Chiropractic sympathectomy: carotid artery dissection with oculosympathetic palsy after chiropractic manipulation of the neck.

The clinical association between carotid artery dissection and oculosympathetic palsy is well-known. However, the occurrence of this combination with chiropractic manipulation of the cervical neck is rare. We describe the case of a 54-year-old Caucasian American male who underwent cervical manipulation for neck pain. The following day he noticed drooping of his left eyelid, with an unequal pupil size. Imaging later confirmed carotid artery dissection. Only three previous cases (one from the United States ), of an association between carotid artery dissection and chiropractic sympathectomy (ocu

Platysma myocutaneous flap including the external jugular vein with special reference to neck dissection.

Although platysma myocutaneous flap (PTMCF) is suitable for a small intraoral defect, combination with neck dissection (ND) decreases the viability. Therefore, a version of PTMCF was devised to increase the viability. This version consists of three points: preservation of the external jugular vein (EJV), preservation of the facial artery after skeletonization from the submandibular gland, and a new incision around the flap. This procedure was applied to 21 patients with intraoral cancer. All 21 patients underwent some kind of ND. The average viable area of transferred skin was 80%. This rate w…

Measurement of the trapezius muscle volume: A new assessment strategy of shoulder dysfunction after neck dissection for the treatment of head and neck cancers.

BACKGROUND: The purpose of this study was to determine the actual degree of shoulder muscle change and its relation to symptoms after neck dissection for head and neck cancers. METHODS: Forty-two patients who underwent unilateral neck dissection were selected. Data obtained from each subject were trapezius muscle volume ratio and a Shoulder Disability Questionnaire (SDQ) score. Patients who had undergone neck dissection with spinal accessory nerve (SAN) preservation were compared with those who had received radical neck dissection. The preservation group was further separated into subgroups b…

Marginal mandibular nerve injury during neck dissection of level IIa, and the influence of different types of dissection: diathermy versus cold knife.

INTRODUCTION: Post-operative injury of the marginal mandibular branch of the facial nerve is consider a complication in neck dissection and can be related to different factors including traction, devascularisation or the conduction block due to diathermy. MATERIAL AND METHODS: Prospective, crossover study including patients treated bilateral selective neck dissection of level IIa during a 12 months-period, was performed to evaluate the efficacy of the Hayes-Martin manoeuvre to prevent nerve injury during the use of cold or monopolar diathermy dissection. RESULTS: 20 patients met the inclusio…

Internal Carotid Artery Course Variation in the Neck Dissection of Oral Cavity Cancer Patients.

Accurate knowledge of cervical vascular anatomy is essential during neck dissection (ND) to prevent serious intraoperative complications. While several variations in the cervical course of the internal carotid artery (ICA) have been documented, an ICA coursing posterior to the internal jugular vein (IJV) remains extremely rare and may increase the risk of vascular injury by altering expected anatomic relationships. Therefore, a thorough preoperative imaging evaluation is critical for a safe surgical procedure. This report presents two oral cavity cancer patients who exhibited an uncommon poste…

A NEW METHOD FOR INTRAOPERATIVE DEFINITIVE DIAGNOSIS OF INADVERTENT PARATHYROIDECTOMY DURING CENTRAL NECK DISSECTION.

BACKGROUND: The most serious complication of central neck dissection (CND) is inadvertent parathyroidectomy. There is no definitive method for intraoperative diagnosis of this complication. METHOD: We studied on CND indicated 17 thyroid cancer patients (14 female, 3 male, age range: 21-67, mean age: 43.4). The excised CND material was kept in 50 ml of normal saline for 30 minutes. A 2 ml sample was taken from this fluid and sent to biochemistry for rapid parathyroid hormone (PTH) measurement. RESULTS: PTH values were <10pg/mL in 14 patients. PTH values of three patients were very high (112pg…

Outcomes of Central Neck Dissection for Papillary Thyroid Carcinoma in Primary Versus Revision Setting.

In recent decades, nanotechnology has emerged as a promising field with diverse medical applications. Silver nanoparticles (Ag NPs) exhibit several biological activities. The aim of the current study was to investigate antioxidant, antimicrobial, and anti-breast cancer properties of green-synthesized Ag NPs. To achieve this, Ag NPs were synthesized using Kocuria sp., and their successful formation and physicochemical properties were evaluated using UV-vis, FTIR, XRD, DLS, zeta potential analysis, SEM, and TEM. The antioxidant capacity of Ag NPs was evaluated using a DPPH scavenging assay. The

Bilateral Chylothorax Following Total Thyroidectomy with Neck Dissection for Papillary Thyroid Cancer: Case report.

Bilateral chylothorax is a rare condition that occurs after a thyroidectomy and neck dissection, which can lead to severe morbidity and, potentially, death, if not managed properly. We report a rare complication of neck surgery and subsequent bilateral pleural effusion in a 35-year-old female patient who presented at Sultan Qaboos University Hospital in 2018 with shortness of breath and respiratory distress. The bilateral pleural effusion and related symptoms occurred one week after a total thyroidectomy with central and left lateral neck dissection. The patient was managed conservatively and …

The necessity of level IIb dissection for clinically negative neck oral squamous cell carcinoma.

AIM: Selective neck dissection reduces the incidence of complications associated with radical neck dissection while achieving the same oncological results, especially in clinically node-negative (cN0) cases. The most common complications associated with selective neck dissection are spinal accessory nerve dysfunction and shoulder disability, which result from level IIb dissection. The aim of the present study was to evaluate the incidence of level IIb lymph node metastasis in cN0 oral squamous cell carcinoma (OSCC) patients to determine the necessity of selective neck surgery. METHODS: The me…

Distal Neo-Neck Formation for Chronic Type B Dissection: False Lumen Closure After TEVAR.

OBJECTIVES:: Patients with chronic aortic dissection often require repeat interventions due to enlargement of the pressurized false lumen or disseminated intravascular coagulation even after additional thoracic endovascular aortic repair (TEVAR) to occlude the entry tear. Residual false lumen flow can persist even after performing the candy-plug technique or branched stent-graft placement in some cases. We have devised a new method for false lumen closure. METHODS:: From December 2010 to May 2017, 5 patients (mean age: 57 [13] years, range: 43-77 years) with chronic dissection at the aortic a

A Limb-Threatening Long Arterial Dissection Caused by Humerus Neck Fracture: A Case Report.

Proximal humerus fracture is a common arm trauma and rarely occurs with vascular injury which however is a serious complication. In this case report, we present a long segment dissection of the axillary and brachial arteries as a rare complication due to fragmented proximal humerus fracture and shoulder dislocation. An 80-year old female patient was seen at the emergency department. Radiograph examination has revealed a fragmented proximal humerus fracture besides dislocation of the head of humerus towards the axillary area. On vascular examination, acute arterial occlusion such as absence of

Esthetic neck dissection using an endoscope via retroauricular incision: a report of two cases.

Various surgical techniques, such as endoscopic surgery and robotic surgery, are developed to optimize the esthetic outcome even in operations for malignancy. A modified face-lift or retroauricular approach are used to minimize postoperative scarring. Recently, robot-assisted surgery is being done in various fields and considered as favorable treatment method by many surgeons. However its high cost is a nonnegligible fraction for many patients. On the other hand, endoscopic surgery, which is cheaper than robotic surgery, is minimally invasive with contentable neck dissection. Although it is a …
