Applications of transcranial magnetic stimulation in movement disorders.

The author reviews the applications of transcranial magnetic stimulation (TMS) in a series of movement disorders--namely, Parkinson's disease, corticobasal degeneration, multiple system atrophy, progressive supranuclear palsy, essential tremor, dystonia, Huntington's chorea, myoclonus, the ataxias, Tourette's syndrome, restless legs syndrome, Wilson's disease, Rett syndrome, and stiff-person syndrome. Single- and paired-pulse TMS studies have been done mainly for pathophysiologic purposes. Repetitive TMS has been used largely for therapy. Many TMS abnormalities are seen in the different diseas…

Spontaneous and drug-induced movement disorders in schizophrenia.

The extrapyramidal side-effects (EPS) of conventional neuroleptics are widely recognized. However, a substantial number of drug-naïve patients exhibit movement disorders which could be mistaken for EPS if drug therapy were started without a careful baseline assessment. Patients' experience of EPS during their first exposure to antipsychotic medication can have lasting effects on their attitudes to medication and compliance. Unmedicated, approximately 60% of patients will relapse in the first year after resolution of an acute psychotic episode, whereas prophylactic medication can reduce the rel…

Horizontal eye movement disorders after posterior vermis infarctions.

The horizontal saccade, smooth pursuit, and vestibulo-ocular reflex gains were recorded in 19 patients with cerebellar infarction documented with MRI, and in a group of control subjects. Bilateral saccade hypometria and a decrease in ipsilateral smooth pursuit gain were found only in patients with a lesion affecting the posterior vermis. These results in humans support experimental findings suggesting that the posterior vermis controls both saccade accuracy and smooth pursuit velocity.

Psychogenic movement disorders: frequency, clinical profile, and characteristics.

Of 842 consecutive patients with movement disorders seen over a 71 month period, 28 (3.3%) were diagnosed as having a documented or clinically established psychogenic movement disorder. Tremor was most common (50%) followed by dystonia, myoclonus, and parkinsonism. Clinical descriptions of various types are reviewed. Clinical characteristics common in these patients included distractability (86%), abrupt onset (54%), and selective disabilities (39%). Distractability seems to be most important in tremor and least important in dystonia. Other diagnostic clues included entrainment of tremor to th…

Prevalence of movement disorders in elderly community residents.

We studied movement disorders (MD) in community residents of Saskatchewan aged > or = 65 years. A representative sample of 70 individuals had detailed neurological evaluation. Those who had definite and possible MD at first examination had a second neurological assessment by a senior neurologist. The most prevalent MD was essential tremor (ET) (14%) followed by Parkinson's disease (PD) (3%). The marked difference in prevalence ratios for PD and ET are attributed to: (a) higher incidence of ET in the general population; (b) a greater possibility that most, if not all, ET cases would reside in t…

[Movement disorders in HTLV-I associated myelopathy (HAM)].

Periodic movements in sleep (nocturnal myoclonus) are characterized by a triple flexion of the ankle, knee and hip, which are particularly evident during 1-2 and 2-3 sleep stages. Iijima et al (1991) reported these movements in 5 out of 7 HAM patients, suggesting that nocturnal myoclonus is not rare in HAM. L-dopa and bromocriptine are reported to be the most effective. Spinal myoclonus (SM) is characterized by symmetric, rhythmic involuntary contractions of muscle groups supplied by one or several contiguous segments of the spinal cord. There has been only one case report of SM by Kanda et al

Cervical myelopathy secondary to movement disorders: case report.

Involuntary cervical movements that result in cervical spondylosis and secondary myelopathy have not been adequately emphasized in the literature. We present two patients with cerebral palsy and long histories of involuntary movements who developed cervical myelopathy. We use these illustrative cases to emphasize that in the presence of underlying neurological symptoms and signs due to advanced cerebral palsy, any recent worsening should provoke a higher index of suspicion for myelopathy. The mechanics of neck movements and surgical treatment are discussed.

[Clinical analysis of movement disorders in reactive states].

Fifty-two patients with reactive states were studied for specific features of their motor disorders. The clinical follow-up data were supplemented by electromyographic examinations. The motor pathology was shown to be characterized by polymorphism and marked atypical nature. It was revealed that under conditions of considerable functional disintegration of the cerebral motor systems, the motor pathology observed was similar to that seen with organic brain diseases.

Treatment of Disorders of Tone and Other Considerations in Pediatric Movement Disorders.

Pediatric movement disorders (PMDs) consist of a heterogeneous group of signs and symptoms caused by numerous neurological diseases. Different neurological disorders in children also share overlapping movement disorders making a diagnosis of the underlying cause of the movement disorder challenging. The similarity of the symptoms across multiple disease types suggests that there may be a final common motor pathway causing the overlapping movement disorders. There are numerous disorders in children associated with disturbances in tone and involuntary movements. This chapter will focus primarily…

Psychogenic balance disorders: is it a new entity of psychogenic movement disorders?

The various reported psychogenic dyskinesias include tremor, dystonia, myoclonus, gait disorder, Parkinsonism, tics, and chorea. It is not easy to diagnose psychogenic movement disorders, especially in patients with underlying organic disease. We describe three patients with balance and/or posture abnormalities that occur when they stand up, start to move, or halt from walking, although their gaits are normal. One had an underlying unilateral frontal lobe lesion. All patients improved dramatically after receiving a placebo-injection or medication. These abnormal features differ from the previo…

Neuroleptic-induced movement disorders in a naturalistic schizophrenia population: diagnostic value of actometric movement patterns.

BACKGROUND: Neuroleptic-induced movement disorders (NIMDs) have overlapping co-morbidity. Earlier studies have described typical clinical movement patterns for individual NIMDs. This study aimed to identify specific movement patterns for each individual NIMD using actometry. METHODS: A naturalistic population of 99 schizophrenia inpatients using conventional antipsychotics and clozapine was evaluated. Subjects with NIMDs were categorized using the criteria for NIMD found in the Diagnostic and Statistical Manual for Mental Disorders - Fourth Edition (DSM-IV).Two blinded raters evaluated the ac…

Cheek-biting disorder: another stereotypic movement disorder?

Recurrent cheek biting, a form of self-injurious behavior is a rare entity which presents mostly to dentists and dermatologists. We report a case of recurrent severe cheek biting in an adult male leading to mucosal ulceration. The stereotypic pattern of cheek biting and associated behavior bears striking resemblance to other impulse control disorders.

Clinical features of adolescent-onset functional motor disorders in tertiary movement disorders centers.

BACKGROUND: Functional Motor Disorders (FMDs) represent a diagnostic and therapeutic challenge in pediatric neurology, particularly among ad … patients with a clinically definite diagnosis of FMD consecutively enrolled at 25 Italian tertiary movement disorders centers. RESULTS: Among 847 patients, 93 (10.9%) had adolescent-onset FMDs. Motor phenotypes did not differ significantly between adolescent- and adult-onset FMDs, with the exception of parkinsonism, which was observed only in the latter. Compared …

Movement disorders rounds: Neurophysiological findings in abdominal movements in CASPR2 encephalitis.

Abdominal movements have been described as a possible unique manifestation of CASPR2-related autoimmune encephalitis. We describe a case of abdominal movements with neurophysiological evaluation showing a mean EMG burst duration of 350 ms and frequency of 1 Hz compatible with myorrhythmia. The case illustrates that awareness of the unique presentations of CASPR2-related autoimmune encephalitis can allow for timely diagnosis and initiation of appropriate treatment.
