A case of atypical scleroderma associated with multiple myeloma is reported. The patient was a 50-year-old male; morphea-like plaques with brownish pigmentation were scattered on the whole body and sclerodactylia-like changes, contracture of fingers and polymyositis were observed. Histological findings in the sclerotic lesions were consistent with scleroderma. A total of 399 patients with multiple myeloma and an equal number of match controls were interviewed about factors possibly related to the causes of their disease. Factors studied included occupation, chemical exposure, radiation exposure, prior diseases, immunizations, chronic infections and markers for defects in immune regulation. Multiple myeloma is a malignancy of the cells of the immune system that frequently involves the eye. Cysts of the ciliary body have been reported in 33-50% of myeloma patients, and retinal vascular lesions have been reported in up to 66%. Corneal and orbital involvement is less common. In this review, systemic and immunologic aspects of multiple myeloma are outlined and ocular manifestations are illustrated.

Authors report one case of intravertebral vacuum phenomenon associated with a multiple myeloma. Initially, there occurred a collapse and a lysis of the L4 vertebral body. Two months later, after chemotherapy and cobalt-therapy, X-ray examination showed a vacuum cleft phenomenon within the body of L4 and a backward displacement of the L4 posterior wall. We report a unique case of meningeal involvement of multiple myeloma in a 49-year-old man. The patient developed weakness late in the course of his disease, and a computed tomographic scan revealed intracranial masses. At postmortem examination these tumors were the predominant extramedullary manifestation of the patient's anaplastic myeloma. To our knowledge this is the first case of intracranial myeloma composed of multiple, discrete masses.

A 62-year-old woman presenting with intracranial lesion eroding the sella with compression of optic chiasma was found to have plasmacytoma of the pituitary area. At the time of initial surgery, the patient had no biochemical, immunologic or marrow findings of multiple myeloma. The intracranial tumor was interpreted initially as chromophobe adenoma on light microscopy, but the diagnosis of plasmacytoma was established by electron microscopic examination of the tumor. The case illustrates the usefulness of electron microscopy as a diagnostic tool.
