 years) with completed or missed internal or family medicine o…

Disparities in State Medicaid Coverage of Tretinoin for Pigmentary Disorders Compared to Acne Vulgaris.

BACKGROUND: Melasma and post-inflammatory hyperpigmentation (PIH) are common cosmetic dermatologic conditions that predominantly affect patients with skin phototypes III-VI. Comparing treatment coverage for these pigmentary disorders to treatment coverage for acne vulgaris may demonstrate disparities in insurance coverage for diseases that primarily affect patients of color. OBJECTIVE: Describe differences in Medicaid coverage for topical tretinoin for melasma and PIH vs. acne vulgaris in all 50 states and the District of Columbia. METHODS: This is a cross-sectional study of Medicaid insuran…
