Impact of sampaio pelvicalyceal system variants on the clinical and surgical outcomes of supine mini-percutaneous nephrolithotomy in adults.

Certain anatomical subtypes are associated with more favorable surgical outcomes, highlighting the importance of routine preoperative assessment of the pelvicalyceal system (PCS) in percutaneous nephrolithotomy (PNL). Considering that PNL performed in the supine position can be more technically challenging than in the prone position, the influence of PCS type may be even more pronounced. In this context, we aimed to evaluate the impact of different Sampaio PCS types on the perioperative and postoperative outcomes of supine mini-PNL. Patients aged 18 years or older who underwent supine mini-PNL

Retention of a Double-J Stent Upper Coil in the Renal Parenchyma Following Percutaneous Nephrolithotomy: A Case Report.

Percutaneous nephrolithotomy (PNL) is a standard treatment for large renal stones, and the use of a double-J (JJ) stent is common for postoperative drainage. Here, we report a rare complication involving retention of the upper coil of a JJ stent within the renal parenchyma following PNL. A 47-year-old man underwent left-sided PNL with placement of an antegrade JJ stent and Malecot catheter. Although initial imaging confirmed correct stent positioning, subsequent attempts at stent removal failed. Ureteroscopy revealed that the upper coil was embedded in the renal parenchyma, likely at the site

Predictors of success and complications of monoplanar renal access for conventional prone percutaneous nephrolithotomy Analysis of 662 cases.

INTRODUCTION: Our aim was to assess how monoplanar fluoroscopy-guided access affects the outcomes of percutaneous nephrolithotomy (PCNL). METHODS: This retrospective study included all patients who had renal stones and underwent prone PCNL using monoplanar fluoroscopy-guided access in a single tertiary care center between January 2015 and January 2024. Preoperative and postoperative patient- and procedure-related variables, such as operative time, intraoperative blood loss, number of tracts, complications, stone-free rate (SFR), and hospital stay, were assessed. Multivariable analysis was per

The significance of clinically insignificant residual fragments after percutaneous nephrolithotomy: an analysis into the relevance of complete stone clearance.

PURPOSE: After treatment for kidney stones, residual fragments with a diameter of ≤ 4 mm are traditionally referred to as 'clinically insignificant residual fragments'. We hypothesize that patients with these fragments are at an increased risk for stone-related morbidity, such as complaints, hydronephrosis, and stone regrowth, when compared to stone-free patients. This study aimed to investigate the relevance of complete stone clearance in surgical treatment of urolithiasis. METHODS: We conducted a single-center retrospective cohort study. Patients who underwent percutaneous nephrolithotomy b

Comparison between Retrograde Flexible Ureteroscopy and Percutaneous Nephrolithotomy for the Treatment of Renal Stones of 2-4 cm.

Background and objectives: Renal stones are widespread, with a lifetime prevalence of 10% in adults. Flexible ureteroscopy enables urologists to treat lower calyx stones or even complex renal stones through the natural orifice and achieve an acceptable stone-free rate. Hence, we analyzed the effectiveness and safety of FURS versus PCNL in treating renal stones between 20 and 40 mm in diameter. Materials and methods: We retrospectively analyzed 250 consecutive patients with large renal solitary stones (stone burden between 2 and 4 cm) from 1 January 2019 to 31 December 2020. The patients were d

Development and internal validation of a prediction model to evaluate the risk of severe hemorrhage following mini-percutaneous nephrolithotomy.

PURPOSE: To develop a new prediction model for assessing the severe hemorrhage events in post mini-percutaneous nephrolithotomy (mini-PCNL) patients and internally validate it, thus to guide decision making in clinical practice. METHODS: The patients who underwent mini-PCNL were retrospectively reviewed. Potential risk factors were included as prediction variables for multivariate logistic regression analysis to identify independent risk factors, and prediction model was constructed. The predictive ability of the model was evaluated using the Concordance index (C-index) and Brier score. Boots

Metabolic stone workup abnormalities are not as important as stone culture in patients with recurrent stones undergoing percutaneous nephrolithotomy.

To investigate the association between metabolic urinary abnormalities and urinary tract infection (UTI) and the stone recurrence status in patients undergoing percutaneous nephrolithotomy (PCNL). A prospective evaluation was performed for patients who underwent PCNL between November 2019 and November 2021 and met the inclusion criteria. Patients with previous stone interventions were classified as recurrent stone formers. Before PCNL, a 24 h metabolic stone workup and midstream urine culture (MSU-C) were done. Renal pelvis (RP-C) and stones (S-C) cultures were collected during the procedure.

Beyond complications: the mayo adhesive probability (MAP) score as an imaging biomarker of surgical complexity in percutaneous nephrolithotomy.

The Mayo Adhesive Probability (MAP) score was originally developed to predict adherent perinephric fat in partial nephrectomy. Its role in percutaneous nephrolithotomy (PNL) remains poorly characterized. We aimed to investigate whether MAP score is associated with markers of procedural difficulty and perioperative outcomes in patients undergoing PNL. We retrospectively evaluated 556 consecutive adult patients who underwent PNL between 2020 and 2025. MAP scores were calculated using preoperative non-contrast computed tomography and patients were categorized into low-MAP (< 3) and high-MAP (≥ 3)

Comparative outcomes of ureteroscopy and percutaneous nephrolithotomy in CKD patients with renal calculi: a propensity-matched cohort study.

BACKGROUND: Kidney stones frequently coexist with chronic kidney disease (CKD), sharing common risk factors and leading to adverse outcomes. While percutaneous nephrolithotomy (PCNL) and ureteroscopy (URS) are both effective treatment options, the safety of PCNL has been well-established in CKD patients, whereas the safety profile of URS remains less clear. METHODS: A retrospective cohort study using the TriNetX database was conducted to compare outcomes in CKD patients undergoing URS or PCNL. Patients aged ≥18 years with a diagnosis of renal stones and CKD were included, excluding those on d
