A study was performed to assess the incidence of previous hysterectomy and dilatation and curettage among women with primary biliary cirrhosis. In 87 patients with primary biliary cirrhosis hysterectomy or dilatation and curettage had been performed significantly more often than among 100 age matched normal controls and 80 age matched patients with chronic active hepatitis or alcoholic liver disease. Forty-eight hirsute women were treated with spironolactone 100 mg twice daily for 3 to 12 months. Both facial and body hirsuties improved by 30-40%, and there was a threefold reduction in frequency of local treatments such as waxing or shaving. Plasma testosterone fell by 30%, though the improvement in hirsuties grading did not correlate with the fall in plasma testosterone. Six subjects discontinued treatment because of lack of effect, and 4 because of menstrual disturbance. Spironolactone was equally effective in the treatment of idiopathic hirsutism and of the polycystic ovary syndrome.

In 20 anovulatory patients who were normoprolactinaemic, 12 developed transient hyperprolactinaemia when they were treated with human menopausal gonadotropin (hMG) for induction of ovulation. The hyperprolactinaemia was probably due to the increased oestrogen production effect on some susceptible patients. The pregnancy rate was found to be lower in those who developed this condition. The dosage of hMG required was found to be significantly higher in this group. The importance of recognizing this transient hyperprolactinaemia and the probably role of Bromocriptine are discussed. Clinical data were compared with the results of study of fractional composition of estrogens excreted with the urine at various phases of the menstrual cycle.
