Dry eye syndrome.

Our understanding of keratoconjunctivitis sicca (KCS), also known as dry eye syndrome, has been changed over recent years. Until lately, the condition was thought to be merely due to aqueous tear insufficiency. Today, it is understood that KCS is a multifactorial disorder due to inflammation of the ocular surface and lacrimal gland, neurotrophic deficiency and meibomian gland dysfunction. This change in paradigm has led to the development of new and more effective medications.

Lotilaner ophthalmic solution 0.25% in the treatment of Demodex blepharitis: A case report.

Demodex blepharitis is a chronic inflammatory ocular condition caused by Demodex mite infestation of the eyelid that can negatively impact quality of life. Currently, lotilaner ophthalmic solution 0.25% is the only FDA-approved treatment for Demodex blepharitis. The Demodex Expert Panel on Treatment and Eyelid Health has established consensus that lotilaner ophthalmic solution 0.25% should be considered the preferred first-line treatment for Demodex blepharitis. We report a patient who presented with collarettes, the pathognomonic sign of Demodex blepharitis, meibomian gland dysfunction, and p…

Mucosal immunology of the ocular surface.

The eye is a sensory organ exposed to the environment and protected by a mucosal tissue barrier. While it shares a number of features with other mucosal tissues, the ocular mucosal system, composed of the conjunctiva, Meibomian glands, and lacrimal glands, is specialized to address the unique needs of (a) lubrication and (b) host defense of the ocular surface. Not surprisingly, most challenges, physical and immunological, to the homeostasis of the eye fall into those two categories. Dry eye, a dysfunction of the lacrimal glands and/or Meibomian glands, which can both cause, or arise from, sens

Applied anatomy and morphology of Meibomian glands in the non-human primate.

Dry eye disease (DED) is a common ocular condition that has been estimated to affect ~ 10 to 55.4% of the global population. Symptoms of DED include eye irritation, ocular pain and discomfort, inflammation, and photophobia, and, if left untreated, can lead to infection, corneal neuropathy, corneal scarring and impaired vision. Studies have shown that over 70% of all DED cases are caused by some form of Meibomian gland dysfunction (MGD). To date the etiology of MGD remains unknown, therefore, there is a need for further research into understanding the development, homeostasis and pathology of t…

Analysis of meibum and tear lipids.

The meibum is a lipid-rich secretion that is the primary component of the external layer of the tear film. The meibomian glands produce the meibum, and meibomian gland dysfunction can lead to degradation of the tear film. Such dysfunction can result in ocular irritation, inflammation, and clinical disease. Understanding this relationship is critical to preventing ocular disease; therefore, a search of peer-reviewed literature focusing on the collection, quantification, and analysis of normal and abnormal meibum and tear lipids was conducted. Numerous collection and quantification techniques ar…

Can tetracycline antibiotics duplicate the ability of azithromycin to stimulate human meibomian gland epithelial cell differentiation?

PURPOSE: Azithromycin and tetracyclines are commonly prescribed in the United States for the treatment of meibomian gland dysfunction (MGD). The efficacy of these antibiotics has been believed to be their antiinflammatory and antibacterial actions, which suppress MGD-associated posterior blepharitis and growth of lid bacteria. However, we recently discovered that azithromycin can act directly on human meibomian gland epithelial cells (HMGECs) to stimulate their function. In this study, we sought to determine whether tetracycline antibiotics can duplicate this azithromycin effect. METHODS: Imm…

Total meibomian gland loss in Parkinson's disease.

Parkinson's disease is characterized by motor dysfunction, affecting blink frequency in the eyes. Reduced blinking and use of anticholinergic medications result in dry eye disease (DED) in these patients. This unique case had complete meibomian gland loss seen on meibography with reduced blinking and lacrimal gland function as well attributed to Parkinson's and its treatment. The autoimmune workup was negative, and the final diagnosis of drug-induced DED was made. The patient was managed with ocular lubricants and anti-inflammatory eye drops.
