We have treated 68 thyrotoxic patients with Graves' disease with a single daily dose of 30 mg methimazole until they were clinically euthyroid and their plasma thyroid hormone concentrations were within normal limits. Sixteen of 56 patients (29%) treated 4.8 +/- 0.2 months (mean +/- SEM; range, 1.5-8.5 for their initial attack of thyrotoxicosis have remained in remission for 54.4 +/- 7.7 months (range, 12-105). The case refers to a premature newborn with anhydramnios and related foot malformations, renal dysplasia (Potter II-IV) and marked ascites. Hypoplasia of one umbilical cord artery was also found. Two cases of propylthiouracil-associated acute hepatitis, one case of fatal methimazole-associated hepatocellular necrosis and one case of propylthiouracil-associated lupus-like syndrome are described. The literature related to antithyroid drug side effects and the mechanisms for their occurrence are reviewed and the efficacy and complications of thyroidectomy and radioiodine compared to those of antithyroid drugs. It is concluded that in most circumstances 131I is the therapy of choice for hyperthyroidism.

In order to investigate the effect of antithyroid drugs on serum titers of antithyroglobulin and anti-thyroid microsomal antibody, these two antibody levels were studied in 38 patients of Graves' disease with hyperthyroidism before and after treatments with carbimazole or methimazole. The results showed no consistent changes in their titers. To explain the inconsistent findings as compared with other authors' studies, lower doses of antithyroid drugs were postulated as one of the causes.
