Use of Ultrasound in Patients With Carpal Tunnel Syndrome: A Cost-Effective Solution to Reduce Delays in Surgical Care.

PURPOSE: Currently, electrodiagnostic testing, which comprises electromyogram (EMG) and nerve conduction studies (NCS), is the most commonly used method for confirming the clinical diagnosis of carpal tunnel syndrome (CTS). Electromyogram and NCS can be costly, can require multiple visits, may induce anxiety, and may be painful for patients. The purpose of this study was to determine whether replacing EMG/NCS with ultrasound (US), performed by the treating surgeon, to diagnose CTS decreases time to surgery and the number of office visits. METHODS: We retrospectively reviewed a database that c

Accurate automated diagnosis of carpal tunnel syndrome using radiomics features with ultrasound images: A comparison with radiologists' assessment.

PURPOSE: Ultrasonography is the most common imaging modality used to diagnose carpal tunnel syndrome (CTS). Recently artificial intelligence algorithms have been used to diagnose musculoskeletal diseases accurately without human errors using medical images. In this work, a computer-aided diagnosis (CAD) system is developed using radiomics features extracted from median nerves (MN) to diagnose CTS accurately. METHOD: This study is performed on 228 wrists from 65 patients and 57 controls, with an equal number of control and CTS wrists. Nerve conduction study (NCS) is considered as the gold stan

Paraproteinemia and neuropathy.

Paraproteinemia is associated with different peripheral neuropathies. The major causes of neuropathy correlated with paraproteinemia are the deposition of immunoglobulin in the myelin, represented by anti-myelin-associated glycoprotein (MAG) neuropathy; deposition of immunoglobulin or its fragment in the interstitium, represented by immunoglobulin light chain amyloidosis (AL amyloidosis); and paraneoplastic mechanisms that cannot be solely attributed to the deposition of immunoglobulin or its fragment, represented by polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin

Pudendal Nerve Block by Low-Frequency (≤1 kHz) Biphasic Electrical Stimulation.

OBJECTIVES: To test the hypothesis that poststimulation block of nerve conduction can be achieved by low-frequency (≤1 kHz) biphasic stimulation (LFBS). MATERIALS AND METHODS: A tripolar cuff electrode was placed around the pudendal nerve in cats to deliver LFBS (1 kHz, 500 Hz, and 100 Hz). Two bipolar hook electrodes were placed central and distal to the cuff electrode to induce external urethral sphincter (EUS) contractions. A catheter was inserted into the urethra to record EUS contraction pressure. Pudendal nerve block by LFBS was confirmed by the failure of the central hook electrode sti

Al-hijamah (wet cupping therapy of prophetic medicine) as a novel alternative to surgery for carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is the most common neuropathy of median nerve causing decreased physical and work performance. Herein, a 37-year-old male manual worker diagnosed with severe CTS exhibited severe pain with frequent awakening from night sleep to put hands in ice. Patients consent and ethical guidelines were carried out. As a novel approach, Al-hijamah was performed to both hands at the anterior and posterior carpal regions (using scarification safety technique) and at the back region. Immediately after Al-hijamah, a dramatic decrease in pain, numbness and parathesia occurred. Nerve

Bilateral Bifid Median Nerve with Bilateral Carpal Tunnel Syndrome: An Atypical Presentation, a Rare Cause and a Familiar Disease.

We report a case of bilateral carpal tunnel syndrome (CTS) with bilateral bifid median nerve (BMN) without a persistent median artery diagnosed by clinical examination, NCV and Ultrasonography. Bilateral BMN to produce bilateral CTS is extremely rare and is contrary to the popular classifications which state that BMN without persistent median artery or its anomalies will not produce CTS. Ultrasonogram is of additional help to identify such aberrations in the median nerve to prevent iatrogenic injuries during carpal tunnel decompression.

Peripheral neuropathy in Tangier disease: A literature review and assessment.

Tangier disease (TD) (OMIM#205400) is a rare cause of inherited metabolic neuropathies characterized by marked deficiency of high-density lipoproteins and accumulation of cholesterol esters in various tissue resulting from reverse cholesterol transport deficiency. We report a case of a patient with TD with multifocal demyelinating neuropathy with conduction block who presents with winging scapula, tongue, and asymmetric extremity weakness. We also present a review of all studies published from 1960 to 2017 regarding peripheral neuropathy in TD. Our search identified 54 patients with TD with pe

Nerve ultrasound characterizes AMN polyneuropathy as inhomogeneous and focal hypertrophic.

OBJECTIVE: High-resolution nerve ultrasound (HRUS) is a painless tool to quickly evaluate peripheral nerve morphology in vivo. This study set out to characterize peripheral nerve involvement in X-linked adrenomyeloneuropathy (AMN) by HRUS. METHODS: Thirteen adults with genetically proven AMN were examined using the Ultrasound pattern sum score (UPSS) to evaluate morphological abnormalities of peripheral nerves, vagal nerves, as well as cervical nerve roots. Ultrasound results were correlated with clinical findings and nerve conduction studies. RESULTS: UPSS was increased in six out of 13 pat

A case report: Non-alcoholic Wernicke encephalopathy associated with polyneuropathy.

We report a rare case of non-alcoholic Wernicke encephalopathy (WE) with polyneuropathy. A 24-year-old woman who had recently served a 4-month prison sentence and underwent a short period of dieting manifested slow response, weakness, language disorder and amnesia. Brain magnetic resonance imaging (MRI) revealed typical lesions of WE. Examination of nerve conduction velocity revealed sensory-motor axonal polyneuropathy. The patient was immediately treated with thiamine. Neurological symptoms were alleviated in a few days and abnormal signals were markedly decreased in a follow-up MRI 1 week la

Bifacial weakness with paresthesias: Serial nerve conduction studies indicate diffuse demyelinating neuropathy.

INTRODUCTION: Bifacial weakness with paresthesias is a rare subtype of Guillain-Barré syndrome (GBS), characterized by facial diplegia in the absence of any other cranial neuropathies, limb weakness, or ataxia. Frequently, patients also complain of distal limb paresthesias before or at the time they develop facial weakness. METHODS: We describe a man who developed post-infective isolated symmetric facial diplegia associated with distal paresthesias. Nerve conduction studies were conducted at 4 time-points over 6 months. RESULTS: A monophasic disease course and presence of cerebrospinal fluid

GEHS Neurophysiological Classification System for Patients with Carpal Tunnel Syndrome.

BACKGROUND: Median neuropathy at or distal to the wrist or carpal tunnel syndrome (CTS) is one of a number of muscle, tendon, and nerve-related disorders that affect people performing intensive work with their hands. Following a thorough history and physical examination, electrophysiological examination including both nerve conduction studies (NCS) and electromyography (EMG) testing may be performed and currently serve as the reference standard for the diagnosis of CTS. The EMG and NCS exams should identify the peripheral nerve, specific location in the nerve pathway, involvement of sensory an

[Molecular Mimicry and Guillain-Barré Syndrome].

One-thirds of patients develop Guillain-Barré syndrome subsequent to Campylobacter jejuni enteritis. Molecular mimicry exists between C. jejuni lipo-oligosaccharides and human peripheral nerve gangliosides GM1 and GD1a. IgG antibodies against GM1 or GD1a are produced in one out of 5,000 patients with C. jejuni enteritis. The autoantibodies bind to gangliosides at the nodes of Ranvier in the peripheral motor nerves and activate complement in situ. This is followed by the disappearance of the voltage-gated sodium channel clusters at the nodes and disruption of axo-glial junctions at the paranode

[Fibromyalgia syndrome: A disease of the small nerve fibers?].

Fibromyalgia syndrome (FMS) is characterized by chronic widespread pain and additional associated symptoms, such as fatigue, sleep disturbances and depressive moods. The pathophysiology of pain in FMS is unclear. In recent years, an involvement of the thinly myelinated A-delta and the unmyelinated C-nerve fibers has been reported in FMS patients. Independent research groups published consistent objective and multidimensional findings of damage to these small nerve fibers, such as disturbances of fiber function, electrical properties and morphological changes. All these alterations are not spec

Idiopathic bilateral carpal tunnel syndrome in a 9-month-old infant presenting as a pseudo-dystonia.

BACKGROUND: Carpal tunnel syndrome is the most common focal peripheral neuropathy seen in most electrophysiological laboratories. Although the incidence of carpal tunnel syndrome in adults is 50 to 150 cases per 100,000 people, it is rare in children. There are less than 200 case reports of carpal tunnel syndrome in children, with mucopolysaccharides and mucolipidosis being the most frequent cause. Idiopathic carpal tunnel syndrome with childhood onset occurs in less than 0.2% of cases. PATIENT: We describe a 9-month-old infant who presented with intermittent abnormal posturing movement of bo

Morphometric parameters of peripheral nerves in calves correlated with conduction velocity.

BACKGROUND: Peripheral nerve injuries are the most frequent neurologic disorder in cattle. So far, no physiologic values have been established for the motor nerve conduction velocity (mNCV) in this precocial species. OBJECTIVES: The electrophysiologic and morphometric reference values of peripheral nerves in calves were determined. It was hypothesized that these parameters would correlate to the high degree of maturity in the first days of life in this species compared to other species. ANIMALS: Twenty-six healthy calves were used in this study. METHODS: The mNCV of the radial and the sciat

Guillain-Barré syndrome in Asia.

Over the past 20 years, the most notable advance in understanding Guillain-Barré syndrome (GBS) has been the identification of an axonal variant. This advance arose chiefly through studies undertaken in East Asian countries and comprised two major aspects: first, the immunopathogenesis of axonal GBS related to anti-ganglioside antibodies and molecular mimicry of Campylobacter jejuni; and second, the observation that distinct electrophysiological patterns of axonal GBS existed, reflecting reversible conduction failure (RCF). As a consequence, the pathophysiology of acute motor axonal neuropathy
