Rocker sole footwear can be indicated as a treatment for forefoot load distribution disorders, such as diabetic ulcers, metatarsalgia, and hallux rigidus, among others, with favorable clinical outcomes. Pressure analysis studies on rocker sole footwear highlight results that explain their clinical benefits for forefoot conditions. There is insufficient data in the literature to understand the changes in foot mobility and anatomy caused by rocker sole footwear. This study proposes a methodology using weight-bearing computed tomography (WBCT) to evaluate the anatomical alterations in the forefoo

The goal of this chapter is to familiarize orthopaedic surgeons with the latest information pertaining to typical problems of the forefoot. Painful and deforming problems of the forefoot are surprisingly common. The understanding of forefoot deformity and pathology has evolved considerably over the past decade. A more precise understanding of the mechanics of deformity and resulting pathology has resulted in significant improvements in nonsurgical and surgical managements. This chapter provides an up-to-date examination of the literature regarding the management of forefoot pathology. First, a

A 33 years old female patient presented with posttraumatic pain in the right foot for which radiographs of the right foot was advised. No fracture was detected on radiographs and patient was managed conservatively on medications and posterior splint immobilization. We found coincidentally a short fourth metatarsal and an accessory navicular bone in the right foot radiographs. After 3 weeks of immobilization, she underwent mobilization of the right foot, weight bearing and intensive physio- therapy for 6 weeks. After two months of injury she was still complaining of pain on the plantar aspect o

Morton's foot syndrome is a hereditary syndrome characterized by a short first metatarsal bone, posterior displacement of the sesamoids, and hypertrophy of the second metatarsal, causing excessive weight to be borne by the second metatarsal head. This condition results in callus formation under the second metatarsal. Pain and tenderness are usually felt at the base of the first two metatarsal bones and at the head of the second. Pain (metatarsalgia) may be disabling. Conservative treatment consists of placing a flexible pad under the first metatarsal and toe to increase the range of motion and

We reviewed the results of 15 patients (16 feet) in whom a hallux valgus procedure had failed. Salvage was by proximal crescentic first metatarsal osteotomy with distal soft-tissue reconstruction. Results based on a clinical scale considering the level of pain, activity limitations, support requirement, footwear limitations, and alignment were good in 11, fair in two, and poor in three. Patients were satisfied with the results in 10 feet, satisfied with reservations in four feet, and dissatisfied in two feet. Complications were: transfer metatarsalgia in three, hallux varus in one, and osteoto

45 patients were included in a prospective study to evaluate the results of Mitchell's osteotomy of hallux valgus. 43 patients complained of pain preoperatively. 44 patients were reviewed after one year, and excellent results were achieved in alleviating pain for 35 of these patients. Four patients developed metatarsalgia after surgery. There was a mean improvement in the hallux valgus angle of eight degrees. The mean shortening of the first metatarsal was 6 mm (1-12 mm). This shortening showed no correlation with postoperative pain. Signs of osteoarthrotic changes in the metatarsophalangeal j

Contrary to the general impression, the hollow foot is much more common than the flat foot. We distinguish three categories of hollow foot: varus hollow (clinical form I), valgus hollow (clinical form II), and pseudo-hollow (clinical form III). These three clinical forms have a common deviation dynamics, the consequence of shortening of the sural-Achilles-calcaneoplantar system. Metatarsalgia often precedes deformation of the front of the foot. Talalgia is linked to excess traction or to a conflict between the heel and the back of the shoe. Common therapy consists of plantar orthoses, eventual

The plantar plate of the foot is formed by the plantar aponeurosis and plantar capsule. The plantar plate arises from the distal plantar aspect of the metatarsal neck and inserts on the plantar aspect of the proximal phalangeal base. This thick plate supports the undersurface of the metatarsal head and resists hyperextension of the metatarsophalangeal joint (MTPJ) [1]. Plantar plate rupture may present as lesser metatarsalgia (the lesser metatarsals are the second through fifth), occasionally with exuberant synovitis. Plantar plate derangement also plays a central role in the genesis of the co

BACKGROUND: The present study was conducted in an attempt to obtain consistent similarities among histologic findings of surgically excised neuromas. Secondly, we looked for a correlation between the presence of a neuroma with certain comorbidities.
METHODS: A total of 22 specimens with a preoperative diagnosis of Morton's neuroma were sent to the pathology laboratory, and evaluation was performed by a single pathologist.
RESULTS: Degenerative changes were seen in 59% of the specimens. Patient age showed trends toward affecting nerve fibrosis, nerve diameter, vessel obstruction, and degenera

BACKGROUND: Metatarsalgia is a common source of patient discomfort and has multiple etiologies. Lesser metatarsal osteotomy is a recognized treatment modality for persistent symptoms after conservative care, yet the location and type of osteotomy remain highly debated topics. The current investigation reports the initial results regarding midshaft, segmental osteotomy of the lesser metatarsals with open reduction and internal fixation.
METHODS: Ninety-five consecutive patients had lesser metatarsal midshaft, segmental osteotomy with open reduction and internal fixation from June, 1999, to Oct

We present the short-term follow-up of 55 symptomatic hallux valgus deformities in 38 patients, treated operatively with a modification of the spike distal first metatarsal osteotomy, as described by Gibson and Piggott in 1962. The age range of the patients was 17 to 72 years at the time of surgery. The postoperative follow-up period was 12 to 55 months. Excellent and good clinical and radiographic results were recorded in 96.2% of our patients. Two of the patients (3.8%) were dissatisfied; one of them complained of metatarsalgia after the procedure, and the other had stiffness of the metatars

We analysed the functional adaptation of the first and second metatarsal bones to altered strain in flexible flatfoot. Fifty consecutive women (20-40 years of age) were enrolled: 31 patients with a flexible flatfoot and metatarsalgia (59 feet) and 19 controls with asymptomatic feet (37 feet). They were compared for cortical thickness (medial, lateral, dorsal and plantar) of the two bones. The null hypothesis of no overall difference between the deformed and healthy feet with regard to cortical thicknesses of the two bones was rejected in a multivariate test (p = 0.046). The groups differed sig

Pain in the region of the lesser metatarsophalangeal joints (often termed metatarsalgia) is a common complaint. It can be due to a variety of causes, and accurate diagnosis is essential for effective treatment. Understanding the anatomy and functions of the extrinsic and intrinsic musculature and the plantar plate, ligaments, and fat pad is important in evaluating metatarsophalangeal joint disorders. Claw toe is a hyperextension deformity of the metatarsophalangeal joint in combination with a hammer toe. Pathologic changes involving an isolated metatarsophalangeal joint may be due to monarticu

Twelve patients who had a total of 18 Mitchell bunionectomies were reviewed to assess the long-term results of the procedure. Although metatarsus varus correction was maintained in all cases, hallux valgus recurred in 11 of the 18 cases. Sixty-seven percent reported complete relief or improvement of preoperative pain. Although lateral metatarsalgia did occur, the most common area of persistent pain remained the first metatarsal. Six of 18 procedures had marked loss of active joint motion, associated with pain and an unsatisfactory result. Of 18 procedures, 11 (61%) were satisfied with the resu

Forefoot arthroplasty is often recommended for the management of metatarsalgia secondary to rheumatoid arthritis. Concurrently, the first metatarsophalangeal (MP) joint can be fused rather than excised. The results in 37 patients with 64 arthroplasty operations (34 with fusion and 30 with excision of the first joint) show that fusion produced a better cosmetic appearance of the foot, facilitated fitting with normal shoes, and improved overall balance. Pedobarograph measurements during gait indicated that relatively more weight was transmitted through the medial ray when the first metatarsophal

PURPOSE OF THE STUDY: Many different osteotomies can be used for the treatment of hallux valgus. The purpose of this study was to evaluate the Scarf osteotomy associated or not with phalangeal osteotomy and to search for deformation cutoff points beyond which corrections appear to be difficult to achieve.
MATERIAL AND METHODS: This retrospective analysis included 87 patients (123 feet) among 130 who underwent hallux valgus surgery between October 1993 and November 2000. Mean follow-up was four years eight months. The serie included 83 women and 4 men. Mean age at surgery was 53.5 years. A Sca

INTRODUCTION: When conservative therapy for hallux rigidus fails, surgical options such as arthrodesis and interposition arthroplasty can be considered. Although arthrodesis of MTP joint is the gold standard treatment. However patients desiring MTP joint movement may opt for either interposition arthroplasty or implant arthroplasty to avoid the movement restrictions of arthrodesis. The purpose of this systematic review was to investigate clinical outcomes and complications following interposition arthroplasty for moderate to severe hallux rigidus, for patietns who would prefer to maintain rang

OBJECTIVE: Perform the translation and cultural adaptation of the questionnaire Foot Functional Index (FFI), which assesses the functionality of the foot, to the Brazilian Portuguese version.
METHOD: The Brazilian version development of FFI questionnaire was based on the guideline proposed by Guillemin. The applied process consisted of: (1) translation; (2) back-translation; (3) committee review; (4) pretesting. The Portuguese version was applied to 40 patients, both genders, aged over 18 years old, with plantar fasciitis and metatarsalgia to verify the level of the instrument comprehension.

BACKGROUND: Osteoarthritis of the second and third tarsometatarsal (TMT) joints is a common cause of midfoot pain. Arthrodesis is a reliable treatment, although optimal joint preparation and fixation remain debated.
METHODS: A retrospective cohort study included 31 feet in 30 patients undergoing isolated or combined second and third TMT arthrodesis for idiopathic osteoarthritis (2016-2022). The technique used trephine-based joint preparation, calcaneal autograft, and nitinol staple fixation. Outcomes included MOXFQ, AOFAS scores, union, and complications.
RESULTS: Radiographic union was achi
