Neuromuscular block by antibiotics: polymyxin B.

Characteristics of neuromuscular block produced by polymyxin B (PXB) were examined in 12 anesthetized cats, using sciatic nervetibialis anticus muscle preparations. The ED50 was 6.7 (+/- 1.4, SD) PXB base/kg body weight. The ED95 was 10.8 (+/- 2.4) mg/kg. Spontaneous recovery from 25 percent of control to 75 percent of control required 72 (+/- 16) minutes. During a 50 percent block, train-of-four twitches elicited at 2 Hz faded with a train-of-four ratio of 0.42 (+/- 0.13), but the tetanus did not fade. Edrophonium Cl, neostigmine methylsulfate, and pyridostigmine Br at sub-clinical dosages we…

Myasthenic crisis-like syndrome due to Cleistanthus collinus poisoning.

Poisoning with Cleistanthus collinus frequently causes cardiac manifestations such as rhythm disturbances and also results in other manifestations such as metabolic acidosis and hypokalemia. We present the case of a patient who presented with a rare myasthenic crisis-like syndrome requiring assisted ventilation due to Cleistanthus collinus poisoning, which responded to treatment with neostigmine.

[Guideline on the safe use of neuromuscular blocking agents].

Inappropriate use of muscle relaxants leads to postoperative residual neuromuscular block (PORNB), which is still a significant problem nowa … rvals during surgery to follow the course of muscle relaxation. The effect of reversal agents (both neostigmine and sugammadex) should always be objectively monitored. The effect of bensilisoquinolin muscle relaxants can only be reversed with neostigmine when there is twitch response to TOF stimulation. Sugammadex is the most effective reversal agent for ster …

Recovery from pancuronium. A comparison between old and young patients.

The rate at which paralysis from pancuronium could be reversed by neostigmine was monitored in two groups of patients, one elderly and the other young adults. Some patients were found to have prolonged recovery times, and this slow reversal occurred more frequently in the older group of patients. Possible reasons for this are discussed.

[Antianaphylactic effects of muscarinic antagonists].

The study was carried out on color guinea pigs, in which anaphylactic shock was induced by sensitizing and challenging doses of normal horse serum (HS). Administration of atropine, ipratropium, or ipratropium in combination with neostigmine led to reduction of sensitizing effect of HS: anaphylactic index was decreased up to (2.2 - 2.4) +/- 0.1 arbitrary units (a.u.). Methacine in combination with neostigmine (0.4 +/- 0.01 a.u.) or atropine in combination with galantamine (1.5 +/- 0.3 a.u.) effectively prevented the pathochemical stage of anaphylactic shock. Changes in plasma protein concentrat…

Effects of adrenergic blockers on central nervous system-mediated hyperglycemia in fed rats.

We studied the effect of adrenergic blockade on hepatic venous hyperglycemia and the activation of a hepatic glycogenolytic enzyme, phosphorylase-a, in response to cerebral cholinergic activation. Neostigmine was injected into the third cerebral ventricle of bilaterally adrenodemedullectomized (ADMX) rats, while somatostatin and insulin were administered intravenously. Hepatic venous plasma glucose concentrations and hepatic phosphorylase-a activity were measured. Intracerebroventricular injection of neostigmine (5 x 10(-8) mol) caused increases in hepatic venous glucose concentrations and hep…

Abnormal colonic motility in progressive systemic sclerosis.

Progressive systemic sclerosis alters smooth muscle function throughout the gastrointestinal tract. In 10 consecutive patients with the disease, colonic spike activity and contractile activity were measured after a 1000-kcal meal, intramuscular injection of neostigmine, or intravenous injection of metoclopramide. The 1000-kcal meal stimulated a significant increase in spike and contractile activity in normal subjects. Nine of the 10 patients had no increase in motility after eating. Neostigmine or metoclopramide stimulated colonic spike (p less than 0.01) and contractile (p less than 0.02) act…

Safety of sugammadex for reversal of neuromuscular block.

Introduction: Sugammadex is a modified cyclodextrin that is able to reverse neuromuscular block induced by aminosteroidal neuromuscular blocking drugs. Compared to reversal with neostigmine, it reverses neuromuscular block quicker and more predictable and without cholinergic side effects. However, there have been concerns about sugammadex ability to bind other drugs and its effects on QT interval and clotting times. In addition, sugammadex might induce hypersensitivity reactions more frequently than initially anticipated. This review summarizes current evidence with regard to these and other s…

Excitatory cholinergic innervation in canine portal and mesenteric veins.

A possibility that the canine portal and mesenteric veins may have an excitatory cholinergic innervation similar to that observed in the middle segment of the inferior vena cava (IVC) was investigated. After adrenergic blockade with prazosin, contractile responses of isolated longitudinal strips of the portal and mesenteric veins to transmural electrical stimulation were greatly diminished. Subsequent anticholinesterase (neostigmine or physostigmine) treatment, however, revealed in the nerve-evoked contraction a component that was antagonized by atropine. The same phenomenon was also observed …

Perioperative efficiency of sugammadex following minimally invasive gastric sleeve surgery: A superiority trial.

BACKGROUND: Studies have proposed that the routine use of sugammadex could provide perioperative time savings and a reduction in the incidence of postoperative nausea and vomiting. The purpose of this study was to test the effectiveness of sugammadex on perioperative times and on the incidences of adverse events when compared with the active control, neostigmine, for minimally invasive gastric sleeve surgery. METHODS: Following institutional review board approval, patient characteristics, type of primary neuromuscular blocking reversal agents, operating room discharge times, post-anaesthesia …

Update on the Diagnosis and Management of Acute Colonic Pseudo-obstruction (ACPO).

PURPOSE OF REVIEW: Acute Colonic Pseudo-obstruction (ACPO) is a cause of large intestinal dilation and obstruction without any physical transition point. It remains difficult to diagnose and treat. We review the recent updates on diagnosis and management of ACPO. RECENT FINDINGS: Recent guidelines have posited that conservative management can be tried in most cases of ACPO, but that early decompression and surgery should be considered. Use of neostigmine is still a viable option but there is also promising data on pyridostigmine as well as prucalopride. Resolution of ACPO should be followed b…

Ocular myasthenia gravis mimicking pseudointernuclear ophthalmoplegia and variable esotropia.

A 70-year-old man with known vascular disease, who had bilateral internuclear ophthalmoplegia, and an 18-month-old girl with a long-term variable esotropia culminating in a postinfectious oculopharyngeal neuropathy both responded with a positive edrophonium chloride (Tensilon) test, strongly suggesting the diagnosis of myasthenia gravis. Both subsequently received maintenance doses of neostigmine methylsulfate (Prostigmin).

Effect of Changing Estimated Glomerular Filtration Rate Formula on Sugammadex Use and Pulmonary Complications for African American and non-African American Patients.

BACKGROUND: Sugammadex is associated with fewer postoperative pulmonary complications than is neostigmine reversal of neuromuscular blockade. However, the Food and Drug Administration-approved package insert states that its use is "not recommended" in severe renal impairment, separately defined as creatinine clearance <30 mL/min. Recently, the formula for estimating glomerular filtration rate (GFR) was updated to remove the race variable. Compared to the prior formula, the new consensus equation lowers the estimated GFR for African American patients and raises it for everyone else. We sought t…

Blockade of nicotinic responses by physostigmine, tacrine and other cholinesterase inhibitors in rat striatum.

1. The acetylcholinesterase inhibitors physostigmine, neostigmine, tetrahydroaminoacridine (tacrine; THA) and diisopropylfluorophosphate (DFP) were tested for possible direct nicotinic actions in rat striatal synaptosomes preloaded with [3H]-dopamine. In this preparation, nicotinic cholinoceptor activation evoked [3H]-dopamine release. 2. Antagonist activity was examined by giving a brief nicotine (1 microM) challenge after 30 min superfusion with an acetylcholinesterase (AChE) inhibitor (0.3-300 microM). Physostigmine, neostigmine and tacrine produced a concentration-dependent blockade. Physo…

Reversal effect of sugammadex sodium on rocuronium-induced neuromuscular blockade in patients undergoing painless tracheoscopy.

OBJECTIVE: To compare the efficacy and safety of sugammadex sodium versus neostigmine for rocuronium-induced neuromuscular blockade during painless tracheoscopy. METHODS: A retrospective analysis was conducted in 278 consecutive patients who underwent painless tracheoscopy under rocuronium anesthesia between January 2023 and May 2024. Patients received either sugammadex (n = 125) or neostigmine (n = 153) for muscle-relaxant reversal. Outcomes evaluated included train-of-four (TOF) recovery times (TOF ≥ 0.25, 0.75, 0.9), complete reversal rates, recovery metrics (eye-opening time, post-anesthe…
