Genetic testing for kidney disease may inform clinical diagnosis, prognosis and therapy, help to guide transplant planning, and advise risk in relatives and family planning. This review provides a summary of the National Kidney Foundation report on testing genetic diseases for patients with chronic kidney disease and individuals who are at risk of chronic kidney disease, including family members and kidney donors, with an emphasis in monogenic disorders and APOL1. We illustrate medical decision-making using case-level interpretation for genetic disorders.

Cholesterol crystal embolism (CCE) is an underdiagnosed systemic condition that often leads to acute kidney injury (AKI) with poor kidney prognosis. Herein, we report a case of dialysis-dependent AKI due to spontaneous CCE with early kidney recovery. RATIONALE & OBJECTIVE: Many nephrology fellows will eventually become dialysis unit medical directors, yet few receive formal training on the responsibilities of this position. This study reports on our 10-year experience conducting a dedicated medical director rotation for nephrology fellows.
STUDY DESIGN: An observational longitudinal study of a month-long medical director rotation designed to prepare nephrology fellows for leadership roles in dialysis facilities.
SETTING & PARTICIPANTS: From 2014 to 2023, 36 nephrology fellows at the Mayo Clinic in Rochester, MN completed the rotation.

METHODS: We used the "Vienna TMA cohort" and the hospital pharmacy database at the Medical University of Vienna to identify patients that received eculizumab treatment between 2012 and 2019. BACKGROUND: Increased ultrasonography (US) use has been correlated with an increased incidence of pediatric renal cysts. For simple and stage II cysts, the malignancy risk is low in adulthood, no follow up is recommended; but there is no consensus on childhood management. Given that pediatric renal cysts may be manifestations of hereditary cystic diseases, a different approach and follow up should be taken for these patients. Cryptococcal infection has been increasing among immunosuppressed population. We report three cases of Cryptococcus neoformans infection in immunosuppressed patients - two renal transplanted and one with lupus nephritis. Early infection (<3months) was diagnosed in two - an allograft Cryptococcus infection and a central nervous system involvement. The third, a 10-year transplant vintage patient, presented with cryptococcal meningitis. Amphotericin B provided good clinical outcomes. We outline the importance of suspicion for cryptococcal infection in immunosuppressed patients.

Our objective was to evaluate our overall experience in pediatric renal transplantation. Between March 1976 and March 2004, 1,600 live-donor kidney transplantations were carried out in our center; 216 of the patients were 18 years old or younger (mean age 12.9 years). Forty-two children, aged 2-21.5 years on hemodialysis with a height below -2.0 standard deviation score (SDS) for age, were selected to receive recombinant human growth hormone (rhGH) therapy at 17 French centers.
