Volar dislocation of the index carpometacarpal joint in association with a Bennett's fracture of the thumb: a rare injury pattern.

We describe a case of volar dislocation of the index carpometacarpal (CMC) joint in association with a Bennett's fracture of the thumb following a motorcycle accident. Volar dislocation of the index carpometacarpal joint is an exceedingly rare but easily missed injury, with only a few reported cases in the literature. This report highlights the importance of a true lateral radiograph and close scrutiny of the film to detect this injury. Closed reduction supplemented with Kirschner wire fixation restored normal anatomical relations and achieved an excellent clinical result.

Vascularized bone grafting from the base of the second metacarpal for persistent distal radius nonunion: a case report.

Although fractures of the distal radius are relatively common, nonunion is a rare complication. We report a case of a distal radius nonunion in a healthy 51-year-old woman who was involved in a motor vehicle accident. Initial attempts to treat the nonunion with open reduction and internal fixation and corticocancellous bone grafting from the iliac crest failed. Preservation of wrist motion was of considerable importance and wrist arthrodesis was deferred. Treatment of the nonunion with a vascularized bone graft from the base of the second metacarpal, combined with biplanar stable fixation and

Sensate first dorsal metacarpal artery flap for resurfacing extensive pulp defects of the thumb.

Finding an appropriate soft-tissue grafting material to close a wound located over the distal phalanx of the thumb, especially the pulp region, can be a difficult task. A sensate first dorsal metacarpal artery flap, mobilized from the dorsum of the adjacent index finger and used as an island pedicle skin flap, can be useful for this purpose. The pedicle includes the ulnar branch of the first dorsal metacarpal artery, the dorsal veins, and the cutaneous branch of the radial nerve. Although this tiny artery is anatomically variable, safe dissection can be achieved by including the radial shaft p

The Catel-Manzke syndrome in a female infant.

Catel-Manzke syndrome in a female infant: We describe a girl with typical features of the Catel-Manzke syndrome. Although most cases described are boys, X-linked inheritance is not very likely now at least four females with this syndrome have been described.

Osteoporosis in Colles fracture.

Previous studies of bone density in patients with Colles fracture have been inconclusive. We measured bone density at the second metacarpal in 36 postmenopausal women with Colles fracture. The patients were found to have highly significant (P < 0.01) or significant (P < 0.05) differences in the parameters of bone density compared with controls matched for age. The results suggest that osteoporosis is of definite relevance to the etiology of Colles fracture in postmenopausal women.

Rolando's fracture treated by closed reduction and external fixation.

In 11 patients with a Rolando's fracture of the first metacarpal, the mini-Hoffman external fixation was used to maintain closed reduction of the fracture, with two pins placed distally in the first metacarpal and one pin proximally in the trapezium. The results were excellent in 7 patients; in 3 patients mild arthritis was noted radiographically, and one patient had a poor result because of severe postraumatic arthritis 1-5 years (mean 30 months) after the injury. This method of treatment is proposed for patients with Rolando's fracture as it is very easily performed and provides good results

Open reduction and internal fixation of combined fourth and fifth carpometacarpal (fracture) dislocations.

BACKGROUND: Traditionally, combined fourth and fifth carpometacarpal fracture dislocations are treated conservatively or by means of Kirschner wire after closed reduction. Since 1983, unstable dislocations have been treated with open reduction and screw fixation or with a temporary plate that bridges the fourth carpometacarpal (CMC) joint to maintain anatomical reduction. METHODS: In a retrospective study, we evaluated the results of this surgical approach in a group of 11 patients and another group of 4 conservatively treated patients. RESULTS: Eleven patients were treated by means of open

The management of common dislocations in the upper limb.

Dislocations are among the many orthopaedic injuries that the general practitioner is expected to manage. Many are fairly straightforward to treat, however, there are also many pitfalls. In this paper we provide an overview of the common dislocations in the upper limb, and some that are less common but important to recognise. We have also described the important complications to watch out for and an indication as to which dislocations should be referred for specialist management.

[Distraction of the first metacarpal bone for thumb reconstruction].

After amputation of the thumb good functional results can be achieved by lengthening the first metacarpal bone, if a suitable technique is used. Since 1987 we have treated six patients in this way. In all of our patients the thumb was mechanically stable and sensitive after lengthening, thus improving the patients' ability to use their hands for grasping. Other advantages include the relative simplicity of the method, the fact that no experience of microsurgery is necessary and that the surgery does not result in soft tissue damage. In most cases treatment is on an outpatient basis. Between th

Surgical treatment of dorsal cortical fractures of the third metacarpal bone in thoroughbred racehorses: 53 cases (1985-1989).

Between January 1985 and May 1989, 53 Thoroughbred horses (mean age 3.2 years) were surgically treated for dorsal cortical fractures of the third metacarpal bone (MC III). All horses were treated with cortical drilling through the fracture line (osteostixis). Diagnosis of the fractures was confirmed by xeroradiography. Lifetime racing records were obtained for all horses. Forty-seven horses returned to racing after surgery (89%). The mean time between surgery and the first race was 6.8 months. Horses had a mean of 10.9 starts before surgery and 16.1 starts after surgery. The mean earnings per

Radiographic features of the bones of the hand and wrist in achondroplasia: report of case.

Achondroplasia is a well-established and documented medical condition. Most of the diagnostic features are discernible from a clinical examination, which are complemented by radiographic findings of the bones of the skeleton. The findings from the hand-and-wrist radiograph of an affected Southern Chinese male infant, age thirty-nine-months, are reported. The findings from this case indicated an infrequently reported abnormality in the development of the hand in achondroplasia.

Functional treatment of metacarpal fractures 100 randomized cases with or without fixation.

Totally, 100 hundred subcapital or diaphyseal fractures of the second through the fifth metacarpal were randomized to either a dorsal/ulnar plaster cast immobilizing the wrist and the joints of the involved digits or a functional cast allowing the wrist and the digits a free range of motion. Due to better retaining ability, functional casting reduced volar angulation by two thirds for metacarpal shaft fractures and by one third for metacarpal neck fractures when compared with plaster cast immobilization. Restriction of wrist, metacarpophalangeal, and interphalangeal joint movements was more fr

Simultaneous dislocation of all five carpometacarpal joints.

A case of unique combination of simultaneous dislocation of all five carpometacarpal joints is presented. The mechanism of injury in this case seems to be some manner of punching with a closed fist rather than direct violence. Satisfactory results were achieved using simple principles of manipulation under anesthesia, open reduction, and skeletal fixation, followed by intensive physiotherapy.

Traumatic volar dislocation of the second, third and fourth carpometacarpal joints: mechanism and manipulation.

Traumatic volar dislocation of the second, third and fourth carpometacarpal joints without associated fracture is extremely rare. A single case, treated successfully by manipulative reduction supplemented with splint immobilization and isometric and isotonic muscle exercise is reported, with restoration of complete anatomical alignment revealed by postmanipulative radiographs. The radiology of carpometacarpal joint dislocation is discussed. The conservative traction-dorsiflexion manipulative reduction for multiple volar carpometacarpal joint dislocations is described and recommended before usi
