[Intracranial diffusion. A complication of retrobulbar anesthesia].

Retrobulbar anesthesia is considered a safe, effective local-regional technique for ocular surgery. However, local complications that can arise from needle trauma during injection, include retrobulbar hemorrhage, perforation of the ocular globe and occlusion of the artery and/or the central vein of the retina. Systemic complications, such as respiratory arrest, convulsions and cardiovascular collapse, have also been reported, and although they only occur in less than 1% of cases, they are potentially life threatening. We report two cases of intracranial diffusion after retrobulbar anesthesia a

Iris ischaemia following penetrating keratoplasty for keratoconus (Urrets-Zavalia syndrome).

A fixed and dilated pupil is an uncommon postoperative complication after penetrating keratoplasty (PK) for keratoconus. Although the clinical features have been well described, the precise aetiology is uncertain. We performed anterior segment fluorescein angiography in the early postoperative period on three patients who developed fixed, dilated pupils after apparently uncomplicated surgery. All of the eyes had severe iris ischaemia. A possible role for a postoperative rise in intraocular pressure in the aetiology of this syndrome is discussed.

Surgical Third Nerve Palsy as a Rare Presentation of Tuberculous Meningitis: Early Clinical Judgement and Advanced Imaging.

A 22-year-old man from Bangladesh presented with a subacute febrile illness, a history of frontal headache, neuropsychiatric changes, and a right third nerve palsy (ptosis and mydriasis). Neuroimaging revealed hydrocephalus and multifocal cerebral infarcts. Cerebrospinal fluid (CSF) analysis showed elevated protein with normal glucose; however, all initial microbiological assays, including GeneXpert MTB/RIF (Mycobacterium tuberculosis/rifampicin) and culture, were negative. A clinical diagnosis of tuberculous meningitis (TBM) was made based on the clinical presentation, neuroimaging progressio…

Chopsticks-Induced Traumatic Choroidal Rupture With Submacular Haemorrhage in a Child.

We report a case of traumatic choroidal rupture in a child caused by accidental injury with chopsticks at school. A healthy 12-year-old boy presented with severe right eye pain, tearing, and significant blurring of vision (6/60), right eye subconjunctival haemorrhage, corneal abrasion, traumatic mydriasis with uveitis, and posterior subcapsular rosette cataract as well as a massive submacular haemorrhage with 360 degrees of peripheral commotio retinae. Optical coherence tomography of the macula revealed areas of choroidal rupture near the optic disc and at the fovea. Conservative treatment was…

Temporal lobe uncal herniation with contralateral superior cerebellar artery infarct.

Herniation of the temporal lobe uncus typically leads to the compression of the ipsilateral oculomotor nerve, resulting in ipsilateral mydriasis, as well as compression of the ipsilateral posterior cerebral artery, leading to infarction in the posterior inferior temporal lobe and medial occipital cortex. In this report, we present the case of a 45-year-old man with a large left subdural hematoma. At autopsy, we observed left cingulate and uncal herniations, along with the characteristic lesions of Kernohan notch phenomenon due to compression of the contralateral cerebral peduncle. Additionally…

Pauci-symptomatic foodborne botulism due to Clostridium botulinum type B with predominant ophthalmologic presentation possibly after consumption of honey.

W … ght the importance of recognizing clinical ophthalmologic botulism symptoms as unreactive bilateral mydriasis with lack of accommodation to contribute to earlier diagnosis in case of pauci-symptomatic botulism.

Bilateral multiple primary iridociliary cysts - A rare case report.

UNLABELLED: Iridociliary cysts are often found in patients as an incidental finding during a routine ophthalmic evaluation and if asymptomatic do not require any treatment per se. However, these are often mistaken as iris melanoma and the patient is managed accordingly causing a great deal of inconvenience to the patient as well as the doctor. CASE PRESENTATION: A 69 year old male presented to the ophthalmic out-patients department for routine ophthalmic evaluation. On examination, he is best corrected visual acuity was 6/9, N6 in both eyes. Post-mydriasis evaluation showed a Grade II nuclear…

[Management of severe traumatic brain injury].

Severe brain injuries, most often occurring in young subjects, are a major source of lost work years. These injuries are medical and surgical emergencies. Prehospital management of severe brain injuries requires intubation and mechanical ventilation aimed at normal arterial carbon dioxide pressure. Signs of transtentorial herniation: Uni- or bilateral mydriasis requires immediate perfusion of 20% mannitol or hypertonic sodium chloride. Neurological disorders after head injury justify emergency cerebral computed tomography. The presence of a mass syndrome or signs of transtentorial herniation a…

Pupillary and cardiovascular responses to the cold-pressor test.

Little is known about the structures and mechanisms involved in the autonomic response to sensory and nociceptive stimulation. In this study, we recorded simultaneously pupil diameter, blood pressure, heart rate and pain levels during a cold-pressor test in 10 healthy subjects. The aim was to evaluate the different components of the Autonomic Nervous System involved in the response to a thermal-painful stimulus. A biphasic pupillary response was observed: an initial, short-lasting mydriasis, followed by a miotic phase, this latter characterized by rhythmic fluctuations of pupil diameter. The b…

Cyclopentolate 1% requires more time to produce mydriasis and cycloplegia in eyes with heavily pigmented irides; however, its final effect on refractive error is apparently independent of iris pigmentation and depends on the amount of spherical refractive error present.

Central anticholinergic syndrome after ofloxacin overdose and therapeutic doses of diphenhydramine and chlormezanone.

A 14-year-old female with no history of psychiatric disease ingested an unknown amount of ofloxacin, diphenhydramine and chlormezanone after an argument with her patients. Approximately 12 hours after ingestion, the patient was admitted to the hospital in a delirious state with extreme mydriasis and warm and dry skin. Analytical data on admission were consistent with ofloxacin overdose and ingestion of therapeutic doses of diphenhydramine and chlormezanone. [Argyll-Robertson and other pupillary abnormalities in neurosyphilis--with special reference to the detection of peripheral lesions].

The pupillary disturbances in twenty five neurosyphilitic patients were investigated. Miotic, light-rigid pupils, i.e., Argyll-Robertson pupils were found in only three of these patients. One of three patients showed unilateral Argyll-Robertson pupil. Irregular margins of the iris were associated with these miotic, light-rigid pupils. Abnormal striations of the iris were found in four of eight non-miotic (medium size), light-rigid pupils. Cocaine or ephedrine produced dilatation of two of these pupils, and, following this mydriasis, the pupils were capable of constricting to light. These abnor…
