Quality of life in inflammatory neuropathies: the IN-QoL.

BACKGROUND: No consensus exists which quality of life (QoL) measure should be used in patients with inflammatory neuropathies. Moreover, most QoL measures are ordinal-based scales with their known deficiencies. OBJECTIVES: To establish a new disease-specific interval-based QoL questionnaire in inflammatory neuropathies (IN-QoL) using the Rasch model and evaluate its scientific properties (validity, reliability and responsiveness). METHODS: 264 patients with inflammatory neuropathies completed six commonly used QoL questionnaires. The obtained data were stacked and subjected to Rasch analysis

Opioid use prior to arthroscopic rotator cuff repair is associated with inferior patient-reported outcomes and lower rates of achieving patient acceptable symptom state in the early postoperative period.

Proportion of patients achieving clinically relevant measures of minimal clinically important difference (MCID), substantial clinical benefit (SCB), and patient acceptable symptom state (PASS) was determined. Preoperative Contralateral Joint And/Or Back Pain Impact on Achieving the Substantial Clinical Benefit for Hip Disability and Osteoarthritis Outcome Score-Joint Replacement.

BACKGROUND: The Centers for Medicare & Medicaid Services recently mandated reporting of patient-reported outcome measures following primary … eral hip pain and severity. Outcomes included achievement of SCB (22-point improvement in HOOS-JR), minimal clinically important difference, patient-acceptable symptom state, and satisfaction at one year. Multivariable logistic regression was adjusted for demographic, clinical, and socioeconomic factors. RESULTS: Patients who had both back and contralateral hip pain had the highest odd …

Contralateral THAs More Than 1 Year Apart: Do PROMs and Healthcare Utilization Differ After Each Procedure?

BACKGROUND: Patients who undergo a second THA at least 1 year after the first one may experience different recovery courses after each THA. It is unknown what the clinically relevant improvements and healthcare utilization are after each THA in patients undergoing contralateral THA > 1 year apart. QUESTIONS/PURPOSES: (1) Do patient-reported outcome measures (PROMs) differ at baseline and 1 year after THA for the first and second hip arthroplasty? (2) Does the likelihood of achieving minimum clinically important difference (MCID) and patient acceptable symptom state (PASS) thresholds differ fo

Doxycycline for community treatment of suspected COVID-19 in people at high risk of adverse outcomes in the UK (PRINCIPLE): a randomised, controlled, open-label, adaptive platform trial.

BACKGROUND: Doxycycline is often used for treating COVID-19 respiratory symptoms in the community despite an absence of evidence from clinical trials to support its use. We aimed to assess the efficacy of doxycycline to treat suspected COVID-19 in the community among people at high risk of adverse outcomes. METHODS: We did a national, open-label, multi-arm, adaptive platform randomised trial of interventions against COVID-19 in older people (PRINCIPLE) across primary care centres in the UK. We included people aged 65 years or older, or 50 years or older with comorbidities (weakened immune sys

Outcome Trends After Hip Arthroscopy for Femoroacetabular Impingement: When Do Patients Improve?

PURPOSE: To determine when patients reach critical thresholds of clinical improvement after hip arthroscopy for femoroacetabular impingement (FAI) using previously defined cutoffs for the minimal clinically important difference (MCID) and patient acceptable symptomatic state (PASS) and to identify risk factors for prolonged recovery. METHODS: Consecutive patients with a diagnosis of FAI who underwent unilateral hip arthroscopy between January 2010 and January 2015 with at least 2 years of clinical follow-up were studied. Dose to the bladder neck in MRI-guided high-dose-rate prostate brachytherapy: Impact on acute urinary toxicity and health-related quality of life.

PURPOSE: To assess the impact of the dose to the bladder neck (BN) on acute urinary toxicity (AUT) and health-related quality of life (uHRQoL) in patients with prostate cancer treated with MRI-guided high-dose-rate brachytherapy combined to external beam radiotherapy. METHODS AND MATERIALS: Sixty-one patients were treated with a single 15-Gy MRI-guided high-dose-rate brachytherapy followed by external beam radiotherapy as part of a prospective Phase II trial. The BN was delineated in retrospect on T2-weighted images. AUT and uHRQoL data were collected prospectively using Common Terminology Cr

Is Arm Dominance Associated With Clinically Meaningful Differences in Outcomes After Shoulder Arthroplasty?

BACKGROUND: Limited available evidence seems to suggest that the increased use of the dominant (versus nondominant) limb may allow for earlier return to function and better ROM in the dominant limb at 12-month follow-up after anatomic or reverse total shoulder arthroplasty (TSA). Nevertheless, whether the earlier achievement of physical therapy milestones is associated with a clinically meaningful difference in patient-reported outcome measures (PROMs) is yet to be determined. QUESTIONS/PURPOSES: (1) What are the 12-month minimum clinically important difference (MCID) and patient acceptable s

Airway Bacterial Load and Inhaled Antibiotic Response in Bronchiectasis.

Rationale: The principal underlying inhaled antibiotic treatment in bronchiectasis is that airway bacterial load drives inflammation, and therefore antibiotic treatment will reduce symptoms. Objectives: To determine the relationship between bacterial load and clinical outcomes, assess the stability of bacterial load over time, and test the hypothesis that response to inhaled antibiotics would be predicted by baseline bacterial load. Methods: We performed three studies. Studies 1 and 2 were prospective studies including adults with bronchiectasis. Study 3 was a post hoc analysis of a randomized

How Can We Define Clinically Important Improvement in Pain Scores After Hip Arthroscopy for Femoroacetabular Impingement Syndrome? Minimum 2-Year Follow-up Study.

BACKGROUND: Patient postoperative pain is being increasingly reported in the field of hip preservation surgery. Currently, there is limited understanding of clinically significant improvement in VAS pain.
