Myasthenia Gravis With Chronic Kidney Disease: A Diagnostic Challenge.

The coexistence of myasthenia gravis (MG) and chronic kidney disease (CKD) presents a diagnostic challenge due to overlapping clinical features. MG is an autoimmune neuromuscular junction disorder, whereas CKD exhibits the gradual loss of kidney function. We report a 65-year-old male with long-standing CKD who developed progressive neuromuscular symptoms, including bilateral ptosis and bulbar involvement. His anti-acetylcholine receptor antibodies were negative; however, electrophysiological studies demonstrated a significant decremental response (> 10%) on repetitive nerve stimulation, suppor…

Pharmacological and speech-language pathology management of dysphagia in patients with myasthenia gravis.

Myasthenia gravis (MG) is a rare, complex autoimmune neuromuscular disorder characterized by fluctuating muscle weakness. Bulbar involvement, particularly dysphagia (difficulty swallowing), is a common and debilitating symptom that significantly increases the risk of aspiration pneumonia, malnutrition, and dehydration, profoundly impacting quality of life. The management of myasthenic dysphagia requires a sophisticated, interdisciplinary approach. This review aims to provide a comprehensive overview of the current and emerging pharmacological treatments for myasthenia gravis and to detail the …

Preperitoneal Myelolipoma Occurring after Video-Assisted Thoracoscopic Surgery for Thymoma: A Case Report.

INTRODUCTION: Myelolipoma is a benign tumor composed of mature adipose tissue and hematopoietic elements that typically arises in the adrenal glands, and involvement of the anterior abdominal region is extremely uncommon. We report a rare case of myelolipoma arising in the subxiphoid preperitoneal region adjacent to the site where a sternum-lifting device had been placed during previous surgery. CASE PRESENTATION: A 62-year-old man had undergone extended thymectomy for thymoma associated with myasthenia gravis at the age of 39. The procedure was performed via bilateral video-assisted thoracos…

Application of Droplet Digital PCR in Sputum Samples in Myasthenia Gravis Patients with Pneumonia.

BACKGROUND: Due to the rapid progression of the pneumonia in patients with Myasthenia gravis (MG), faster pathogen detection techniques are needed. The droplet digital polymerase chain reaction (ddPCR) has the ability to detect pathogens in about 3 h. Thus, this study focused on application of ddPCR in sputum samples in the MG patients with pneumonia and analyzed the association between ddPCR and other laboratory results. METHODS: We prospectively enrolled 22 MG inpatients with pneumonia and collected 24 sputum samples. All samples were analyzed using traditional culture, ddPCR and metagenomi…

Association of sleep disorder with bulbar weakness and short-term outcomes in myasthenia gravis.

BACKGROUND: Sleep disorder is increasingly recognized in myasthenia gravis (MG). However, its specific link to bulbar muscle weakness and short-term outcomes is not well established. This study aimed to investigate the prevalence of subjective sleep disorder (based on Pittsburgh Sleep Quality Index [PSQI] score) in a cohort of acetylcholine receptor (AChR) antibody-positive MG patients, and to explore its associations with bulbar muscle weakness, disease severity, and its value for the short-term outcome of MG. METHODS: We conducted a prospective cohort study of 163 AChR-MG patients. Subjecti…

Fiber-type-specific architecture and pathophysiology of the neuromuscular junction.

The neuromuscular junction (NMJ) is a specialized synapse essential for translating neuronal signals into muscle contraction. This review ex … ce NMJ morphology, with fast-twitch junctions more prone to degeneration in conditions such as ALS, myasthenia gravis, and diabetic neuropathy. Slow-twitch NMJs often resist early deterioration due to superior trophic support, metabolic stability, and more robust expression of synaptic organizers, such as agrin and PGC-1α. Several key signaling pathwa …

Eculizumab as a Fast-Acting Therapy in a Myasthenic Crisis Patient With Poor Response to Efgartigimod: A Case Report.

INTRODUCTION: Myasthenic crisis (MC) is the most serious manifestation of myasthenia gravis (MG) and it poses a big challenge in clinical management. Plasma exchange and intravenous immunoglobulin are the first-line therapy for MC, but a subset of patients exhibit poor response. Recently, neonatal Fc receptor (FcRn) inhibitors and complement inhibitors showed great efficacy in MC. However, there is no report on whether eculizumab can serve as a rescue therapy for patients who have not responded to efgartigimod. CASE REPORT: This case report describes a Chinese patient with generalized MG who …

Surgical Management of Unexpected Thymic Anatomical Variations During Thoracoscopic Thymectomy.

OBJECTIVES: Complete thoracoscopic thymic resection is crucial for achieving sternotomy-comparable outcomes in myasthenia gravis patients. Thymic anatomy varies significantly, presenting features like retro-left brachiocephalic vein (LBCV) horns and thymic ring. Surgeons encountering these variations may miss abnormal thymic tissue or face intraoperative challenges. This research aims to report on the thoracoscopic management of thymic variations, illustrating their perioperative outcomes. METHODS: This retrospective observational study was conducted from September 2016 to September 2024. A t…
