NYX-2925, A Novel N-methyl-D-aspartate Receptor Modulator: A First-in-Human, Randomized, Double-blind Study of Safety and Pharmacokinetics in Adults.

NYX-2925, a new chemical entity, acts as a co-agonist to glutamate at the N-methyl-D-aspartate receptor (NMDAR). At low concentrations of endogenous agonists (glycine/D-serine), NYX-2925 partially activates NMDARs, modulating neural pathways relevant for chronic pain. NYX-2925 is being developed for the treatment of chronic pain conditions, including painful diabetic peripheral neuropathy and fibromyalgia. In this first-in-human, phase I, single-ascending dose (50-1,200 mg) and multiple-ascending dose (150-900 mg) study, the safety, tolerability, and pharmacokinetics (PKs) of NYX-2925 were eva

Multiple associations between a broad spectrum of autoimmune diseases, chronic inflammatory diseases and cancer.

BACKGROUND: Many recent studies suggest the immune system plays a significant role in the pathogenesis of autoimmune diseases, chronic inflammatory diseases, and cancer. MATERIALS AND METHODS: Literature published between 2001 and 2011 was reviewed for risk of cancer development in patients with autoimmune and chronic inflammatory diseases. Mode of risk assessment employed did not limit inclusion of studies. Autoimmune conditions developing after diagnosis of a pre-existing cancer were also considered. RESULTS: We report a pervasive, largely positive association between 23 autoimmune and inf

Pharmacologic Management of Chronic Pain.

Pharmacologic management of chronic pain is one component of a patient-centered care plan. Multiple classes of medications are available and can be used individually or in combination. Choice of medication is determined by the type and cause of pain, safety profile of the medication, patient values and preferences, comorbid conditions, cost, and availability. Incorporating shared decision making is critical when implementing a pharmacologic pain management regimen.

Fatal pulmonary complications in an immunodeficient child with chronic active Epstein-Barr virus infection.

Primary Epstein-Barr virus infection in children typically presents as infectious mononucleosis and in immunocompetent individuals severe pneumonitis proves to be a rare complication. Chronic active Epstein-Barr virus infection (CAEBV) is associated with multiple life-threatening conditions, including interstitial lung disease with fibrosis and lymphoid and lymphohistiocytic infiltrations. We report on a pediatric patient in whom CAEBV resulted in severe pneumopathy with a fatal outcome.

The role of ghrelin in anorexia-cachexia syndromes.

Anorexia, sarcopenia, and cachexia are common complications of many chronic conditions including cancer, rheumatoid arthritis, HIV infection, aging, and chronic lung, heart, or kidney disease. Currently, there is no effective treatment for muscle atrophy or wasting conditions although they typically take a significant toll on the quality of life of patients and are associated with poor prognosis and decreased survival. Ghrelin affects multiple key pathways in the regulation of body weight, body composition, and appetite in the setting of cachexia that may lead to an increase in appetite and gr

The US Department of Veterans Affairs Gulf War Era Cohort Study: Results From the 30-Year Follow-up Assessment.

OBJECTIVES: This study aimed to report the physical and mental health of a population-based cohort of Gulf War and Gulf Era veterans 30 years after the war. METHODS: A follow-up health survey of 12,377 Gulf War and Gulf Era veterans included questions about general, physical, mental, and functional health. Adjusted odds ratios weighted by final survey weights were calculated to determine risk of health conditions associated with deployment. RESULTS: Gulf War veterans report a higher prevalence of a range of physical and mental health conditions. The population has a significant burden of dis

Chronic thrombotic thrombocytopenic purpura masquerading as other disease entities.

A 37-year-old woman with a chief complaint of nausea, headache and, prolonged menses was diagnosed with thrombotic thrombocytopenic purpura based on a peripheral smear with active microangiopathic hemolytic anemia and a platelet count of 4,000/mm3. Her past medical history was significant for several conditions including multiple sclerosis, Bell's palsy, Raynaud's syndrome and HELLP syndrome. In retrospect, it appears that this patient's clinical history was most consistent with one unifying diagnosis, chronic thrombotic thrombocytopenic purpura. Physicians should search for the elusive clinic

Periapical cemento-osseous dysplasia masquerading as asymptomatic chronic apical periodontitis in a Chinese woman: A case report.

KEY CLINICAL MESSAGE: Cemento-osseous dysplasia (COD) belongs to a group of fibrous osseous disorders that can masquerade as periapical inflammatory conditions in the jawbones. We present a rare case of COD occurring in a patient who also had periapical periodontitis concurrently. When faced with a patient exhibiting no symptoms, diagnosis may be challenging. ABSTRACT: COD is a fibrous osseous disorder similar to periapical chronic inflammatory conditions and other cysts in the jaw bones on the radiograph. There is a rare case of COD occurring in a 49-year-old Chinese woman who also had chron

Daily Saline Nasal Douching for Chronic Allergic Rhinosinusitis: An Autobiographical Case Report.

Chronic allergic rhinosinusitis is a debilitating condition in a group of susceptible individuals with multifactorial etiology. I present my case of suffering from chronic allergic rhinosinusitis for 25 years treated with multiple drugs and surgeries with no relief. Adjunctive daily saline nasal douching has provided significant relief for three years to date. Genetic susceptibility with defective ciliary clearance, mucosal metaplasia with increased goblet cells secreting thick and inspissated mucus blocking the normal drainage, and stasis of secretions with inadequate mucosal immunity predisp

Different types of housing and respiratory health outcomes.

Evidence has shown that housing conditions may substantially influence the health of residents. Different types of housing have different structures and construction materials, which may affect indoor environment and housing conditions. This study aimed to investigate whether people living in different types of housing have different respiratory health outcomes. The data from the 1999-2006 National Health and Nutrition Examination Survey were used for the analyses. The types of housing included houses, townhouses, apartments, and mobile homes. Respiratory symptoms included wheezing, coughing,

Actinomycetoma of the wrist: diagnostic insights from a pathology perspective (case report).

Actinomycetoma is a chronic, progressive subcutaneous infection caused by filamentous bacteria of the Actinomycetales order, characterized by tumefaction, sinuses, and grains. We report a rare case of actinomycetoma affecting the wrist of a 45-year-old male, which presented as a chronic, slowly enlarging swelling with multiple discharging sinuses. Initial clinical suspicion was broad, encompassing various chronic infections or soft tissue tumors. However, definitive diagnosis was established through meticulous histopathological examination of a biopsy specimen, revealing characteristic "sulphu

Transplantation of splenic tissue after splenectomy: A case report.

Transplantation of splenic tissue is a rare condition that usually occurs after splenic trauma and splenectomy. It usually requires surgery for diagnosis and treatment. A 38-year-old Asian male with familial hemolytic disease underwent laparoscopic splenectomy for a traumatic rupture of the spleen one year prior. The patient developed middle-upper abdominal pain without any obvious cause, radiating to the back and chest seven months prior to presentation. The condition improved with conservative treatment but the patient experienced recurrent episodes. Abdominal CT suggested multiple gallstone

Autoimmune disorders in Kabuki syndrome.

Kabuki syndrome is associated with abnormalities in multiple organ systems. While many of the anomalies are congenital malformations, other clinical manifestations may not appear until later in childhood. Among these associated conditions, autoimmune abnormalities have been described in several patients. These include idiopathic thrombocytopenic purpura (ITP), hemolytic anemia, thyroiditis, and vitiligo. In this report, we describe five affected patients with autoimmune manifestations. Four patients had ITP, and two of these patients had concurrent hemolytic anemia. The fifth patient had vitil

The relationship between chronic lung diseases and lung cancer - a narrative review.

Lung cancer and chronic lung diseases are currently two of the main causes of death in the world. Both conditions have common etiological factors and multiple research directions in the last decades demonstrated the presence of some common relevant biological mechanisms which can explain why patients with chronic respiratory diseases are at higher risk of developing lung cancer. In this review, we discuss the role of chronic pulmonary diseases, such as chronic obstructive pulmonary disease, tuberculosis, sarcoidosis, asthma, pneumoconioses, idiopathic pulmonary fibrosis and their impact on lun

Diagnosis and Management of Central Nervous System Demyelinating Disorders.

The spectrum of demyelinating diseases affecting the central nervous system is broad. Although many have a chronic course, neuroinflammatory conditions often present with acute to subacute onset symptoms requiring hospitalization when severe. This article reviews the acute phase assessment and management of these disorders, with a particular focus on multiple sclerosis, neuromyelitis optica spectrum disorder, myelin oligodendrocyte glycoprotein antibody disorder, and several atypical demyelinating diseases.
